PALLIATIVE CARE

Offers a support system to help the family cope during the patient’s illness and in their own bereavement

Is applicable early in the course of illness, in conjunction with other therapies that are intended to therapy, or antiretroviral therapy and includes those prolong life, such as chemotherapy or radiation investigations needed to better understand and manage distressing clinical complications

CCW Provide Access to Care

Palliative components gives special care for the impoverished and under served with emphasis on those falling through the cracks

Patient identification, support, referrals, education….

Inter-disciplinary TeamCCW as PART of a team (Dr,

Nurses, Social workers, OT, physio, spiritual, etc)

More than multi - patient moves between all as required

A professional nurse supervises HBC delivered by trained CCW, caregivers and volunteers

Pain and Symptom Control

Is the core of CCW workCCW taught to use a specific tool

to categorize for referral and support

High quality training essentialCultural norms are respected

Holistic Care

Patients AND their families have access to physical, psycho-social and spiritual care

Pediatric Palliative care

Appropriate plans are developed and initiated with regard to the care of children

Bereavement Care

The bereaved are supported during the process of mourning and their adjustment to loss

Care of Caregivers
