Allegation Summary:#1. On AFH tour observed Named Resident (NR) in the home. Two resident said they are well cared for and had not fallen in thehome.Provider said NR fell in afternoon while trying to get up from couch by self. Provider called 911 and NR went to hospital forevaluation. NR returned to the AFH after hospital visit.Record showed NCP was not updated with fall risk. Record showed NCP was last updated 02-2016. Record showed the fall wasreported. Record showed staff credentials current.
