Pretibial Myxedema with Graves' disease.

30 year old male. 2 year history of Grave´s disease. 1 year history of plaques involving the

pretibial region of the lower legs.

Pretibial Myxedema with Graves' disease.

Dermatological examination revealed well defined indurated plaques, mildly erythematous, lobulated surface, with loss of hair on anterior aspect.

Pretibial Myxedema with Graves' disease.

A punch biopsy revealed hyperkeratosis, epidermal hyperplasia and edema located throughout the papillary dermis.

This stained positive for mucin with Alcian blue.

Hematoxylin and eosin stain shows mucin accumulation causing separation and fraying of collagen fibrils.

Pretibial Myxedema with Graves' disease.

Patient was treated with topical corticosteroids under occlusion.

The applied treatment was nighttime dressing of 0.05% betamethasone dipropionate in cream base under occlusion with plastic film.
