consent for GP letter received & passed to
Tele-Interview er.
Clinical AssessmentRecord clinical information on
database and Action Plan. Client to give
w ritten consent at initial appointment.Treatment
Commences.
Case ManagerCase Manager contacts client
follow ing initial appt and once consent from received to
review case.
Case ConferenceCase Conference (led
by Case Manager) w ith clinicians
involved.
Treatment Concluded
At conclusion of treatment, Case Manager to be
informed so discharge procedures can
commence.
DischargeDischarge
questionaire, COPM, EQ5D & GHQ12 to be repeated. Database
updated.
Satisfaction Questionaire/GP Discharge Leter
Tele-Interview er w ill send GP letter informing of discharge. Satisfaction
questionaire sent to client/employer.
