A little over ten years ago, a community came together to help a free clinic get off the ground. A humble country church used their resources to help their neighbors. Not only were
they an integral part of launching a sustained and vital effort for health in our community, but they brought people together
around a mission, to unleash the full potential of their community. I can’t think of a more healing act.
Underneath the policy, payment and politics, health is about making connections to support what we know is best for us. Whether it is cooking with your family and friends, socializing in neighborhood spaces, or uniting to pray, study or play, health happens every day and is built through connections with others.
At a time when connecting with those around us has never been more urgent, HFC’s model of community-engaged care not only provides vital health access, but also breaks down barriers entrenched in our society
and culture. Our history has showed us how connecting with those around us can be a learning and healing act.
We are collaborators by name and nature, and wouldn’t be here without your interest, engagement and support.
You are pushing our unique models of care beyond the clinic walls, and we couldn’t do it without you. Your support, in all its ways, empowers communities to be healthy in the way they know best.
For this reason and so many more, we will always be a church basement organization. As we move on to our next phase, we are forever grateful for the unsolicited actions of the Little Prairie community who provided support vital to our mission, and a decade that will forever live on in our character.

stablished in 2005, HealthFinders is a growing nonprofit in the dynamic field of community health care. We believe that health happens where we
live. By combining clinical care with community organizing, collaboration and systems change, HealthFinders staff work to establish healthy and equitable communities.
HealthFinders plays a unique role as a community-embedded health center providing a comprehensive access point to health services for families in Rice County. Through primary care, medication assistance, education and patient advocacy, HFC works across the continuum of wellness to collaborate and engage communities in their own health. Our staff works within the local community not only to identify health issues in their neighborhoods and lead implementation of programs and services but also to provide leadership and facilitate neighborhood-driven health initiatives.

BUILDING COMMUNITY
In 2005, HealthFinders opened its doors for Rice County’s uninsured in the
basement of the Little Prairie United Methodist Church in rural Dundas.
Parishioners, friends and community supporters converted a former kitchen
in the church basement into a professional and welcoming clinic setting for
HealthFinders patients. Now, after 11 years, HealthFinders has outgrown its
Little Prairie home and moved to Northfield. We would not be where we are today
without the foundation that Little Prairie church gave HealthFinders. Little Prairie has
made us who we are and will forever be a part of our history.

HealthFinders’ dental program has

wo HealthFinders CLINICS are staffed by wonderful
volunteer providers, nurses, interpreters and receptionists. In-kind lab and imaging are provided by Allina Health, Mayo Health System and Northfield Hospital & Clinics.
The MEDICATION ASSISTANCE PROGRAM (MAP) provides prescription medications free of charge to residents of Rice County. Sterling Drug partners with us to create a unique community pharmacy offering services such as prescription delivery.
DENTAL SERVICES are provided at our Faribault clinic by volunteer providers, a staff dental coordinator and hygienist, and a dental assistant. No patient is turned away for inability to pay.
HealthFinders’ PATIENT ADVOCATES are certified MNsure Navigators. They provide assistance with MNsure applications and enrollment. They also work to connect patients and families with other public healthcare programs for which they are eligible.
Our PURA VIDA program offers healthy living, exercise and nutrition programs for adults plus summertime physical activity classes and events for children. Childcare is available and all the classes are free and open to families residing in Rice County.
HealthFinders’ DIABETES PROGRAM offers patients living with diabetes extra support with their chronic disease. Support groups and classes include sessions on eating for diabetes control, blood glucose testing, making the most of your
medicine, and exercising for your health.
Our MESA (Mejorando la Salud de los Adolescente) program is a collaborative project funded through the Minnesota Department of Health’s Eliminating Health Disparities Grant Program. The goal of MESA, which translates to “improving adolescent health,” is to promote teen and family connections through school, medical providers and supportive adults in a variety of education and outreach programs.

served by volunteer providers, nurses and interpreters, including Dr. Evans, above, one of the first volunteer providers at HealthFinders’ clinic.

HEALTHFINDERS COLLABORATIVE
ealthFinders was selected as one of the three organizations in Minnesota to receive a grant through Medtronic Foundation’s HealthRise
program. The goal of HealthRise is to improve the health of underserved populations living with cardiovascular disease and/or diabetes.
The HealthRise project supports and extends HFC’s care coordination program connecting patients with a broad range of community wellness resources and community health workers. To do so, HFC utilizes a coordinated care hub to connect its patients to these community resources, in partnership with Mayo Health System, Northfield Hospital & Clinics, District One Hospital, Allina Health Clinics and Rice County Public Health, to bring coordinated care to patients across the county.
Blending community and clinical resources is at the heart of the HealthRise project. Patients will receive clinical care, assistance in accessing and attending wellness programs, as well as coordination with their clinical team.
Charlie Mandile, HFC’s Executive Director, said, “It is exciting to unite healthcare providers across Rice County around one goal. This project works to expand the traditional definition of healthcare teams, engaging communities right where health is happening.”

BEYOND THE CLINIC
CARE COORDINATIONCare coordination is now an essential component of HealthFinders and is part
of a broad effort in the U.S. to improve heathcare outcomes in underserved
populations. In this model, the patient is at the ‘hub’ with directed individualized
support for connecting to a variety of chronic disease management and wellness
programs, clinical care, and community services. HFC’s care coordination program
works to address issues and barriers that might occur for the patient and coordinate
their care across HFC’s medical, wellness and community-based programming
alongside healthcare providers, community partners and other entities.

Juliana moved to Northfield from São Paulo, Brazil. Originally
trained in electrical technology, Juliana became interested in
wellness by taking charge of her own health, and soon became
certified as a personal trainer and Zumba instructor.
As Pura Vida Program Lead, Juliana works to engage the
community in exercise and wellness, and hopes to use her own
journey back to a healthier lifestyle to inspire others.
She says, “Exercise can be boring without social support, so
when people are engaged in a group and having fun, you can
really impact people’s willingness to stay with the program.”
She is currently creating programs that will help community
members stay with healthy options year-round, and is expanding
Pura Vida to include nutrition and other wellness topics in
addition to exercise.
