Dr.  Vince  Kokich  ~  Special  Offering    Treating  the  “Damaged”  Incisor:  Forced  Eruption,  Decoronation,  or  Controlled  Extraction?      Patients  will  occasionally  have  accidents  that  traumatize  their  maxillary  incisors.  

In  some  cases  a  traumatic  blow  will  result  in  a  subgingival  root  fracture.  When  this  occurs,  the  dental  team  must  choose  the  appropriate  treatment  plan.  If  the  root  was  fractured,  is  root  canal  therapy  always  necessary?  Could  a  tooth  with  a  fractured  root  be  moved  orthodontically?  If  the  fracture  results  in  separation  of  the  crown  from  the  root,  could  the  root  be  erupted  orthodontically  and  restored?  What  parameters  should  the  team  evaluate  prior  to  forced  eruption?  If  forced  eruption  were  inappropriate,  and  an  implant  is  planned  for  the  site,  should  decoronation  be  attempted  to  preserve  the  bone  for  the  eventual  implant?  If  so,  when  and  how?    If  decoronation  were  not  advisable,  could  orthodontic  controlled  extraction  be  used  to  develop  the  implant  site?    What  are  the  indications  for  controlled  extraction?    Does  it  always  work?    In  this  in-­‐depth  web-­‐based  seminar  Dr.  Vincent  Kokich,  Sr.  will  use  several  clinical  examples  to  answer  all  of  these  important  questions.    At  the  conclusion  of  this  course,  attendees  will  be  able  to:    

1. Learn  how  to  diagnose  which  root  fractures  have  a  favorable  prognosis.  2. Understand  the  six  criteria  that  favor  forced  eruption  and  restoration.  3. Know  whether  the  eruption  should  be  rapid  or  slow.    4. Recognize  when  decoronation  is  an  appropriate  solution  to  preserve  bone  5. Learn  the  four  criteria  that  favor  controlled  extraction  for  site  development

Dr.  Kokich  is  a  Professor  in  the  Department  of  Orthodontics  at  the  University  of  Washington  in  Seattle.  He  also  maintains  a  private  orthodontic  practice  in  Tacoma,  Washington.  He  has  published  18  book  chapters,  67  scientific  articles,  and  48  review  articles.  He  has  given  over  650  presentations  around  the  world.  He  was  awarded  the  Sicher  Research  Award  by  the  American  Association  of  Orthodontists  and  has  also  received  the  Strang  Award,  the  Schluger  Award  the  AAED  President’s  Award  for  Teaching  Excellence,  and  the  Distinguished  Alumnus  Award  from  the  University  of  Washington  School  of  Dentistry.  Dr.  Kokich  has  been  elected  to  Fellowship  in  both  the  American  College  of  Dentists  and  the  Royal  College  of  Surgeons  of  England.  Dr.  Kokich  is  Editor  of  Case  Reports  for  the  American  Journal  of  Orthodontics  and  Dentofacial  Orthopedics,  Associate  Editor  of  The  Angle  Orthodontist,  Associate  Editor  of  Practical  Reviews  in  Orthodontics,  and  serves  on  the  Advisory  Board  of  the  Journal  of  Esthetic  Dentistry,  and  the  Editorial  Boards  of  Seminars  in  Orthodontics,  Clinical  Orthodontics  and  Research,  Dental  Traumatology,  The  Australian  Orthodontic  Journal,  and  The  British  Journal  of  Orthodontics.  He  has  served  as  President  of  both  the  American  Academy  of  Esthetic  Dentistry  and  the  American  Board  of  Orthodontics.
