ObjectivesfromtheAbstract:

Identifylearningresourcesabouttreatmentplanningappropriateformedicalphysicists.Discusscompetenciesintreatmentplanning,whichincludeinterpretationofthephysician’sprescription,rigidanddeformableimageregistration,contouringguidelinesanddosetolerancesfornormaltissuestructures,planningtechniquesappropriateforcommontreatmenttypesandavoidanceregions,andmanagementofplanobjectivesusedininverseplanning.

In2016theAAPMSubcommitteeonGuidelinesforCompetencyEvaluationforClinicalMedicalPhysicistinRadiationOncologypublishedtheirrecommendationsintheJACMP.Thedocumentlaysoutaframeworkforperformingongoingcompetencyassessment.

Evaluationofthesecompetenciescanbedonethroughauditsofworkproduct,suchastheplanPDF.

“Competencytestsshouldemphasizeknowledge,skills,andbehavior.Aswithcredentialing,thismustbedocumented.Thetypeandfrequencyofeachcompetencymustbedetailed.Thefrequencyshouldbedeterminedbyeachfacilitybutshouldbeatleastonceperyear.”

Thedocumentcontainsasamplelistofcompetencyassessmentfrequencies.Thisexamplelistcontainstherecommendationthatmedicalphysicistsshouldperformaminimumoftwelvegeneraltreatmentplanseachyeartomaintaintheircompetency.Thismayseemexcessive,butconsideringtherangeofdifferenttypesoftreatmentplanswerevieweachyear,itisreasonablebefamiliarwiththestepsinvolvedincreatingthesedifferentplancategories.

Thesamplelistalsorecommendsthatamedicalphysicistshouldcompletefourspecialproceduretreatmentplanseachyear.Itwouldbeusefultostorethesetwosamplerecommendationsinyourshorttermmemory.

Theknowledgecategorieslistedhereareintegraltotreatmentplanningandthequalifiedmedicalphysicistshouldhaveadequatecompetenceinthem.

Backinthe1990s,apopularreferencefornormaltissuetolerancesforconventionaltherapywaspublishedbyDr.Emami.WiththeadventofIMRTandadvancementsinmodeling,theQUANTECprojectundertookasystematicreviewofthedataandpublishedtheirresultsin2010intheRedJournal.TheirtolerancesaresummarizedinaWikibookspage.ForSBRTtreatments,TG-101isausefulresourcewhichusesresultspublishedearlierbyDr.Timmermanandothers.Itisusefultoconverthypofractionationdosesto2Gy/fxequivalentdosesusingtheEQD2equationformakingcompositedoseassessments.
