Personal Reflections – key drivers to safer operations
• Planning – work the plan and drive out unplanned work
• Operations – operate as per design – don’t overstress to squeeze incremental performance
• Leadership engagement – leaders at all levels are fully bought in and will “walk the talk” if required
• Leadership engagement 2 – well informed leaders carry out frequent and meaningful front line visits

• Incentivise desired behaviours – e.g. “stop the job heroes”
• Carry out After Action Reviews – why did it go well – what can be improved – encourage and incentivise continuous learning
• Good quality RCFA’s, 5 Why and Incident Investigations – dedicated investigators who turn incidents into case studies
• Contractor partnerships – what are the underlying incentives for performance?

Human Factors
• Resilience Engineering – people will make mistakes – how can we make the system tolerant of mistakes
• “Speak-up” and “Stop the job” is anti-social or at least alien to social survival
• Training and realistic drills to get the outcome required – think military
• Often we make it easy for the execs and hard for the front line – it should be the other way around
