RESPONSIBILITIES…

Communication through routine meetings with other allied healthcare professionals

Going to the wards to monitor patients’ response to the medication

Checking of the medication

Providing education and counseling to the patients

Involved in prescribing by assisting the physician in selecting appropriate dosage regimen for the patient

Having the authority in checking the accuracy of doses, side effects of drugs and its calculations

Extemporaneous preparation of medication

Ensures best medication to manage illness and promote good health

Ensures the medication is correct upon discharge

Generating drug orders

HOSPITAL PHARMACY: HERE AND ABROAD

Differences: The pharmacists abroad are more

exposed to the patients compared to the pharmacists here in our country.

Pharmacists abroad have routine meetings with other allied health professionals while in our country, the doctors usually talk with the pharmacists when they are in need of them.

SUGGESTIONS… There should be a face-to-face

relationship or interaction between the pharmacist and the patient. A pharmacist should be available at the bedside or near the bedside.

The pharmacists should take the drug history of the patient upon admission.

The pharmacists should also be involved in talking with the patient upon discharge and make sure that the medications are appropriate.
