Introduction Provided for your reference is a contact list of funded Division of Health Wellness, and Disease

Control grantees for the calendar year 2015 that provide some level of Partner Services (PS)

intervention. We encourage agencies to use this tool as a reference to help facilitate

communication and collaboration between working entities to promote PS delivery for persons

diagnosed as having HIV or another Sexually Transmitted Disease. This tool provides information

that allows for direct contact with Community-Based Agency staffing, Local Health Department

HIV/STD PS and Counseling, Testing and Referral (CTR) staff members as well as Division

Program Consultants, Technical Coordinators, and Care Consultants. For ease of use, contact

information is divided into sections.

The first section of the contact list provides information on funded Community-based agencies and

their Executive Directors, Program Coordinators and Technical Coordinators. The second section

is a list of HIV Partner Services and CTR representatives from Local Health Departments.

The third section of the contact list contains information on funded agencies that provide direct

care services, names of their Executive Directors, and Division Program Coordinators. The forth

section is a list of STD Disease Intervention Specialist (DIS) their Managers and Outreach

Specialist. This team is primarily responsible for providing various levels of STD PS prevention

services for syphilis, gonorrhea and/or chlamydia infection within their regional health

jurisdictions. Some exceptions to this process do apply where these team members also provide PS

for HIV. Where applicable, these instances are highlighted by a double asterisk (**).

We ask that you inform our office of any contact changes so that we can keep this list current.
