MISSION STATEMENT

Promote and facilitate programs that support the maintenance of good

health as well as the elimination of chronic health disparities in

communities of color through education, health advocacy, and

optimal utilization of health resources.

• A difference in health outcomes across of the population. Health disparities are linked to social, economic or environmental disadvantages.

• An example is shortened life expectancy or earlier death from poorly treated chronic disease.
