Shock, horror: I started as a medical student at a ‘premiere’ medical school in 1978; I am a member of the RCGP; we teach medical students; before Nov 2014 I had not heard of ‘the science of improvement’ or ‘continuous improvement’ in healthcare. My local hospital, a teaching hospital and ‘centre of excellence’, makes the same mistakes over and over. Why?
