How to Request Official Transcripts

Download a Senior Transcript Request form

Student Services website or pick up from the

transcript office (D107) in Student Services

Complete the form and return it to the

transcript office: Form must be signed by parent

Applicable Fee(s) must accompany the request before it will be

processed

This form will be maintained in the transcript office, available for

future request updates

Office hours: 9:30 – 3:00 Monday - Friday

Official transcripts are mailed by SCHS and must

be requested 15 business days (approx. 3 weeks) prior to the college’s deadline ($5.00 late fee)

Three transcripts are free, each additional transcript

request requires a fee of $5.00

Seventh semester grades will be automatically sent

in February 2019 to all applied colleges

SCHS does not send SAT, ACT, or AP scores
