be signposted to a “community resource”.
• What type of resource is most in need?
• How acceptable is the recommendation to be signposted?
• How easy is it for GP practices to collect and use the information?

with GP or Nurse required signposting to Community
resources.
Approx 70 - 80% of recommendations
were then made by GP or Nurse.
50-60% of people accepted the
recommendation to the resource.

• Key Learning Points?• All practices now aware of large range of support
resources in their local areas and are enthusiastic to use them.
• Most practices have taken time to visit the resources or invite them into the practice.
• Improved relationships and knowledge will increase the use of these resources.
• Time is required to develop and update the practice directories of resources, how can this be sustained.

• How can these links between GP practices and local resources be strengthened?
• Education role for Practice Staff?• Provide Link workers across larger areas?• Provide ability for community resources to make
their information more readily available to GP practices via……………ALISS ?
