• Nyahururu hospital is a tier 4 hospital in Laikipia
West sub county ,Laikipia County in Rift valley
• The hospital serves Laikipia Nyandarua and Nakuru
counties with a catchment population of 41,053
persons
• HIV counseling and testing services are fully offered
to mothers admitted in the maternity with no HIV test
done, due for a retest

• This also targets the spouses/partners of the index
client
• There is a good referral and linkage system between
the maternity and the PMTCT clinic in the facility
where the HIV positive clients and the HEI are
followed up

www.chskenya.org BACKGROUND• Over 90% of pediatric HIV infections are from mother
to child transmission
• Transmission occurs during the ANC, labour and
delivery and during the breastfeeding period
• The WHO among many interventions to prevent
MTCT of HIV also recommend HIV retesting of
previous HIV negative pregnant women in the 3rd
trimester, during labour and delivery and the breast-
feeding period

• This strategy is meant to pick out new HIV infections
that may have occurred which offers an opportunity to
provide life saving interventions through PMTCT for
mother and baby that is geared towards elimination of
mother to child transmission of HIV

METHODS AND MATERIALS• A retrospective review of health records was done on
MOH registers (405,406 and 333) and compared to
the reporting tools on the MOH (711 and 731) were
reviewed between June 2014 to May 2015
• The index client was a pregnant woman admitted in
the maternity during the review period

• A Total of 3975 were admitted during June 2014 and
May 2015, 2506 clients were due for a HIV re-test
and were retested, 27 new HIV infections of which 24
were women and 3 partners were identified
• There was 100% referral and linkage to the PMTCT
clinic within the facility and interventions as per the
new guidelines were provided to the mothers,
partners and HIV exposed infants

• Testing of new mothers and re-testing during 3rd
trimester, labour and delivery and postnatal is
important in elimination of mother to child
transmission of HIV (eMTCT)

• Institutions and stakeholders should maximize on
re-testing of pregnant and breast feeding clients as
an important strategy in elimination of mother to
child transmission
RECOMMENDATION

• 27(1.08%) Tested HIV positive and were provided
with interventions and enrolled for further follow-up
• Discordant couples were also enrolled into care and
are on follow-up
• HIV positive Mothers, partners and HIV exposed
infants were linked and enrolled in the integrated
eMTCT clinic for the care and follow-up
