screenings saVe liVes. It is important for men and women ages 50 and older to have colon cancer screenings. These tests find precancerous polyps so they can be removed before they turn into cancer. The Parish Nursing Program promotes senior independence while caring for individual health needs. The program’s intention is to promote health and well-being to individuals who have decreased access to socialization, peer interaction and family relationships. Programming includes health education, counseling and support, health care management, health assessment, observation and referrals to care, loss and bereavement support groups, and Starquest Transport. The number of parishioners we serve grew from three to five targeted churches. As the homeless population continues to grow in Broward County, Parish Nurse Services has expanded to serve homeless adults at a day shelter program and a church.
In 2012, the Parish Nursing Program served 2,411 participants.

Control MontH
The Mobile Health Unit has been providing health care in our community since 2009 and is dedicated to expanding access to health services for the homeless and minority populations in targeted geographic zones. Services onboard the van include rapid HIV and OraSure testing, health exams by a doctor or advanced registered nurse-practitioner (ARNP), health screenings (cholesterol, diabetes, blood pressure and pregnancy), care management, linkage to health care services, childhood immunizations and flu vaccine administration.
Staffing onboard varies depending on the type of services being performed. Holy Cross Medical Group physicians, an ARNP, Community Outreach nurses and coordinators all provide the various health services described above. The Mobile Health Unit operates up to six days per week.
In 2012, the Mobile Health Unit served 1,823 individuals.

The Growing Healthy Kids program is dedicated to the health and well-being of our underserved youth. The services provided included: » Tools for teachers, students and parent
volunteers to help improve nutrition and physical activity.
» Free vision, hearing and scoliosis screenings; childhood vaccinations and health education classes.
» On-site produce gardens and fruit-bearing trees that were planted, cultivated and harvested.
» Free healthy lunches to those eligible—the program is a National School Lunch Sponsor.
» A Pilates for Girls program to improve the mental and physical health of students and their teachers.

Our response to our community’s homelessness issue included: » The placement of a Parish Nurse at a homeless
day shelter and church-based homeless feeding program
» The placement of Outreach screening services including breast health, blood pressure, blood sugar, cholesterol and HIV testing as well as linkages to the care system for those who required additional follow-up services
» The remodeling of a Wellness Center with homeless service partner Hope South Florida that was intended to provide on-site health care services and education for homeless families who entered the homeless system
In 2012, Holy Cross served 5,044 homeless individuals.
In response to our community’s cries of hunger, Holy Cross Hospital assists in the collection, distribution, donation, facilitation and/or provision of food to small food pantries in Broward County. Every day, Associates contribute dry food items, canned goods and change in the cafeteria and Heart Healthy Café.
In 2012, Holy Cross distributed more than 3 tons of food to Broward’s food pantries.

The School Health Program provides access to school-based vision, hearing, immunization and scoliosis screenings; health education classes and immunizations for thousands of students. In addition, classes on CPR, blood-borne pathogens and student-related health concerns (diabetes and HIV/AIDS, for example) are made available to school personnel. The program also provides health education in nontraditional school settings, such as the Broward County Juvenile Detention Center.

The Goodstart Program provides health education and wellness promotion to at-risk preschool students at their daycare facility. In addition, monthly caregiver workshops are presented to educate providers on children’s health issues. At these workshops, caregivers obtain the necessary contact hours in order to maintain licensure and good standing for employment.
In partnership with Impact Broward, Goodstart presents monthly in-service for seniors working with young children. In addition, a partnership with the Jan Moran Collier City Learning Library promotes health literacy through a monthly book club. The combined efforts of this multigenerational program yield thousands of hours of essential health and wellness education.

The Partners in Breast Health (PIBH) program aims to increase access to breast health services in underserved communities throughout Broward County. The hope is to save lives by reducing barriers and providing education and early detection. A mobile medical health unit visited communities in central and northern Broward County that have the highest percentage of poor-quality health indicators. A physician and/or an advanced registered nurse-practitioner provided one-to-one education, general
health assessments, clinical breast exams and mammogram prescriptions for women ages 40 and older. Follow-up care was given to assist patients with diagnostics, treatment and special needs. PIBH receives private and grant funding for screening services, and eligible, approved clients receive treatment and care via the financial assistance program.
In 2012, PIBH provided 996 screenings (mammograms, ultrasounds, magnetic resonance images [MRIs] and biopsies).

Holy Cross Hospital is committed to the well-being of our Associates and their families. With contributions to the Sister Catherine McAuley Fund and direct donations to our Community Outreach program, Holy Cross assists our own Associates with various initiatives throughout the year. The Back to School Backpack and School Supplies Drive help those Associates who have difficulty getting their children (PK4 through high school) prepared for school. In addition, the Thanksgiving Basket Drive ensures that our Associates are able to celebrate a fruitful holiday with their families. At Christmastime, the Angel Tree offers help for our Associates who have difficulty providing Christmas gifts to their children due to economic hardship.

Since 2008, Holy Cross Hospital has provided easy access to rapid HIV testing to our community aboard the Mobile Health Unit. All clients are provided with pre- and post-test counseling, and those individuals confirmed to be positive are referred and linked to appropriate care and treatment. In addition to the HIV test, individuals are given blood pressure, blood glucose and cholesterol screenings.
In 2012, Holy Cross provided 8,796 individuals with HIV testing services and 3,936 HIV tests. The positivity rate was 0.4 percent.
