FUN WEEK AND BUDDY WEEK

ALLSTAR MARTIAL ARTS ACADEMY’S

During Fun Week/Buddy Week we take a break from patterns and self defense, as a reward for all of the hard work leading

up to the testing and we just HAVE FUN—martial arts games, races, obstacle courses, etc! Moms, dads, brothers, sisters,

friends and more are welcome to attend!

WHEN: Tues., December 13 through Fri., December 16, 2011

WHAT: Bring your friends and/or family members to class with you

so they can see how much fun it is! (Minimum age 4.5 years old).

VERY IMPORTANT: Friends/family members MUST bring in a

completed & signed Buddy Pass Waiver, signed by their parent if

under 18. Buddy Passes are emailed to every student and are on

Facebook. Feel free to forward the Buddy Pass to your Buddy, so that

he/she can bring it in completed and signed on class day.

(Only the Buddy’s parent may sign the Buddy Pass. The

student’s parent, that he/she is a guest of, may NOT sign for

any child that is not their own.)

AND BELT DURING BUDDY WEEK!

Students and Buddies can wear an Allstar Martial Arts Academy

T-shirt to class during Buddy Week, so get yours today!

(Students wear their regular uniforms if they do not have an Allstar

T-shirt. Buddies can wear any comfy T-shirt and pants.)

In consideration of being allowed to participate in any class, training, and/or program at Allstar Martial Arts Academy the undersigned, on his or her behalf, and on the behalf of the participant(s) identified below, acknowledges, appreciates and agrees to the following conditions: I represent that I am the parent or legal guardian of the participant(s) named below, or I have obtained permission from the parent/legal guardian of the participant(s) named below to execute this agreement on their behalf. I agree that the participant(s) named below and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any training and/or program at Allstar Martial Arts Academy. I am aware that there are inherent risks associated with participation in Allstar Martial Arts Academy programs, training, and/or use of the martial arts equipment and I, on behalf of myself and the participant(s) named below, knowingly and freely assume all such risk, both known and unknown, including those that may arise out of the negligence of other participants; and, I, for myself and the participant(s) named below, and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release and hold harmless, Master Athletics, Inc., DBA Allstar Martial Arts Academy, their affiliates, officers, members, agents, employees, other participants, and sponsoring agencies from and against any and all claims, injuries, liabilities or damages arising out of or related to our participation in any and all Allstar Martial Arts Academy programs, activities, and/or training and/or the use of the martial arts equipment. I agree that pictures, videos, testimonials etc. of participant may be used in any way by Allstar Martial Arts Academy. I have read and fully agree to the terms of this waiver and release.
