Association of Der p 23–specific IgE levels (y-axes) (ISU/ml) from 158 HDM allergic patients with Der
p 1 (C) or Der p 2 (D)–specific IgE levels (x-axes).

Correlation of rAlt a 1-specific IgE antibodies determined by ImmunoCAP and ISAC. Alt a 1-specific IgE antibodies from 80 Alternaria-sensitized patients are indicated as kUA/L (y-axis) or ISAC Standardized Units (ISU) (x-axis), respectively.

Welche Immuntherapie würden Sie diesem Patienten verschreiben?
Komponenten-basierende Diagnose
keine genuine
Birkenpollen- und
Unkrautpollen-
sensibilisierung!!
Beispiel 1: Sekretärin, 52 Jahre
Symptome: Leichte Rhinokonjunktivitis im Frühling und
Frühsommer, schwere Rhinokonjunktivitis im
Hochsommer; Verzehr von Haselnüssen verursacht OAS
Extrakt-basierende Diagnose: Prick RAST
- Birkenpollen + (RAST 2)
- Lieschgraspollen + (RAST 2)
- Beifusspollen + (RAST 1)
- Ragweedpollen + (RAST 1)
- Prick-to-Prick mit Haselnüssen +
Serologische Diagnostik mit rekombinanten Allergenen:
rPhl p 1/5: - rPhl p 2/6: - rPhl p 7: - rPhl p 12: + (CAP 2)
Resultat: Isolierte Sensibilisierung auf Profiline
Vorgeschlagene Therapie:
Symptomatische Therapie,
Vermeiden von Haselnüssen
rBet v 1: - rBet v 2: + (CAP 2)
Beispiel 2, Studentin, 27 Jahre
Symptome: Rhinokonjunktivitis von März bis September, beginnende asthmatische Symptome, OAS nach Genuss von Äpfeln und Nüssen
Extraktbasierde Diagnostik: Positiver SPT and CAP RAST
- Birkenpollenextrakt - Wiesenlieschgrasextrakt
Komponenten-basierende Diagnose
rPhl p 1: + rPhl p 2: + rPhl p 5: + rPhl p 6: + rPhl p 7: - rPhl p 12: -
Ergebnis: Sensibilisierung gegen Gräser- und Birkenhauptallergene
Vorgeschlagene Therapie: SIT mit Gräser- und Birkenpollenextrakten
rBet v 1: + rBet v 2: -

richtige Verschreibung
gute Qualität der Vakzine
Immunantwort des Patienten
Monitoring der IT mit rekombinanten Allergenen
Messen von allergen-spezifischen IgG Antikörpern
Hauptallergene: Phl p 1, Phl p 5
Flicker S, Valenta R, Int Arch Allergy Immunol,2003;132:13-24
Allergenquelle Allergenextrakt
IgG spezifisch für nicht-allergene Komponenten
Allergen-spezifisches IgG
IgE-Epitop spezifisches IgG = Blockierede IgG
Monitoren der IT
Rekombinante Allergene können Diagnostik und Therapie verbessern
1. Diagnostik:
