2. MANDATE Ontario Health Teams are being introduced by the Provincial Government to

provide a new way of organizing and delivering services in local communities. An Ontario Health Team (OHT) (also called an Integrated Care Delivery

System in the Act) is a team of health care providers who work together to design a system for their population and deliver care to allow for a seamless transition for patients accessing health care. To be considered as an OHT, the OHT must have the ability to deliver at least three types of health services (e.g., primary care services, home care, acute care, long-term care, mental health and addictions services, palliative care services, etc.). The Ministry’s guidance document states that preference will be given to submissions that include a minimum of primary care, hospital, home care, and community care. As OHTs roll out to full maturity, they will be expected to provide the full continuum of care.

The deadline for Intake one of the Self-Assessment process was May 15, 2019.

Four submissions that included Lanark County or a portion of the Lanark County boundary were submitted. The Province has committed to validating provider readiness by assessing all Intake 1 submissions by early July. At that point, each of the 4 submissions will be notified of their state of readiness (In Discovery (requires more information) or In Development (invited to submit a full application).

The purpose of the Lanark County Ontario Health Teams Working Group is as

follows:

i) Upon completion and announcement of the provincial review and provider readiness of Intake 1 self-assessment submissions, undertake a thorough review and analysis of the Ontario Health Team self-assessment proposal(s) (assuming one or more submissions are approved to move onto Discovery Stage); to determine best suits the needs of Lanark County support. This includes discussion with the respective submission lead(s), and presentation to the Working Group, if deemed necessary.

3. WORKING GROUP STRUCTURE

Comprised of the following:

Chair, Lanark County Community Services Committee One representative from each local municipality (member of County

Council), including the Town of Smiths Falls Warden (ex-officio)

4. MEETINGS & QUORUM

The Lanark County Ontario Health Teams Working Group shall meet at the call of the Chair, to a maximum of four meetings, as required during Q3-Q4 in 2019. Quorum shall be deemed as 50% plus one voting members of the Working Group. The meetings will be held in a Meeting Room at the County of Lanark Administration Building, 99 Christie Lake Road Perth, unless otherwise determined.

5. REPORTING PROCESS

The Lanark County Ontario Health Teams Working Group will provide regular verbal updates to the Community Services Committee, through a report from the Chair, and will make final recommendations to the Community Services Committee, through a Staff Report, which will reflect the direction of the working group.

6. STAFF AND SUPPORT SERVICE

The Lanark County Ontario Health Teams Working Group will be led by the Chair of Community Services. The County Clerk or his/her designate, the Director of Lanark Lodge or his/her designate and the Director of Social Services or his/her designate shall participate in the Working Group. In addition, the County Clerk will provide additional staff resources that may be required as deemed necessary, by the Working Group.

7. BUDGET

Council remuneration will be paid in accordance with the Elected Official – Meeting and Professional Development Remuneration Policy. Travel reimbursement shall be paid in accordance with Travel Management and General Expenses Policy. All amounts will be expended to the Governance Budget for salaries and wages and travel expense. This only applies to County Council representatives.

8. TERM

The term of the Lanark County Ontario Health Teams Working Group will continue until the final report / recommendations have been presented to County Council, to a maximum of four meetings.

Right now there are four Ontario Health Team applicants in the region and the purpose of the meeting was to explore the possibility of working together on their approach. The Upper Ottawa Valley Ontario Health Team includes the Pembroke Regional Hospital, Algonquins of Pikwakanagan and partners. The Highway 60 Health Team includes the Renfrew Victoria Hospital, St. Francis Memorial Hospital in Barry’s Bay and partners. The Three Rivers Ontario Health Team includes Arnprior Regional Health and partners, and North Renfrew Health is made up of the Deep River Hospital and partners. Warden Jennifer Murphy says the meeting was an opportunity for them to lead the way and set an example of patient-first primary care. She is hopeful that a collaboration will be formed to support the community in the delivery of rural and locally focussed health care. Elected officials had the opportunity to review the applications. Ontario Health Teams will replace the Local Health Integration Network in an effort to have a seamless transition for patients accessing health care. To be considered an Ontario Health Team, you must deliver three types of health services such as primary care, home care, acute care, palliative care, or mental health and addiction services.

facility in Orléans that will bring health care services from across Ottawa under one roof.

The Orléans Health Hub is a one-storey building that will bring together bilingual services from

three hospitals and four community service providers to better support co-ordinated care and

reduce wait times, the government said.

"[It's] the first of its kind in the province of Ontario. It has been suggested to me this can be a

prototype for other parts of Ontario, and I would certainly agree," said Health Minister

Christine Elliot, who announced the funding at the planned building site on Mer-Bleue Road.

"We know this service is needed in this part of Ottawa," Elliot said. "It's a key part of our

broader plan to build a connected and sustainable public health-care system centred around

the needs of patients."

The new building will be about 96,000 square feet and will be located at the northeast corner

of Mer-Bleue Road and Brian Coburn Boulevard.

Opening 2021 

The hub, which is scheduled to open in the summer of 2021, will offer a variety of

services including specialist dermatology, pediatrics and orthopedics clinics, as well as mental

health services, senior care programs and diagnostic imaging services. 

The project, led by Ottawa's Montfort Hospital, is part of the government's plan to invest $27

billion over the next 10 years in hospital infrastructure projects across Ontario.

Dr. Bernard Leduc, president of the Montfort Hospital, said the initial concept was much larger

than what's currently planned, and that he hopes expansion will be possible in the future.

"We are celebrating the beginnings of phase 1 of the hub," Leduc said. "But we have saved

some space around here for phase 2 and 3."

The hub will provide space for a variety of hospitals and health organizations including

Montfort, CHEO and Bruyère Continuing Care.

Elliot said the total cost of the project will be up to $83 million. The hospital will continue to

raise up to another $12 million.
