Award Bid #16-01-0716: Printing of NCR Forms Opening Date: July 16, 2015 Term of Contract: October 1, 2015 – September 30, 2016 with an option to extend for two additional years in one-year increments (Supporting Information – Bid Recommendation Memo, Award Report, and Disclosure Forms) The Purchasing Office solicited bids to procure the printing of NCR forms for various Eastern Suffolk BOCES locations. Bid specifications were developed in collaboration with the Communications and Research staff. Bids were requested for a total of 392 items. Bid packets were distributed to 15 vendors as a result of advertising and direct solicitation. Two bids were received. Based on the results received for this bid, the estimated annual expenditure is approximately $20,000. The Purchasing Office contacted the lowest responsible bidders, Mark Palmer Assoc. of Mamaroneck, NY and W.B. Mason Co., Inc. of Hauppauge, NY to negotiate better prices and the vendors indicated that the prices were firm. Mark Palmer Assoc. of Mamaroneck, NY and W.B. Mason Co., Inc. of Hauppauge, NY have provided services to Eastern Suffolk BOCES in the past and their performance has been satisfactory.

The monthly cost is changing from $30 for maintenance only and a per copy charge per month to $371.47 for 20,000 copies per month. This is a replacement for an obsolete machine. include the Hosted Web Application Use Fees - Schedule J for the use and support of the WinCap software with Eastern Suffolk BOCES. The fees shall be extended for an additional one-year term commencing on July 1, 2015 and ending on June 30, 2016 at a cost not to exceed $76,000.

This is a license agreement with Proquest, LLC that will provide access and the use of products listed on the revised order form for internal research or educational purposes related to the Virtual Reference Collection Service for a one-year term commencing on August 26, 2015 and ending on August 25, 2016 at a cost not to exceed $520,000.

Learning to continue to provide access and service maintenance for the URL/gateway linking the Eastern Suffolk BOCES Assessment Reporting System (BARS) to NYLearns assessment questions and resources and 100 NYLearns user accounts providing system access in the registered environment specifically as it relates to My Curriculum and Publish Your Best!, including assessments, educational resources, and instructional content for an additional one-year term commencing on September 22, 2015 and ending on September 21, 2016 at a cost not to exceed $10,000.

S. Recommended Approval of an Amendment to the Agreement with Advanced Assessment Systems, Inc. (d/b/a LinkIt!)

(Supporting Information – Amendment to Agreement and Disclosure Form) This amendment to the agreement with Advanced Assessment Systems, Inc.

(d/b/a LinkIt!) will enable Eastern Suffolk BOCES to respond to program requests for which it provides New York State reporting services in order to assist such school districts in utilizing LinkIt!, which is a comprehensive software tool that can manage student assessment data aligned with state and Common Core standards, as well as the school districts’ own locally developed curricula initiatives for a one-year term commencing on September 1, 2015 and ending on August 31, 2016 at a cost not to exceed $150,000.

T. Recommended Approval of a Multi-Year Service Agreement – Project #FWAN-HAU-060315-2015-2020/Fiber WAN Project for the Hauppauge Union Free School District

(Supporting Information – Agreement) This is a five-year, multi-year service agreement for the renewal of the original

Flexible Private Line Fiber WAN Network for the Hauppauge Union Free School District. The estimated cost for the project is $165,528.

U. Recommended Approval of an Agreement with Forecast5 Analytics, Inc. (Supporting Information – Agreement and Disclosure Form) This agreement with Forecast 5 Analytics, Inc. will allow Eastern Suffolk BOCES

to offer 5Sight, which delivers data and interactive analytics covering key areas of school operations including staffing and compensation, academic performance, enrollment trends and demographics, English language learners, and budget performance. This one-year agreement commences on September 1, 2015 and ends on August 31, 2016 at a cost not to exceed $100,000.

RECOMMENDED MOTION: “BE IT RESOLVED, that the Board of Cooperative

Educational Services, First Supervisory District of Suffolk County, hereby approves the agreement with Forecast5 Analytics, Inc. for a one-year term commencing on September 1, 2015 and ending on August 31, 2016 at a cost not to exceed $100,000.”

V. Recommended Approval of an Agreement with TechTiles, LLC (Supporting Information – Agreement and Disclosure Form)

This agreement with TechTiles, LLC will allow Eastern Suffolk BOCES to offer a software application to effectively and efficiently perform forms automation and workflow design and process for a one-year term commencing on September 1, 2015 and ending August 31, 2016 at a cost not to exceed $250,000, which will be offset by district participation. RECOMMENDED MOTION: "BE IT RESOLVED, that the Board of Cooperative Educational Services, First Supervisory District of Suffolk County, hereby approves the agreement with TechTiles, LLC for a one-year term commencing on September 1, 2015 and ending on August 31, 2016 at a cost not to exceed $250,000, which will be offset by district participation.”

W. Recommended Approval of an Agreement with iTutor.com, Inc. (Supporting Information – Agreement and Disclosure Form) This agreement with iTutor.com, Inc. will allow Eastern Suffolk BOCES to offer an

online tutoring platform for all subjects, as well as offer K-12 test prep programs, advanced placement instruction, credit recovery, and face-to-face homebound instruction. This one-year agreement commences on September 1, 2015 and ends on August 31, 2016, at a cost not to exceed $60,000.

RECOMMENDED MOTION: “BE IT RESOLVED, that the Board of Cooperative

Educational Services, First Supervisory District of Suffolk County, hereby approves the agreement with iTutor.com, Inc. for a one-year term commencing on September 1, 2015 and ending on August 31, 2016 at a cost not to exceed $60,000.”

X. Recommended Approval of an Amendment to the Agreement with eSchool Data, LLC

(Supporting Information – Amendment to Agreement and Disclosure Form) This amendment to the agreement with eSchoolData, LLC will delete the existing

Exhibit B - Fee Schedule and replace it with a new Exhibit B - Fee Schedule, which includes small and large school district rates. The term of this agreement commenced on July 1, 2015 and ends on June 30, 2016, at a cost not to exceed $2,000,000. All other terms of the existing agreement shall remain in full force and effect.

RECOMMENDED MOTION: “BE IT RESOLVED, that the Board of Cooperative

Y. Recommended Approval of an Amendment to the Agreement with Custom

delete the existing Exhibit A, Section 2.1 of the agreement and replace it with the new Exhibit A, Section 2.1, in order to include a full fee schedule and description of products. The term of this agreement commenced on July 1, 2015 and ends on June 30, 2016, at a cost not to exceed $3,000,000. All other terms of the existing agreement shall remain in full force and effect.

Z. Recommended Approval of Pricing for School Lunch Program (Supporting Information – A` La Carte Pricing List) A review of the current breakfast and lunch prices charged by Eastern Suffolk

BOCES, compared to neighboring school districts, indicates that the current meal price of $3.00 per meal should remain unchanged for the 2015-2016 school year. Due to the requirements of the Healthy Hunger-Free Kids Act, Whitsons’, the agency’s food service management company, will continue to charge $3.422 per meal. The difference in the per lunch cost is covered by an allocation in the Special Education budget, as the $3.00 per lunch cost is already higher than most of the surrounding school districts. It is therefore recommended by the Manager

of Administrative Services that the Board approve the student charge of $3.00 per meal for breakfast and lunch, as well as the a` la carte pricing for the 2015-2016 school year.

AA. Recommended Approval and Acceptance of Policy Statement for Free and

Reduced Price Meals or Free Milk and 2015 - 2016 Income Eligibility Guidelines for Free and Reduced Price Meals

(Supporting Information - Policy Statement and Income Eligibility Guidelines) Federal regulations require that school districts wishing to participate in the

federally assisted National School Lunch Program must approve the policy statement for free and reduced price meals including the family income eligibility criteria. Although the policy is the same as in the past, the income eligibility numbers have been adjusted for inflation. It is recommended by the Manager of Administrative Services that the Board approve and accept the Free and Reduced Price Meal Policy Statement including the Family Income Eligibility Criteria.

RECOMMENDED MOTION: "BE IT RESOLVED, that the Board of Cooperative

BB. Recommended Approval of an Amendment to the Lease Agreement with the Port

Jefferson Union Free School District (Supporting Information – Amendment to Lease Agreement) It is recommended by the Manager of Administrative Services that the Board

approve this amendment to the lease agreement between Eastern Suffolk BOCES and the Port Jefferson Union Free School District for the use of the Jefferson Academic Center for a five-year period commencing on June 29, 2017 and ending on June 28, 2022. The total annual base rent from June 29, 2017 to June 28, 2018 will be $399,609.57. The annual base rent will increase in years two through five by the property tax cap allowable growth factor (i.e. the lesser of 2% or the CPI-U). In addition to the annual base rent, Eastern Suffolk BOCES will pay $67,928.32 each year, representing agreed-upon compensation for the district issuing a bond to fund renovations to the property as described in the lease. Such additional rent shall not increase during the lease term extension. This lease agreement was developed by a committee of supervisory district and Eastern Suffolk BOCES representatives, fulfills the New York State Education Department Special Education Space Requirement Plan, and is in the best financial interest of the supervisory district.

CC. Recommended Approval to Extend the Long Term Disability Group Policy with

Reliance Standard (Supporting Information – Extension Letter from Reliance Standard) Reliance Standard has agreed to extend the existing Long Term Disability Group

Policy for a one-month term commencing on September 1, 2015 and ending on October 1, 2015 with no change in rate. A new contract with Unimerica will be recommended for approval at the September 30, 2015 Regular Board Meeting.

(Supporting Information – Agreement) RECOMMENDED MOTION: “WHEREAS, the Board of Cooperative Educational

Services, First Supervisory District of Suffolk County, as a Participating Employer in NYSHIP, does hereby notify the Employee Benefits Division (EBD) that it has elected to extend eligibility for coverage to the domestic partners of its employees, vestees, and retirees.

NOW, THEREFORE, BE IT RESOLVED, that the Board of Cooperative

Educational Services, First Supervisory District of Suffolk County, authorizes the Board President to execute the aforementioned agreement on its behalf and agrees to use the eligibility criteria and enrollment forms provided by the EBD.”

FF. Recommended Ratification of the Memorandum of Understanding Between

Eastern Suffolk BOCES and the CSEA Adult Nursing Instructors Unit and Authorization of the Chief Operating Officer to Execute the Resulting Collective Bargaining Agreement

(Supporting Information – Under Separate Cover) RECOMMENDED MOTION: “WHEREAS, the negotiating representatives for the

Board of Cooperative Educational Services, First Supervisory District of Suffolk County (hereinafter referred to as “ESBOCES”), reached agreement on the terms of a Memorandum of Understanding (“MOU”) with the CSEA Adult Nursing Instructors Unit (hereinafter referred to as “CSEA ANI Unit”) setting forth the terms of a collective bargaining agreement between the parties for the period from July 1, 2013 through June 30, 2020 that is subject to Board approval; and

WHEREAS, that Memorandum of Understanding provides that it is subject to

ratification by members of the bargaining unit and the Board; and WHEREAS, the Board has been advised that members of the bargaining unit

have ratified the Memorandum of Understanding; THEREFORE, BE IT RESOLVED, that the Memorandum of Understanding with

the CSEA ANI Unit, setting forth the terms of a collective bargaining agreement between the parties for the period from July 1, 2013 through June 30, 2020 is hereby ratified; and

BE IT FURTHER RESOLVED, that the Chief Operating Officer is hereby

authorized to execute the resulting collective bargaining agreement between ESBOCES and the CSEA ANI Unit on behalf of ESBOCES.”

GG. Recommended Approval of the 2015-16 Professional Development Plan (Supporting Information – 2015-16 Professional Development Plan) Under Section 100.2 (dd) of the Commissioner's Regulations, superintendents of

schools and district superintendents must annually certify to the New York State Education Department that their districts or BOCES have complied with their professional development plans for the current school year and have professional development plans in place for the upcoming school year.

RECOMMENDED MOTION: “BE IT RESOLVED, that the Board of Cooperative

Educational Services, First Supervisory District of Suffolk County, hereby approves the Professional Development Plan for the 2015-16 school year.”

HH. Recommended Approval for Students and Staff to Visit the U.S. Military Academy,

West Point, and the Purple Heart Museum Recommendation is made to approve the attendance of 50 National Junior ROTC

cadets and five chaperones from the William Floyd High School National Junior ROTC Program to visit the U.S. Military Academy, West Point, and the Purple Heart Museum in New Windsor, NY on Monday, September 14, 2015. Visiting these destinations aligns with the current curriculum and will further the cadets' knowledge in the National Junior ROTC Program. The total cost to the agency for transportation is $1,493. The funds required to cover the cost of this trip were appropriated in the 2015-2016 budget.

RECOMMENDED MOTION: “BE IT RESOLVED, that the Board of Cooperative

Educational Services, First Supervisory District of Suffolk County, hereby approves the attendance of 50 National Junior ROTC cadets and five chaperones from the William Floyd High School National Junior ROTC Program to attend a field trip on Monday, September 14, 2015 to the U.S. Military Academy, West Point, and the Purple Heart Museum, in New Windsor, NY.”

Society Convention Recommendation is made to approve the attendance of 42 students and four staff

members from the Harry B. Ward Technical Center’s Audio Production class to attend the Audio Engineers Society Convention at the Jacob Javits Convention Center located at 655 West 34th Street, New York, NY on Thursday, October 29, 2015. Students will be able to see and handle all of the newest products and immerse themselves into an industry related event. They will become more familiar with current industry related technologies before the general public. The only cost to the agency is approximately $1,203 for bus transportation.
