Hospital Discharge Process – Hospital Social Work Team – Mark Harris Mark Harris gave a presentation which gave an overview of the Hospital Social Work Team, the processes they work towards in terms of facilitating appropriate discharges from the hospital, the various colleagues they work with, how the team accesses the Reablement Service and how the team has developed over the years since 2011. Yvonne stated that some patients are still being discharged from the Hospital without the carers being well informed and time for the carers to arrange support at home and a lack of accessing the Carers Lounge. Yvonne felt that if carers were given enough notice of a discharge they will be able to arrange some support and assist appropriate discharges and avoid inappropriate readmissions. Mark stated that the Hospital Social Work Team are continually encouraging hospital staff and wards to make use of the Carers Lounge and request support from the Hospital Social Work Team and is aware that this support is sometimes bypassed. Mark agreed to liaise with the Carers Lounge to establish again and build rapport. John informed the Board that it is important to note that the Hospital Social

Work Team will not be made aware of all hospital discharges as it may be that the individual has a clinical need rather than a social aspect and therefore staff will not refer the individual via the Hospital Social Work Team. It is challenging and something the team are looking to minimise. Tracey asked if the Hospital Social Work Team get involved at the pre assessment stage in advance of a planned operation. Mark stated that the Team are currently working towards getting involved at the pre assessment stage as early intervention is key to a successful admission and smooth discharge from Hospital. Tracey stated that some patients may think they are ok and do not require support, whereas the carer may feel otherwise and asked if the Team has dealt with similar cases. Mark agreed with Tracey that some patients do believe this and are able to make their own decision if they have capacity but to note that when assessing a patients needs, the carers needs need to be considered in the form of a carers assessment and contingency plans must be arranged. Glenda complimented the good work of the Hospital Social Work Team as she personally knows of someone who has recently accessed the service. Tracey asked if out of county visitors can access the same support. Mark explained that their Local Authority will be responsible for supporting the patient but at times the Hospital Social Work Team is able to complete assessments on behalf of other Local Authorities. Mark explained that the same process would apply to Bedford Borough residents who require healthcare from an out of county hospital. Tracey stated that the 7 day service is beneficial to patients. John asked what support the Hospital Social Work Team can offer for people with a physical disability or sensory impairment. Mark explained that the Team have several links to specialists within the hospital e.g. Occupational Therapists, audiologists and are able to provide various pieces of equipment to aid the assessment process. Joyce T stated that she had heard of incidents where patients hearing aids / equipment had been stolen whilst in hospital and asked what is being done to support people. Mark explained that the Hospital Social Work Team are able to advocate on behalf of the patient, signpost to advocacy services and look at whether the case needs to be investigated and a safeguarding alert be raised. Mark stated that it is important all patients have appropriate equipment. Tracey asked if there is a delay for equipment and whether this would affect someone’s discharge. Mark stated that there can be delays in ordering equipment but that the Team have good links with the hospital and are able to order equipment to support a discharge and short notice. Mark stated that no one should be discharged without the appropriate information or equipment in place.

Sam asked what happens when the equipment is no longer needed. Jodi informed the Board that there should be a label with contact details on the equipment and the provider can be contacted to see if they are able to recycle the equipment. Jodi is aware that some pieces of equipment may not be recycled because the cost is more to clean and recycle than it would be to purchase a new piece of equipment and it is likely that individuals will be asked to dispose of the equipment themselves, but it is best to speak to the equipment provider in the first instance.

Effectiveness of Partnership Boards Feedback The report was presented to the Board for comments. 6 responses were received, however due to the range of responses, it has been difficult to draw any firm conclusion. The findings from this report will be passed onto Corporate Performance who are currently reviewing all Partnership Boards across the Council. Board members raised the following:

Partnership Boards are invaluable. Need more service user representation but are unsure who can attend

and what the criteria is to become a member, as there may be people with an impairment but do not have reason to access social services support. People will want to know how they will benefit from attending the Board.

District Access Group value the Partnership Board as they see this Board as way to raise the profile of a particular issue.

Need to raise awareness of the Partnership Board and look at other forums.

Stuart explained that as facilitator of the District Access Group, they would like to increase the variety of people attending and encourage more young people to attend and is therefore linking with Bedford College. Yvonne stated that carers would struggle to attend all partnership boards as they have caring responsibilities to attend so therefore carers views are gathered at the carers groups and feedback via one representative who sits on the Partnership Board. Glenda agreed with Yvonne’s approach and stated that service users views are gathered at various advocacy groups as sometimes service users would find this meeting intimidating and more chaotic if too many people were to attend. John thanked members for their comments and explained that he sees the Partnership Board is there for people who do access social services and for those who do not access services. The Partnership Board is an opportunity for representatives of various groups to raise issues on behalf of several service users, carers or providers and work as a partnership to improve services and plan future services. The Board is a good opportunity to share information.

Physical Disability & Sensory Impairment Strategy Action Plan Updates The latest updates were presented to the Board and the following was highlighted: Priority 1, item 4 – Headway used to have a link with Bedford College for people with a brain injury. There has been problems with accessing grant money and therefore this service has ceased. It was agreed to invite Katrina O’Brien to provide an update about why this has happened and what are the options for the future. It was agreed to invite the Jobs Hub to give a presentation as some members were not aware of the work of the Jobs Hub and what they can do for people with a disability. Priority 2, item 1 – It was agreed to invite Diane Meddick form the Bedfordshire Clinical Commissioning Group to talk about Telehealth. In terms of Telecare, numbers are increasing and awareness of the service is increasing and an improved responder service. Priority 3, item 2 – Joyce spoken on behalf of a friend who is deaf in one ear but does not like to be referred to as deaf and was unaware of the services / support she may be able to access. Joyce explained that they had looked at the Council’s website under Hearing Impairment to see how someone can be added to the register despite not requiring services. Her friend has approached the audiology department who appear to have some bits of information but a lot of it is out of date and has been advised that no one seems to make the audiology department aware of services available and where to go next. John stated that the audiology department should be able to provide information on what to do next and support services available and look at who commissions this service. Jodi agreed to ask the Deaf and Hearing Impairment Team how someone becomes registered and to ensure the Team improves information sharing with the audiology department. Priority 3, item 3 – Tracey felt transport is a concern for people with a disability who would like to access local community facilities. John explained that the most appropriate forum for issues concerning transport need to be raised at the Transport User Forum. Priority 3, item 2 – POhWER to provide updates for the action plan. Priority 4, item 3 – The Government plans to look at regulating new housing. The District Access Group has identified new housing in the Borough which does not meet accessibility requirements. A report will be submitted to the committee in February. Stuart to provide updates for the action plan.

5.11 Priority 4, item 3 – The first resident will be moving into St Bedes this week. The extra care housing facility is for people over the age of 55. 24/7 onsite care is available to those who require the service.

Mental Health Partnership Board Feedback No significant updates.

Service User news Bedford & District Access Group – The minutes of the September meeting were noted. A sight/hearing impaired Access Group member had reported a serious incident when she was knocked over by a cyclist on a pavement. She felt that there was a lack of awareness of the needs of disabled people, particularly those with a ‘hidden’ disability such as hearing and/or sight loss. This had been a concern of the Access Group for many years. It was agreed that:

The Council’s Cycling and Walking Officer should be asked to raise awareness

A Freedom of Information request should be submitted to Bedfordshire Police – asking for details of penalty notices given to cyclists in the pedestrianized area of Bedford and in the other town centre areas

The Physical Disabilities & Sensory Impairment Partnership Board should be asked to raise this matter with the Police, and ask for a senior police officer to attend one of the Board’s meetings

POhWER – The contents of the report were noted. Contents from a report provided by Anona were noted. The report highlighted actions taken since the last meeting on Welfare Reform changes and a Carers Forum. It was agreed to circulate the report.

Carer News Yvonne informed the Board she will be retiring from Carers In Bedfordshire. Helen Satterthwaite has been appointed and plans to attend the Board meetings. Yvonne stated that there are plans to establish a Carers Lounge within the Luton and Dunstable Hospital and patients from the Bedford Borough area will be able to access advice and information from there. Board members thanked Yvonne for her contribution to carers and the board and wished her well in retirement.

circulated to members. A suggestion was made to include online information on how to log an issue and upload to the website once the leaflet has been reviewed. Stuart informed the Board that there was a programme on 30th October on BBC4 on ‘Disability over the years’ and how things have changed which was an interesting watch.

Referrals on the increaseIn late 2010 the team averaged 60 Referrals a Month.

The current average of 120 Referrals every month.

The impact on Winter Pressures has also spread out across the year and where we have seen peak’s and trough’s of referrals in the past we are now seeing a constant demand.

The vast majority of people are referred on to the Reablement team who have 2 Coordinators based in the team.

Constant Improvement January 2011 – Older Person’s Assessment and Liaison

(OPAAL) trial starts within Bedford Hospital working with people aged 65+.

April 2011 – Weekend working trialled over the bank holidays (22/04 – 02/05) – During this period 64 bed days were saved and 36 additional discharges were brought forward.

May 2011 – OPAAL trial reviewed and agreement reached for it to become a mainstream service.

June 2011 – OPAAL expanded to include people aged 18 – 65.

August 2011 – Reablement Coordinators join the Hospital Social Work Team.

October 2011 - 7 day a week working starts in the Hospital Social Work Team

Constant Improvement March 2012 – Hospital Social Work Team take over

management of the discharges from Community Health Rehab Beds and outlying Hospitals to provide a seamless patient journey.

October 2012 – Welfare rights advisor based at the hospital to increase benefits gains for the patients of the Hospital – We are currently projecting a potential annual benefit gain of over £220,000

Jan 2013 – Increased referral rate to Telecare and the introduction of devices such as the sonido listening device to improve involvement of patients in the assessment process.

April 2013 – OPAAL transformed into the Navigator project

patients to enable them to start the assessment process before surgery

What is the Navigator Project? Developed from the Older Person’s & Adults

Assessment and Liaison (OPAAL) model which facilitates same day discharge.

Made up of Discharge Planning Team members, Occupational Therapists, Physiotherapists, Social Workers, Reablement Coordinators.

Discharge can include support from Red Cross, Reablement (Bedford Borough), Rehab and Enablement and Rapid Intervention (South Essex Partnership Trust)

Referrals received from all wards within the hospital where the length of stay is under 72 hours from 9am to 3.30pm 7 days a week.

The service aims to fully assess and discharge patients within 2 hours of the referral – If this isn’t possible the patient remains with the Navigator team and they aim to discharge the patient within 72 Hours.

Bob’s Journey (Navigator) Attends Accident and Emergency Department - Sunday 1.30pm

– Screened by Consultants Team – No reason for acute admission - main concerns around an inability to cope at home.

2.00pm Referral to Navigator Team – Via Discharge Planning who complete single assessment paperwork, gained agreement from Bob to be assessed, and then refer to Social Work Department and on call Hospital Occupational Therapist (OT).

3.00pm assessments completed by OT, Social Worker and Reablement Coordinator.

3.15pm Reablement confirmed availability of same day care –Red Cross contacted to arrange transport for 4.30pm.

Red Cross settle Bob at home and the first care visit arranged for 6pm that evening.

After 6 weeks Bob was assessed as being self caring and the Reablement Team ended their involvement.

Bob continues to live independently.

Ruth’s Journey (Hospital and Intermediate Care) Single assessment paperwork was received from the discharge

planning team Ruth had suffered a stroke and the referral requested a package of care The social worker contacted the Occupational Therapist and questioned

the suitability of Rehabilitation On review the occupational Therapist agreed that inpatient

rehabilitation would be beneficial for Ruth Ruth was transferred to the Archer Rehabilitation Unit (South Essex

Partnership Trust Community Health Service) The Hospital Social Worker continued to work with Ruth and advocated

for further intervention from the Rehab and Enablement Team (South Essex Partnership Trust Community Health Service) on discharge

The Social Worker continued their work into the community and carried out a community review 6 weeks after discharge

Ruth was then supported with a package of care through self directed support which enabled her to make informed empowered choices about her care provision.

Why length of stay is important

After the acute phase of an illness has been treated it is estimated that a patient looses 5% of their functionality every day they remain in the acute setting – This greatly impacts on both the patient and the level of support they will require on discharge

A stay in an acute environment has risks attached to it such as : The risk of developing a hospital acquired infection –

MRSA, Hospital Acquired Pneumonia etc. The risk of institutionalisation

The Delayed Discharge Act

Gives the relevant local authority 48 Hours to carry out an assessment and provide appropriate care to facilitate discharge

If this doesn’t occur the Hospital can issue a section 5 notification which will charge the local authority for every day the discharge is delayed

Effectiveness of Partnership Boards (Physical Disability and Sensory Impairment Board)

1. Earlier this year we asked members of all the Adult Service partnership

boards to complete a short questionnaire. The questionnaires were issued to help us understand why individuals and organisations attend partnership board meetings.

2. The responses from the questionnaires have been collated and the main results regarding this Board can be found overleaf.

3. The information from the questionnaire is feeding into a whole system review of partnership arrangements being carried out by the council’s Corporate Policy and Performance Team on behalf of the Mayor and Chief Executive. The review is tasked with designing a new robust partnership structure to replace the Local Strategic Partnership including the partnership boards which effectively manage and deliver the actions set out in the local strategies.

The number of responses was small (6) and the range of responses broad making it difficult to draw many conclusions.

50% of respondents either agreed or strongly agreed that there was good service user representation at meetings whilst 50% strongly disagreed, only one of the responses came from a service user.

Next steps

Members of the Physical Disability and Sensory Impairment Partnership Board are asked to respond to the feedback received and record what the Board intends to do as a result of that feedback.

The Board’s response will feed into the overall review of partnerships and proposals arising from it.

We have seen an increase in referrals over the last two months. All service users undergoing a period of reablement are considered for Telecare and cases are challenged weekly at panels if Telecare hasn’t been discussed.

3. Make it easier for people with physical disabilities or sensory impairments to arrange for their own equipment, assistive technology or home adaptations.

Support for Deaf and Hearing Impaired People

1. Drop-in Advocacy Service

The drop-in advocacy service continues to be well-attended. The drop-ins are held on the first Thursday and third Tuesday of the month. This regular format is now becoming well-known to service users. Two fully qualified interpreters are present throughout. Up to five people have attended at any one session. It generally takes about twice as long to investigate issues for a deaf person as it does with hearing people. Issues dealt with include problems with utility companies, employment issues, benefit issues and housing problems and access to medical services. POhWER continues to educate organisation with regard to the need for them to provide interpreters for their clients.

2. Social Event

POhWER facilitated an evening social event for deaf and hearing impaired people at the Gordon Arms in Bedford on 26th September. Approximately 30 people attended. A presentation was given on the new Welfare Reforms. Two interpreters were available for the event, as well as two members of POhWER staff. Feedback on the event has been very positive. A meeting has been arranged with Bedford Borough Council commissioners in early November to evaluate the event and discuss the way forward.

Support for visually impaired people

Emma Robinson, Community Development Worker, has been in contact with members of the Wednesday Club. They have asked her to look into providing easy-read read or large print versions of our leaflets. Emma is in consultation

with our Media and Marketing Department to investigate what suitable materials are available and will share her findings with the Wednesday Club for their opinion. She is also consulting with Advocacy for Older People, our partner agency, with a view to providing their materials in a suitable format.

Support for people with an acquired brain injury

POhWER has been working on an individual basis with service users who have an acquired brain injury; a considerable amount of work has been done with people who are living at Park House in Bedford. Issues include support to move on from Park House following rehabilitation, and assistance with financial issues. One resident has needed to be provided with a Russian interpreter.
