Post: Food Action Together Project Worker (Health)
Responsible to: Chief OfficerResponsible for: Volunteers working with FAcTKey Relationships: Older people engaging with FAcT; Local hospital

Purpose of this Post
To support the development of the Food Action Together (FAcT) programme, focussing particularly on:
o Developing links with primary care to promote healthy diets and exercise, building on the Government initiative around obesity;
o Developing health walks and cycling in the area;
o Supporting patients from households in three wards who are discharged from hospital and at risk of malnutrition/dehydration to develop better diets through one-to-one educational support on issues such as labelling and learning easy to cook recipes.
Key Duties & Responsibilities

1. To develop a range of local initiatives in three wards to support the FAcT programme, working on a ‘test and learn’ basis.
2. To establish and maintain appropriate systems able to record activity, outputs, impact and involvement by the public and volunteers. This must be at least sufficient to provide evidence to funders and other interested parties.
3. To recruit volunteers from within the local community to support the development of walking and cycling groups. To oversee the appropriate recruitment, selection and training of potential volunteers and offer on-going, arms-length support.
4. To link with the Greater Manchester Nutrition and Hydration Programme and ensure a local fit with GM wide activity
5. To initiate and deliver health fayres in community settings across the eligible wards
6. Working with dietetic and other staff, develop and implement community wellbeing plans for older people identified as at risk of malnutrition and dehydration when at home. Provide practical support on a time limited basis including assisted shopping trips and (healthy) food preparation. Where appropriate ensure that each plan includes welfare benefit /Keep Warm check by Age UK WB Information & Advice team.
7. To advertise the programme and make appropriate links to generate referrals of suitable older people.
8. To develop appropriate risk assessments for activities being set up.
9. To explore opportunities to engage local health services and identify the benefits of participation by individuals.
10.To contribute to the FAcT website, updating with related material and encouraging local people to contribute material, recipes, tips etc.
11.To support the organisation to fulfil its responsibilities under Health and Safety. You may be asked to take on roles such as First Aider, Health and Safety representative or Fire Marshall for which appropriate training will be given.
12.To support students on placements. This may include having students that shadow your work or performing a supervisory role. Appropriate training will be given.
13.To participate in Age UK Wigan Borough promotional events as required.
14.To work within all the guidelines, policies and procedures as set out by Age UK Wigan Borough.
