1. I am the deponent herein and a citizen of India.

2. I was born on Mar 17, 1944 at Bikaner, Rajasthan, India.

3. I was married to BHATT Jaydevbhai Hasmukhlal on Oct 7, 1965 in Surat, Gujarat, India.

4. As it is customary, I had gone to my parents place at Bikaner, Rajasthan, India for the delivery of our first child in Nov, 1967.

5. Smita was born on Dec 17, 1967 in Bikaner, Rajasthan, India. I have the personal knowledge of her birth because I am her mother and I gave her birth.

6. Smitas full name at the time of birth was given as BHATT Smita Jaydevbhai.

7. Smitas birth was registered later because it was not prevalent at that time to register the birth of children.

8. As it is customary in our region to add the suffix tag ben for females for giving respect purpose, when she was admitted in the school, her first name was provided as Smitaben.

9. Smita was married to JADAV Dipesh Morarilal on Apr 2, 1992 at Ahmedabad, Gujarat, India. I had attended their wedding.

10. Therefore, the following names refer to the one and the same person:
