The danger of working with and around electricity is well
Publicized.
It has the potential to kill or cause severe injury.
Hazards include:
Electric Shock and burns from contact with live parts
Exposure to arching or fire from electrical install
Other types of injury related to the above occurrences
such as falling from height.
So vigilance and robust control processes for working with
and around electricity is critical and required at all times.
These controls are particularly important on large multi
phased projects such as LBSR were construction teams are
working in and around live electrical assets with varying sizes
of plant and tools frequently.

Since the Time to focus session on electricity last November we have
seen an increase in the number of time to talks carried out around
electricity
Close calls
Total close calls raised 2012 to 2017 = 1967 (59 Months) = 33 per
month
1st Nov 2017 – 3rd Aug = 745 (9 Months) = 83
Since the time to focus session we have again seen a significant rise in
the number raised

LV cable was damaged by the action of the bucket of a 3t 360 excavator. A flash resulted.
There were no injuries and there was no one in the immediate vicinity at risk of injury.
The activity was in the area immediately adjacent the Shipwright Arms and involved the
installation of drainage as part of the Tooley Street urban realm works.
The drainage had been installed and the excavation was being backfilled with local and
imported material. It is reported that it was during the movement of the spoil that the bucket
came into contact with a previously identified and exposed cable. The machine was not
digging but dragging back the material - CCTV footage made available by the Shipwright
Arms substantiates this.
The immediate cause of the incident is the contact of excavator bucket with the cable
causing a short circuit. The underlying causes include the failure to follow process and
supervisory deficits.

Damage to Cable No 97 above ER-E20 feeding MCC-EO-23 block A mezz
Kilnbridge fire sealer was filling the hole above ER-E20 on the mezz on the North side with
compound when all of a sudden the compound was blown everywhere.
With better lighting we found a small nic on cable No 97 which is feeding MCC-EO-23 on
block A mezz, we confirmed this was the cable by hand tracing it back to SWBD D1-1 in ER-
E20. Contacted the permit office so they could inform the station that we need to isolate the
MCC.
Isolated Cable No 97 MCC-EO-23 and fitted safety locks and issued a permit to work No
06473. Then insulation resistance tested the cable and it was clear.
Once the cable was removed the damage was clear and the only way to restore power was
to cut the cable and insert a torpedo with a new temporary section of cable.
We installed the torpedo and new section of cable, retested it and re-energised it at 1330.

Details of event: An incident occurred on Saturday 10th March with a BROKK damaging
what is now known to be a redundant station communication cable during the task of
exposing existing drainage between 2 no. Drainage pits {7 & 8} in the Western Passage. no
injury

This investigation relates to the discovery of a failure to adequately secure access to a
permit controlled LV room and the modification of the limitation of access permit issued for
the planned works being carried out in this room.
During a routine inspection on the BOH area by Gary Smith NR interface manager the door
to room ER-E05 was found open and paperwork for the planned work activity was found on
the floor.
Subsequent investigation identified that the limitation of access permit for the room had been
modified to include the name of the engineer who had arranged for it and also that
scaffolding was being carried out as part of the works.
Works were suspended and the room was shut.
The principal causes of the failures are the modification of a limitation of access permit after
it was issued by the permit issuer. And the permit controlled room being left open and
accessible to anyone who wanted to gain unauthorised access to the room.

Following the successful HV Interconnector Testing & Commissioning
of the Black Building Test, all HV works are now complete. The
station is fully operational as per the design intent.

110v Tools and leads are within their 3 month PAT test date

From September onwards as mentioned above anyone working in any
Main LV Switchrooms must have VIR/SPR and endorsed by NR –
Richard Burgess.
Note HV Rooms will remain under NGB safe system of work, permit to
work.
