“Rivers and Channels: For man who is on the average eight chi, the skin and flesh, and the external boundaries and limits may be measured, and the pulse taken. Upon

death, examination can reveal and disclose. There are great standard measures to the existence of the viscera being: strong or weak; the bowels being large or small; the valleys (the amount of grain inside the hollow organs) being plentiful or sparse; the vessels being long or short; the blood being clear or turbid; the qi (oxygen) being

plentiful or sparse”.

Huang Di Nei Jing, Su Wen, Chapter 39 (206 BC–220 AD) :

經脈流行不止，環周不休，寒氣入經而稽遲。泣而不行，客於脈外，則血少，客於脈中則氣不通，故卒然而痛。

“The vessels/ channels run ceaselessly and circle incessantly. Cold evil aggresses on the vessels, the blood will be made scanty there. If it resides

inside the vessels, the qi will be obstructed. So, pains ensue suddenly”.

寒氣客於脈外則脈寒，脈寒則縮踡，縮踡則脈絀急，則外引小絡，故卒然而

痛，得炅則痛立止；因重中於寒，則痛久矣。

“If the cold reside outside the vessels, then the vessels become cold. When the vessels become cold, they shrink. The shrinkage makes the vessels spasmodic, and the spasms radiate to the little blood vessels outside, so the pain occurs

abruptly while it ends quickly when being given heat. If the patient is hit by cold repeatedly, the pain will be protracted”.

寒氣客於背俞之脈，則脈泣，脈泣則血虛，血虛則痛。

“The cold settles in the foot greater-yang bladder vessel and obstructs it. The obstructed vessel causes the blood to be

deficient. The blood deficiency produces pain”.

寒氣客於經脈之中，與炅氣相薄，則脈滿，滿則痛而不可按也。寒氣稽留，炅氣從上，則脈充大而血氣亂，故痛甚不可

按也。

“The cold resides in the vessel/channel and wrestles with the hot qi, so the vessel becomes full, and the fullness makes the pain

resistant to pressure (fullness causes pain that has an aversion to pressure). The cold lingers and the hot qi fights against it, so the

vessels are dilated and the blood and qi become disordered. Therefore, the pain is severe and cannot bear pressure”.

From these ancient quotes it is obvious that the term “meridian” refers to vessels that were

visible to the naked eye.

• 206 BC – 220 AD: 灵枢 [Ling Shu]: – Refers to the stimulation of a vessel area (mai) not a precise point;

• 259 AD (3 Kingdoms Dynasty): 鍼灸甲乙經[The Systematic Classic of Acupuncture and Moxibustion]:

– Stimulation and location of acupoints/nodes (jie) were added; – Acupuncture prescriptions were added;

• 300 AD: 肘後備急方 [Handy Therapies for Emergencies]:– Refers to area, not acupoints/ nodes– Acupuncture for acute situations

• 625 AD: 千金要方 [Prescriptions Worth a Thousand in Gold] and

• 752 AD: 外台秘要 [Secret Formulas of a Frontier Official]

– Emphasis on use and description of moxibustion rather than acupuncture stimulation;

– Stresses the use of an area rather than a single acupoint/ node;

• 1027 AD: 銅人腧穴鍼灸圖經 [Illustrated Manual of Acupuncture Points of the Bronze Figure]:– chest and trunk points were arranged in rows, only limb points were arranged into

the vessel system

• 1111-1117 AD: 圣濟總錄 [Sheng Ji Zong Lu: Collected Prescriptions for Divine Relief From Suffering]:– Points on the chest and back were added into vessel system diagram

- These ancient texts clearly show that there was no emphasis on the prescribed use of an exact acupoint/ node in treatment protocols, but rather suggest a deep understanding of the biophysical relationships within the body.

- Because of this historical precedence, and my own clinical experience, I believe needling a broader treatment area is the most effective way to achieve efficacy in pain relief.

• Pain Pathway

– To feel pain, 2 nerves are involved

• 1. Sensory Pain Nerves (Nociceptors) – Tells the brain there is pain: “ouch”

– 2 types

» A Delta Fibers: sharp, burning pains

» C Fibers: dull, throbbing pains

– Sends signals to L5 → Up the spine → Tract of Lissauer → Midbrain

– To feel pain, 2 nerves are involved

• 2. Proprioceptive Nerve Fibers

– Tells the brain the location of the pain: where the “ouch” is at

– Fires continuously at the affected area → Sends signals up the back of the spine → Midbrain

• Pain Pathway– Midbrain releases endorphins / enkephalins that bind to

pain receptors along the spine and the capillary beds where the pain exists in order to reduce pain

• Patients with chronic pain– Proprioceptive neural threshold is too low– Signal to midbrain is weak

• Patient cannot pinpoint the EXACT location of pain• Midbrain does not release endorphins / enkephalins

• Acupuncture creates a strong stimulation and “re-boots” the proprioceptive nerve pathway

– The sensation created by the needles ‘jumps’ the neural signal threshold and stimulates the brain to release endorphins / enkephalins

– After a few hours, the signal gets weak again

– The next day, needle the patient and stimulate the signal again

Keep needling and jumping the threshold until the body REMEMBERS and RE-ESTABLISHES the normal threshold to signal the brain

Functional Anatomy

The ancient Chinese were among the first to

develop a standard terminology to describe

various anatomical features, locations, and

regions of the body.

- The standard Chinese anatomical reference

position involves a LIVING PERSON standing

erect with head and feet pointing straight

ahead, and the arms hanging down the sides

with the palms facing inward and toward the

legs.

- This position is in contrast to the Western

anatomical position, which is derived from a

cadaver lying on its back with the palms of

the hands facing upward.

Functional Anatomy

Recognizing the longitudinal organization of the body was a major step for the

ancient Chinese and is fundamental to most physiological aspects of Chinese

medicine.

- The longitudinal vessels were named by the six peripheral body regions (noted in 6

yin-yang terms) that they traverse.

- The upper and lower portions of the six main regions of the body are subdivided

into hand and foot assignments.

- This results in twelve specific regions on each side of the body, each related to a

particular Jing Mai distribution vessel and internal organ. The net result is an

anatomical notation system that is consistent with the longitudinal organization and

distribution of blood vessels, nerves, muscles, fascia and their related tendons.

Perhaps the most innovative discovery of ancient Chinese Medicine is the highly integrated view of the body involving the internal organs, the neurovascular systems (JingMai) and the external body, which includes the musculoskeletal system (JingJin).

These major aspects of the body give rise to the various physiological phenomenon:

- viscerosomatic (internal organ-to-body)

- somatovisceral (body-to-internal organ)

- somatosomatic (body-to-body)

- viscerovisceral (internal organ-to-internal organ)

The recognition of these physiological relationships expressing signs/symptoms along the longitudinal organization of the body led Ancient Chinese physicians to incorporate systematic

treatment methods to stimulate the external body to achieve a directed restorative response.

JingJin Functional Anatomy

The description of the twelve JingJin distributions in the NJLS 13 (Muscle

Distributions) demonstrates a thorough understanding of the musculo-

fascial-skeletal system.

- Muscles are assigned to longitudinal distributions for each of the

twelve yin-yang anatomical regions of the body. Each of the JingJin

distributions is served, both in terms of blood circulation and neural

innervation, by one of the twelve JingMai distribution vessels.

- When pathology can be attributed to a specific Jing Jin distribution

and the associated Jing Mai vessel, treatment zones can be

LOGICALLY SELECTED.

In the past, DNA has described treating the medial vessels of one extremity to treat both the the medial, as well as the lateral vessels of a different extremity.

- It has been imperative for the DNA method to ascertain a correct understanding of the Chinese physiology in order to establish the most effective baseline for clinical practice.

- The Jing Jin spatial organization of the body provides more effective DNA treatment approaches based on the functional anatomy of the human body.

There is no other treatment method that holds this functional anatomical understanding or specific treatment applications.

In the previous slide you learned the four steps in the DNA method. Steps one through three involve determining the location and region of pain.

Step four involves determining the effective joint and myofascial needling sites. The next several slides will focus on step four and break down the treatments for pain conditions.

Determining effective needling locations for all pain conditions involves three steps.

1. Pain to joint technique (Ipsilateral)

2. Needle the opposite extremity – or – needle the

opposite side arm/leg pair.

3. Needle the same side arm/leg pair

Let’s examine an extremity pain treatment:

DNA steps 1-3 led to diagnosis of left side - anterior lateral shoulder pain.

1. Pain to joint technique (Ipsilateral)- Using our example we are drawing an imaginary line

from the left side, anterior lateral shoulder down the arm to the anterior lateral wrist joint site.

Note: if the pain covers two or more regions, then you will needle all of the corresponding wrist joint sites.

2. Next we have two options:

Option one:The first option is to Needle the opposite extremity in the same or a related joint site. In this case, we would needle the opposite wrist in either the same or a related joint site.

You will also incorporate needles into the myofascial zones for greater effect.

Option Two:The second option is to Needle the opposite arm-leg pair in the same or a related joint site. In this case, we would needle the opposite ankle in either the same or a related joint site.

After needling Option 1 or Option 2 - Be sure to check in with the patient to determine how much their discomfort has improved.

If the patients improvement is less than 70% then you want to then try the option for step two that you haven’t yet used and add in myofascial zones as needed.

So if you used option one but the improvement is not as much as you want, use option two and vice versa.

Let’s examine a torso pain treatment:

DNA steps 1-3 led to diagnosis of left side - lower abdominal pain - anterior lateral torso pain located below the diaphragm. Our last step is to determine the effective needling sites.

***With all torso and head pain, if pain is above the diaphragm – needle the upper extremity first. If pain is below the diaphragm – needle the lower extremity first.

***If the pain is above the diaphragm and bilateral – needle both upper extremities first. If the torso pain is below the diaphragm and bilateral – needle both lower extremities first.
