A variety of resources and referrals are offered to seniors and families in the Claremont

community regarding age-related concerns:

Abuse/Neglect; Referrals to APS/CPS or other programs

Financial Resources; Money Management, Social Security

Homelessness; Emergency Shelter Referrals

Housing Resources

Legal Assistance Referrals; Legal Assistance Program

Medical Resources; Medicare, Medi-Cal

Mental Health Counseling; Tri-City Mental Health

Services are offered Monday through Thursday at the Joslyn Center from 9:00 a.m. to 2:00 p.m.

Real Connections Village (In-Home Aging):

This is a membership program that helps one age happily, healthfully, and successfully in their

own home. The Village model provides connection, community, and a network of resources

that address practical needs, as well as social, cultural, and educational activities. It is designed

to make life less complicated and more fulfilling for seniors aging at home. Call your local village

Retirement Homes/Assisted Living Communities: These are generally large hotel-type buildings that accommodate anywhere from 50-100+

residents. Basic amenities include meals, daily activities, exercise programs, and transportation

to scheduled outings or appointments. Most homes are licensed to provide assisted living and

can offer help with dressing, bathing, escorting to meals, and medications. Some assisted living

facilities also have dementia units. Costs vary depending on location of home and care needs of

the resident.

Skilled Nursing, Alzheimer’s, and Rehabilitation Facilities: These facilities offer skilled nursing care and are licensed by the Department of Health Services.

Licensed trained professionals staff them, such as: registered nurses (RN), licensed vocational

nurses (LVN), and certified nursing assistants (CNA). Many offer occupational, speech, and

physical rehabilitation services. Some will accept patients who have I.V.’s, g-tubes, and/or

tracheotomies. Alzheimer’s facilities offer specialized programs for residents living with

Alzheimer’s disease or other forms of memory loss. Residential, Assisted Living, or Nursing

Facilities can offer these programs.

Adult Day Care: Adult day care provides a secure, professional, and supervised social activity program including

Hospice: Hospice Facilities are specialized to accommodate and support terminally ill patients and their

families. The focus of Hospice care is to provide as much comfort as possible physically,

emotionally, and spiritually. In addition to the medical and nursing care that is offered by most

healthcare programs, Hospice extends bereavement and supportive services to the terminal

individual and their loved ones. Hospice goes where the patient goes, which means hospice is

often at home or at the hospital.
