Narrative: A narrative on the technology planning process to include:

The members of the Pocahontas School District technology committee were chosen because theyrepresent a cross sectionof the faculty and staff in the district. The District Committee is made up ofrepresentatives from each of the four buildings andthe Library Media Specialist from eachbuilding. The Technology Coordinator was included to provide an overall district viewof currenttechnology in place and needs for the future. Additionally the Parental Involvement coordinator from thehigh schoolwas included so that the needs of the patrons of the school could be addressed. Themembers of the committee representvarious different experience levels and each brings their ownunique perspective to the committee.

The committee met and evaluated the previous plan and then began working on updating and revisingthe new plan. One ofthe shortcomings of the previous plan was that there was no evaluationcomponent of the plan and that the plan had not beenrevisited since its last revision and approval.

The technology committee will meet twice a year to evaluate the plan to report progress, makemodifications, and performevaluations on the plan.

Vision Statement

The Pocahontas School District technology vision focuses on preparing the entire learning community to be lifelong learnersand contributing participants in a changing global community by equitably integrating technology throughout the schoolprogram.

We envision classrooms that are connected to each other and the world through technology. Instruction will be accomplishedby the use of a variety of techniques and technologies so that all students achieve goals established by them, their parents,their teachers, and state benchmarks/common core.

Through partnerships and collaboration with our community and all stakeholders, we envision a future where all students willbe able to develop their full potential and continually go beyond the classroom walls.

Current Technology Assessment

A. Technology Integration with Curriculum and Instruction

The Pocahontas School District uses the ACSIP process to identify the curricular strengths and weaknesses. Each year datais collected and disaggregated at each building level and at the district to determine which areas need intervention. Currentlythe focus is in the areas of literacy and mathematics.

A variety of tools are used to address student needs and build upon strengths. At all levels we use Compass Learning to notonly address the academic improvement plans but to also supplementand enrich classroom activities. All classrooms K-6 areequipped with one Internet connected teacher station with a multimedia projector and in room audio system as well as 3stations available to students for remediation or enrichment activities using Compass Learning. Each of these classrooms isalso equipped with a document camera. To address Tier II and III interventions the district has also purchased Math Facts ina Flash, Fastmath and Headsprout.

Classrooms 7-12 have an Internet connected teacher station connected to a multimedia projector. Many 7-12 classroomsalso have document cameras, student response systems, interactive tablets.

Students have access to all technology and utilize it to produce classroom products and involve themselves in the process oflearning.

Student competencies such as those established by the International Society for Technology in Education (ISTE) havebecome a model accepted by practitioners across the country. Standards developed by ISTE are know as the NationalEducational Technology Standards (NETS). These standards delineate specific and meaningful benchmarks for qualityeducational experiences for all children. The standards are conceptual and flexible and may be addressed by variousinstructional strategies through the use of many types of resources.

A logical and appropriate extension of the adoption of the NETS Standards by the Pocahontas Public Schools is thecorrelation of these standards to the Common Core standards. Teachers and staff in each of the content areas shouldcorrelate the technology standards to the Common Core standards and provide examples of specific instructional activitiesthat reflect best practices.

B. Professional Development

Teachers, administrators, and support staff will receive training that will lead to demonstration of proficiency of theinternational Society for Technology in Education (ISTE) Technology Foundation Standards for All Teachers.

Each school campus within the district meeting this goal will demonstrate the following:

A continuum of professional development opportunities that incorporate student-centered learning and demonstratesuccessful integration of technology into teaching and learningCompletion by all instructional and administrative staff of a minimum of six clock hours in professional developmentrelated to integrating technology into the classroomDesignation of staff responsible for developing and implementing professional developmentA variety of experiences, including the train-the-trainer concept to deliver appropriate professional developmentAllocation of at least 20 percent of the district's technology budget for professional developmentAn on-going assessment of staff needs for professional development in technology.Use of information assessed by the Northeast Arkansas Educational Cooperative to determine technology professionaldevelopment

Professional development is central to the success of school reform efforts. Staff development is widely recognized as anessential element contributing to successful implementation of technology programs. Rich technology resources inthemselves will not result in increased student learning if teachers do not have the skills necessary to utilize the technologyresources in the process of learning. Teachers need to be better prepared to integrate technology meaningfully into theirteaching and learning activities. They must adopt efforts where technology is one intervention used to improve classroomteaching and learning.

A process of continuity and appropriateness shall guide the Pocahontas School District as it provides for continualprofessional development to help prepare teachers for their changing roles in a technological environment. In addition toteachers, staff development should also include those that influence student learning (school board, administrators, supportstaff, parents, and other stakeholders). Year round, the school district and the cooperative provide in-service in the use ofboth software and hardware with an emphasis on meeting curricular expectation and differentiation. Using the ACSIPprocess, testing data, and both formal and information assessments, the district determines the effectiveness of professionaldevelopment activities.

FUNDING

The Pocahontas School District has made a firm commitment in the purchase of hardware, software and support. Ongoingfinancial support must also be provided for the sustained delivery of quality professional development in technology use. Anannual allocation for professional development is required to make this happen. Pocahontas School District is committed tothe allocation of at least 20 percent of technology resources for professional development. The allocation must also includefunds for providing opportunities and support for educators to develop model practices using technology.

BEST PRACTICES

The Pocahontas School District is committed to the use of best practices in developing professional development. Manytimes training is too focused on the mechanics and functionality of technologies. This type of training is sometimes referred toas "one shot" instruction. To use technology effectively for learning, teachers need more than mechanical training on how tooperate equipment and software. A new model for staff development allows educators to see actual methods used inclassrooms, try out new techniques and get feedback on their efforts, and observe their peers. This model of staff trainingfocuses on effective strategies to integrate technology into the curriculum, as well as into the teaching and learning process.

For this to occur school personnel need hands-on learning, time to experiment, easy access to equipment, and availability ofsupport personnel who can help them be effective users of technology.

The Pocahontas Public School District in planning professional development will incorporate characteristics of effectivenationally recognized professional development standards. The International Society for Technology in Education (ISTE),National Council for Accreditation of Teacher Education. (NCATE), National Staff Development Council (NSDC), andConsortium for Policy Research in Education (CPRE) all have developed characteristics of effective professionaldevelopment that are philosophically close. Each of these models requires that each individual be responsible for his/her ownprofessional growth. These standards also require that institutions at all levels change the way they approach professionaldevelopment. According to the National Staff Development Council standards, effective staff development takes a variety ofapproaches. The standards call for

C. Equitable Use of Technology

Pocahontas Public Schools will ensure a quality education for all children. Equitable access to technology will be providedregardless of race, ethnic background, gender or special needs. Technology is available throughout the school day in theclassroom, libraries and lab settings. The technology staff is trained in a variety of technologies and is trained on a point ofneed basis to provide support for assistive and adaptive technologies.

The district provides access to the following assistive technologies:

The district also provides the following specialized hardware:Kurzweil reading systems - The Kurzweil system allows students who have difficulty reading to scan a portion of printedmaterials into the computer. The computer will then read the scanned document back to the student in a user selectablevoice.In Room Audio - Classroom audio systems are installed in Kindergarten through sixth grade and in new classrooms atPocahontas High School. The teachers voice and audio from the computer or TV/VCR are amplified and broadcast overthe system at increased volume.

Policies and Procedures

The goal of the Pocahontas School District is to have a seamless infusion of technology in every classroom at every gradelevel. The objective is to provide students with the necessary tools to meet their educational needs and to graduate studentscapable of using technology tools regardless of their post secondary choices.

The strategy to accomplish this is to provide teachers with a process to acquire technology and obtain training and support touse technology.

To meet our goals and objectives the following strategies are used:

Technology tools are used to communicate with and involve parents in the educational process. Some examples are theon-line grade book, school web page, email, phone notification system and online forums.Technology is integrated into curriculum and instruction to help meet AR curriculum frameworks/common corestandards. Examples are document cameras, multi-media projectors, Smart boards, interactive tablets, scientificcalculators, classroom audio systems, student response systems, portable laptop labs, credit recovery through theCompass Learning, distance learning, and technology project-based learning.Technology is used to assist in the delivery of library media services. Examples are a web-based card catalog, on-linedatabases, computer access, and home access to media services.Technology is used to provide the sharing of resources among the school, home, and public library. Students not onlymay access the public library from school, but also may acquire library materials through services provided by theschool. Students 10-12 are also give Google Apps for Education accounts which provide cloud based storage andcollaboration via email and document creation and sharing.Efforts are made to increase/improve technology access for teachers and students. Examples include portable laptoplabs, access to computers in the media centers before, during, and after school, online grades, sets of scientificcalculators, computers the classroom, distance learning, cloud based collaborative document creation via Google Appsfor Education, the Moodle learning environment and staff work-at-home rights for Microsoft applicationsTechnology is used as a tool to assist in the delivery of staff development. Staff may view and register on-line forin-service offerings through the school and the local educational cooperative. Staff communicates through email,teleconferencing, WebEx, and the web for a variety of staff development opportunities.

The Pocahontas School District has adopted the ISTE standards as their guiding standard. Specific technology strategies tomeet the Arkansas Curriculum frameworks/common core standards and to help students achieve individual and schoolimprovement goals are listed in the each school's ACSIP plan as well as the district's plan. The ACSIP technology goals,objectives, and strategies are included in the Implementation component of this plan.

Teachers use a variety of on-line assessments from textbook publishers and other sources to assess classroom progress aswell as classroom response systems in many of the literacy and mathematics classes. The school district also uses testingand demographic data from ORME/NORMES and other sources to assist in the disaggregation of data.

The student and faculty acceptable use policies were prepared in accordance with the Children's Internet Protection Act(CIPA). The policies are approved by the school board and are distributed to each student and staff member to be read andsigned. The policy is also available on the school web site and included in the student handbook and the staff personnelpolicies. The technology director completes ongoing formal and informal security audits to maintain the integrity of thenetwork. Individual authentication helps to safeguard the privacy of data. The district employees the Fortiguard firewall andcontent filter appliance.

User Accounts

All users will sign an agreement acknowledging the Policies and Procedures governing the use of the District network. Indoing so, the user acknowledges that he/she will be liable for any abuse of the network should that abuse be traced to theiraccount. Anything done or stored on the network or district owned equipment is considered district property.

Each account is assigned one user. User accounts may not be shared. The user assigned to the account is responsible forall actions of the account.

Student accounts are grouped by elementary and secondary. Elementary students will not be given individual login accounts.They will use a generic Student account. Elementary students will not have home directories in which to save documents.

Junior/Senior High school students will be assigned individual login accounts once they and their parent/guardian havesigned and returned the District’s Acceptable Use Policy. The password for this account will be set by the TechnologyDepartment and students can not change it. Students will be assigned an account for instructional use. Each student accountis grouped by graduation year. After graduation the student account will be deleted. Student accounts will be retained untilgraduation, but all files in the account’s home directory will be deleted at the end of each school year. The district suppliesstudents with email accounts in grades 10 - 12 only.

Storage on computers and the network is finite. It is the responsibility of the user to delete unneeded or unused files. Shoulda user consume too much space on district owned equipment, the Technology Department reserves the right to remove fileswithout notice.

Faculty and Staff

Faculty members are assigned an account when all needed information can be gathered from the employee after he or shehas been hired. All faculty members are given an email account. There are certain mapped drives on the server that containfolders for various faculty related applications. Faculty is given access to these drives as well. Disk space and responsibilitiesare the same as for students.

Guests

Guests will not be assigned accounts on the Pocahontas School District Network. This includes, but is not limited to: studentteachers, internships, and substitute teachers.

�Sharing Accounts

Any abusive activities initiated from your account will be traced back to you and you will be held accountable. If the abuse issuch that network privileges are terminated the account holder will be held responsible. It is, therefore, policy that thenetwork accounts NOT be shared. Each account has one user.

Changing a Password

Passwords must be changed every 90 days but the user can choose to change his/her password at anytime. Passwordsmust be at least 8 characters and passwords cannot be reused. Passwords must not be shared with anyone. The exceptionto this rule is, it may be necessary to give your password to Technology Staff to aide in the troubleshooting of problems orinstallations of software.

Network Resources

Physical Access to Equipment

Most faculty and students have access to network resources. These resources include printers, workstations, network wiringor connection equipment, etc. Anything that is connected to the network can be considered a resource.

Adding Hosts to the Network

All additions of computers to the network must go through the District Technology Department. This includes computers,laptops, printers, and any other “intelligent” machine. Network hardware/host installation and maintenance will be handledonly by authorized staff.

Hardware/Software Acquisition

All requests for additional hardware and software must be made to the Technology Department. All hardware and softwaremust be approved by the Technology Coordinator and Administration. Installation of unauthorized software is strictlyforbidden. Where possible, workstation permissions will prevent the execution of software installation packages.

Rights and Responsibilities

Use of Licensed Software

There is a large quantity of copyrighted and licensed software available for use on the network. Typically, every applicationhas a different licensing method. Some software may be licensed to a particular machine or person. Other software mayhave a “floating node” license that limits the number of concurrent users. Some applications may be such gross consumers

of network resources that their use is restricted to particular machines or users.

Copyrighted and licensed software and documentation may not be duplicated unless it is explicitly stated thatyou may do so.When in doubt, DO NOT COPY.

�Use of Storage Resources

Print Resources

Many printers are available to network users for print services. The types of printers vary from inkjet printers to color laserprinters.

District network printers shall only be used for printing information which is used for instructional, research or educationalpurposes. All other use is prohibited. Abuse of network printer resources is defined as:

Printing multiple copies of the same document; use copy machines insteadLoading or printing any media for which the printer is not designed to use; prohibited material includes some resumepaper, transparencies, envelopes, labels, etc.Any activity which could harm the printer or serverAny activity which would deny the service of the printer to others

The use of e-mail is encouraged for inter-office communications. E-mail is very easy to use and has the added benefit ofproviding detailed information as to when the message was submitted. See the section on email use for more information.

Abuse of Network Resources

Every means available to the District will be used to detect, restrict and/or prosecute individuals responsible for the abuse ofnetwork resources. This section serves to provide specific examples of the types of abuse not tolerated. This list is by nomeans complete and is subject to change without notice as new ways of abusing resources are discovered. Penalties forabuse of network resources include, but are not limited to, temporary restriction of network privileges, permanent restrictionof network privileges, suspension/termination and criminal prosecution.

Governor’s Policy Directive

Governor’s Policy Directive GPD-5, 1997 (http://www.state.ar.us/governor/gpd5.html) clearly states that … “Use of any andall State-owned equipment and supplies shall be restricted to official state use only. Unauthorized or personal use ofequipment or supplies may be grounds for dismissal.”

Theft and Vandalism

Theft and vandalism of network resources will be handled by the appropriate authorities (local/State/Federal Police with therequired jurisdiction). The District will pursue and support criminal prosecution of individuals suspected of theft and/orvandalism.

Unauthorized Use of Network Services

Anyone, for whom an active account does not exist, found using any District network services without permission from thebuilding principal’s or Technology Department will be asked to leave the campus immediately. If direct expenses areincurred by the District during unauthorized use (ex. Paper, printer supplies, etc.) the District reserves the right to pursue fullreimbursement of those costs from the individual. If unauthorized users are using network resources to commit an unlawfulact then the proper authorities will be contacted.

Use of restricted network services without authorization is considered an abuse of privilege and may result in restriction ordenial of network access.

Breaking into Accounts

Any attempt to gain access or to use an account other than by the owner will be considered a severe violation of networkpolicy. Such attempts include, but are not limited to, gaining access to a user’s account while the user is away from theworkstation or efforts to determine another user’s password by closely watching a login. If you find a machine that is loggedin, but the user is nowhere to be found, log that user out.

Possession of tools which can be used to subvert security is grounds for account suspension.

Cracking Passwords

Any attempt to crack or otherwise obtain passwords is prohibited. Storing or transferring encrypted or unencrypted passwordinformation is prohibited. Writing, transferring, compiling, or running programs designed to guess passwords or otherwisegain unauthorized access to user or system accounts or passwords is prohibited. This includes programs or techniquesdesigned to trick users into divulging their password.

Sharing Accounts

An account is assigned to an individual. That individual is solely responsible for all actions traced to the account. Sharingaccounts or account passwords is prohibited.

Modifying Files

Modifying files anywhere on the system without consent of the files’ owner is prohibited. This includes writing or modifyingfiles which have file permissions set to allow modification or writing. This also includes creating new files, renaming, ordeleting existing files in directories which may have directory permissions set to allow creation or modification of files. Whenin doubt, do not write.

Receipt and Distribution of Copyrighted Material

Use of network services for the receipt, distribution, or use of any copyrighted software or material, which includes music,without the express consent of the copyright owner is prohibited.

Personal and Financial Use of Resources

Use of the District network resources for personal or financial purposes is prohibited.

Licensing and Copyright Infringement

Most software packages and applications are licensed and/or copyrighted. Most licenses and copyright agreementsspecifically prohibit copying or unauthorized use of the software or data. When in doubt, do not copy.

File Storage

There is a sufficient amount of disk storage available to users on the network. User accounts are set up such that each userhas their own personal drive space for storing files. These directories are spread out among different file servers. There is alimited amount of free space on these drives. Therefore, users are encouraged to control their disk usage by archiving anddeleting old or unused files. Users should also try to avoid duplicating files which are available elsewhere on the network

Internet

Purpose of Internet Access

The Internet provides a wealth of information useful for educational purposes. With Internet access an employee can utilizethe many research and resource tools available online. These tools can aid in preparing lessons or projects for classroomuse.

Substitute Teachers and Use of Internet

Substitute teachers are not to use the Internet or allow students to use the Internet unless specifically instructed by theregular classroom teacher. Long term substitutes may use the Internet and/or supervise students’ use of the Internet onlywith permission of the building principal. The principal may grant such permission only if he/she deems that the substitutehas sufficient expertise and understanding of district policy.

Filtering of Internet Content

The Pocahontas School District utilizes the Fortiguard Firewall and Filter appliance, which is extremely helpful in blockingaccess to inappropriate material. However, the nature of the Internet is global and dynamic. Therefore, users (and parents ofminor users) must understand that neither the Pocahontas School District nor any district staff member controls the contentof the information available on systems outside the district network that might be accessed via the Internet.

Electronic Mail and Communications

Purpose of E-mail

Electronic mail is provided to support open communication and the exchange of information between staff and otherappropriate entities that have access to a network. This communication allows for the collaboration of ideas and the sharingof information. E-mail is a necessary component of teamwork at Pocahontas School District. Due to the design goals of theInternet, electronic mail security is not very strong. Therefore e-mail should not be used to transfer secure or confidentialinformation. Employees should be advised that under Arkansas law, both e-mail and computer use records maintained bythe district are subject to disclosure under the Freedom of Information Act. It should be noted that Pocahontas School Districtdoes not archive emails.

Electronic Mail Storage

Staff should move important information from E-mail message files to shared folders and drives to ensure proper backup.Messages no longer needed must be purged from personal storage areas. Please set up Email accounts to permanentlydelete mail upon exiting. Technical staff will monitor storage usage and advise when limits are reached and purging isrequired.

In the event that an individual fails to use an inbox responsibly, the District, without notice, reserves the right to:

1. Archive and delete any inbox that has not been accessed during the past nine weeks.2. Move the entire contents of an inbox to the owner’s home directory and notify the owner by email of their location.3. Extract and delete large mail messages from mail lists.4. Extract and delete all messages from large volume mail-lists over 30 days old.5. Delete messages in an inbox that are more than one semester old.

Electronic Mail Privacy

Electronic mail should be considered private in the sense that U.S. Mail is. Do NOT attempt to read, copy, or otherwisedisturb another user’s e-mail. The technology department reserves the right to inspect an individual’s mail and/or accountshould that user be suspected of a crime or account abuse.

Electronic Communications Privacy Act

Electronic mail is covered under the Electronic Communications Privacy Act of 1986. This act provides for prosecution ofindividuals found capturing, reading or altering another’s e-mail without permission.

Controversial Mail/Postings

Electronic mail is delivered directly to a user’s account. Mail deemed obscene, or otherwise abusive by the recipient will beconsidered an abuse of network privileges. Do not send chain letters or hoaxes on school email. Do not send, forward, oropen executable files (.exe) or other attachments unrelated to specific work activities, as these frequently contain viruses.Refrain from sending unnecessary or large attachments exceeding 1.5 MB in size. Do not e-mail any messages you wouldnot be willing to sign and put in the mail.

�Forging

Any attempt to forge an e-mail message will be considered an abuse of network privileges. If a user receives mail that couldhave been forged, it is in the best interest of all parties involved to confirm the e-mail with the supposed sender via personalcontact. If it is determined that the e-mail is a forgery, contact the technology department.Please save a complete copy of the message for further investigation.

Other Forms of Abuse

Worms and Viruses

Anyone knowingly attempting to reproduce, write, or ftp worms or viruses of any size, shape, or form will be remanded forcriminal prosecution.

System Administrator’s Responsibilities

Systems administrators are held to a higher standard than the average user because they have the capability andresponsibility to maintain system integrity. Thus systems administrators must be entrusted with the security and privacy of all

data on the network.

Privacy Considerations

Systems administrators have access to users’ private information. Systems administrators are required to protect theconfidentiality and integrity of this information.

Liability

Systems administrators are not liable for any loss of data or loss of service on the District network.

Investigation of Violations

Systems administrators are charged with investigating violations of the District network policy. During such investigations, thesystems administrators may have complete access to all data on the District network as needed for the investigation.

Enforcement

Temporary Access Restriction

An individual account’s access to the District network may be temporarily restricted due to many reasons,including:

Maintenance or servicing of network resourcesInvestigation of network policy violation

Temporary access restrictions are intended to be short lived and usually require the account’s owner to contact networkservices for reactivation. Note that investigations of network policy violations may require any number of potentially affectedaccounts to be temporarily restricted. The owner of the account may not be the object of the investigation if, for example, itmay be suspected that the user’s password has been cracked by a third party.

Permanent Access Restriction

Permanent access restrictions must be approved by the Administration. All accounts assigned to a user maybe restricted andfuture network privileges denied.

Reporting Problems

Physical Security

Physical security is the most important part of system security. Obviously electronic security means nothing if the wholemachine is stolen. Users should be aware of what networked machines are in their vicinity and keep an eye out for anysuspicious activity. Doors to labs should be closed and locked if there is no one to supervise use in the lab.

Theft and Vandalism

Theft and vandalism should be reported to the building principal, Superintendent’s office, and Technology Department.

Electronic Security

Electronic security is also the responsibility of all users. Users should periodically examine their files for unusual activities.Contrary to popular belief, systems administrators are not omniscient or omnipresent, so suspicious activities are oftenreported first by wary users. Depending on the nature of the violation, electronic mail or telephone is the best method ofalerting the system administrator.

Broken Machines

Broken machines or other hardware/software problems should be reported on a District Repair Form. The technologydepartment will not address problems/repairs until the paperwork is completed. No attempts beyond basic steps (restarting,checking plugs, ink, etc.) should be made by personnel to resolve technology related problems.Student Confidentiality

No images of a child shall be posted on any school sponsored website without explicit written permission of theparent/guardian of that child. No personal information is to EVER be posted about a child on a school sponsored website (ex.Name, age, grade, parents name, address, etc.). Group pictures may be used of students only if the students are in a public

place where anyone could take a picture and no identifying personal information is given on any of the students in thepicture.

No personal or identifying information of a child is to ever be posted on a school sponsored website. Persons found postingthis type of information will be considered in violation of this policy.

Technology Infrastructure

The Pocahontas School District computer network is divided into two distinct networks. Each network is managed by thetechnical support staff. With support from the Superintendent and the School Board we have been able to stay ahead ofnetwork growth thus far by building up the network before a large influx of technology cause degraded performance. Theschool network security policy complies with the state supplied IT Security Policy.

Pocahontas High, Junior High and MD Williams Intermediate Schools

The larger of the two local area networks (LAN) connects the campuses of MD Williams Intermediate, Pocahontas JuniorHigh and Pocahontas High School and all the outlying buildings on the main campus including the District AdministrationBuilding. The buildings are connected with 1 GB fiber connections to the network core switch located in the BusinessEducation Building on the High School Campus. Internally the buildings are wired with GB copper connections with two dropsper classroom with drops terminating in strategically located wiring closets. Wireless B/G/N access coverage is located in allbuildings across with the main controller being connected to the core switch located in the Business Education building. Thenetwork is a Windows PC based network with the average age of systems at about 2 years. We are currently developing areplacement plan for aging technology items. In 2011 Carl Perkins funding was used to add a lab of 24 iPad2 tablet devicesto our Business Education program.

The campus is connected to the Internet via two bonded T1 lines, 1 10 MB/s Cable connection and 1 6 MB/s DSLconnection. The load is balanced across these 3 connections via the Fortigate 200B appliance which provides firewall,redundancy, load balancing and a CIPA compliant content filter.

Alma Spikes

The other local area network is made up of 1 GB fiber connections connected to a core switch located in the network closetin the back of the library. Each building is also covered by B/G wireless access connected to a controller also at the networkcore. The internal wiring of the building is 1 GB copper connections with 2 drops per classroom terminating at strategicallylocated wiring closets.The network is a Windows PC based network with the average age of systems at about 3 years. Weare currently developing a replacment plan for aging technology items.

The campus is connected via a single T1 line and 1 10 MB/s Cable connection. The load is balanced across these 2connections via theFortigate 200B appliance which provides firewall,redundancy, load balancing and a CIPA compliantcontent filter.

Compass Learning Software

A remediation lab placed in each school in the district. These labs provide students who scored basic or below basic accessto software that will help them improve in those areas. We are using the Compass Odyssey learning system at each level. Inaddition to remediation, teachers have access to the software in their classrooms and can use the software for enrichment.Kindergarten through sixth grade classrooms have 3 additional computer stations which can provide additional access foradditional in class remediation or enrichment depending on the needs of the students. Learning paths are created from theNWEA assessment results, by the RTI coordinator for remediation or by the teacher mapped to in classroom pacing guides.

Mobile Labs

The district has mobile labs available to classrooms on the High School, Junior High and M.D. Williams Campus. Each labcontains 30 laptop computers and a high speed laser printer. These labs provide the ability for whole classroom instructionand research utilizing online resources purchased from various vendors as part of the Arkansas Traveler Project or theInternet. Each laptop includes the full Office Suite from Microsoft. There are 5 labs at Pocahontas High School and 2 each atPocahontas Junior High and M.D. Williams.

Computer Labs

In addition to the remediation labs Pocahontas Junior High has two general use labs that can be scheduled through thelibrary for class projects using both local and Internet resources.

Telephone Capacity

The district has an aging phone system that provides telephone access in offices and other strategic locations throughout thedistrict. It allows for dial by extension and voice mail and provides detailed call accounting information that is maintained inthe central office. We have 10 incoming lines with access to the system secured by dialing codes. Public access phoneshave no dial codes and emergency calls are routed directly out of the system. The number of classroom telephones is very

limited. We are currently evaluating a new system that would allow for a phone to be placed on each teachers desk thatwould replace the existing system as well as the school intercom system.

Distance Learning

The district has a distance learning platform installed in the Business Education Building on the Pocahontas High SchoolCampus. The system was manufactured by Tandberg. We are currently using the system for professional development andvirtual field trip opportunities. We are exploring other enrichment opportunities to add additional classes to our High Schoolschedule. We have used it for this purpose for several years in the past.

Support Staff

The district employees has two employees working in the technology department, the technology director and a supportspecialist. Both positions are 12 month positions. The challenge each year is getting work orders and new technology installscompleted in a timely manner. The network is growing at a very rapid pace and it is often difficult to keep up with existingtechnology as well as evaluate new technologies with only two full time staff members. The staff members attend technicalworkshops and training throughout the year to stay up to date on current technology.

E-Rate

The E-Rate program has allowed us to add additional Internet bandwidth at a considerable savings to the district as wellmonthly savings on local and long distance services. In addition the reimbursement moneys we receive in the form ofdiscounts is ear marked for technology purchases each year.

Technology Plan Evaluation

Each action in the school ACSIP plan that addresses technology has a timeline and a method of evaluation. Those methodsinclude checklists, data collection, surveys, and test data. The plan will be evaluated annually. Each school's ACSIPcommittee reviews their school improvement plan on an ongoing basis. Feedback is given to make changes to thetechnology plan as needed.

School District Acceptable Use Policy

Employee Internet Use Agreement

The Pocahontas School District agrees to allow the employee identified above ("Employee") to use the district's technologyto access the Internet under the following terms and conditions:

1. Conditional Privilege: The Employee's use of the district's access to the Intemet is a privilege conditioned on theEmployee's abiding by this agreement.

2. Acceptable Use: The Employee agrees that in using the district’s Internet access, he/she agrees to obey all federal andstate laws and regulations. Internet access is provided as an aide to employees to enable them to better perform theirjob responsibilities. Under no circumstances shall an employee’s use of the district’s Internet access interfere with, ordetract from, the performance of his/her job related duties.

3. Penalties for Improper Use: If the Employee violates this agreement and misuses the Internet, the Employee shall besubject to disciplinary action up to and including termination.

4. “Misuse of the District's access to the Intemet" includes, but is not limited to, the following:

1. using the Internet for any activities deemed lewd, obscene, vulgar, or pornographic as defined by prevailing communitystandards;

2. using abusive or profane language in private messages on the system; or using the system to harass, insult, or verballyattack others;

3. posting anonymous messages on the system;4. using encryption software;5. wasteful use of limited resources provided by the school including paper;6. causing congestion of the network through lengthy downloads of files;7. vandalizing data of another user;8. obtaining or sending information which could be used to make destructive devices such as guns, weapons, bombs,

explosives, or fireworks;9. gaining or attempting to gain unauthorized access to resources or files;

10. identifying oneself with another person's name or password or using an account or password of another user withoutproper authorization;

11. using the network for financial or commercial gain without district permission;12. theft or vandalism of data, equipment, or inteIIectual property;13. invading idle privacy of individuals;14. using the Internet for any illegal activity, including computer hacking and copyright or intellectual property law violations;15. introducing a virus to, or otherwise improperly tampering with the system;16. degrading or disrupting equipment or system performance;17. creating a web page or associating web page with the school or school district without proper authorization;18. attempting to gain access or gaining access to student records, grades, or files of students not under their jurisdiction;19. providing access to the District’s Internet Access to unauthorized individuals;20. taking part in any activity related to Internet use which creates a clear and present danger or the substantial disruption

of the orderly operation of the district or any of its schools.21. making unauthorized copies of computer software;22. personal use of computers during instructional time.

5. Liability for debts: Staff shall be liable for any and all costs (debts) incurred though their use of the computers or theInternet including penalties for copyright violations.

6. No Expectation of Privacy: The Employee signing below agree that in using the Internet through the District's access,that he/she waives any right to privacy the Employee may have for such use. The Employee agrees that the district maymonitor the Employee's use of the District's Internet Access and may also examine all system activities the Employeeparticipates in, including but not limited to e-mail, voice, and video transmissions, to ensure proper use of the system.

Student Internet Agreement

The Pocahontas School District agrees to allow the student identified above ("Student") to use the district's technology toaccess the Internet under the following terms and conditions:

1. Conditional Privilege: The Student's use of the district's access to the Intemet is a privilege conditioned on the Student'sabiding to this agreement. No student may use the district's access to the Internet unless the Student and his/her parentor guardian have read and signed this agreement.

2. Acceptable Use: The Student agrees that he/she will use the District's Internet access for educational purposes only. Inusing the Internet, the Student agrees to obey all federal and state laws and regulations. The Student also agrees toabide by any Internet use rules instituted at he Student's school or class, whether those rules are written or oral.

3. Penalties for Improper Use: If the Student violates this agreement and misuses the Internet, the Student shall be subjectto

Personally identifying information includes full names, address, and phone number.17. using the network for financial or commercial gain without district permission;18. theft or vandalism of data, equipment, or inteIIectual property;19. attempting to gain access or gaining access to student records, grades, or files;20. introducing a virus to, or otherwise improperly tampering with the system;21. degrading or disrupting equipment or system performance;22. creating a web page or associating web page with the school or school district without proper authorization;23. providing access to the District’s Internet Access to unauthorized individuals;24. failing to obey school or classroom Internet use rules; or25. taking part in any activity related to Internet use which creates a clear and present danger or the substantial disruption

5. Liability for debts: Students and their cosigners shall be liable for any and all costs (debts) incurred though the student'suse of the computers or the Internet including penalties for copyright violations.

No Expectation of Privacy: The Student and parent/guardian signing below agree that if the Student uses the Internetthrough the District's access, that the Student waives any right to privacy the Student may have for such use. TheStudent and the parent/guardian agree that the district may monitor the Student's use of the District's Internet Access

No Guarantees: The District will make good faith efforts to protect children from improper or harmful matter which maybe on the Internet, At the same time, in signing this agreement, the parent and Student recognize that the District makesno guarantees about preventing improper access to such materials on the part of the Student.

Department of Information Systems

E-Rate Letter of Agency

The Participant, (Cooperative/District/School/Library Name) POCAHONTAS SCHOOL DISTRICT  authorizes the Departmentof Information Systems (DIS), State of Arkansas, to act as a consortium lead on its behalf in matters relatedspecifically to any State and Federal discount programs for the purpose of securing E-rate discounts.

The Participant authorizes DIS to engage on its behalf in the procurement process, in the submission of FCC Form 470, FCC Form 471, and other E-rate forms, and in the overall administration of these programs solely for the purpose of facilitating application for those discounts on E-rate eligible services, which shall include, but not be limited to, Telecommunications Services, Internet Access, Internal Connections, and Internal Connections Basic Maintenance.

I, the authorized signer, understand that DIS will be making certifications on behalf of the Participant. By signing this letter of agency, I make the following certifications:

a. I certify that the schools or educational service cooperatives in the Participants district are all schools under the statutorydefinitions of elementary and secondary schools found in the No Child Left Behind Act of 2001, 20 U.S.C.§§7801(18) and(38), do not operate as for-profit businesses, and do not have endowments exceeding $50 million.

b. I certify that the libraries or library consortia in the Participants system are eligible for assistance from a State LibraryAdministrative Agency under the Library Services and Technology Act of 1996, Pub. L. No. 104-208,§211 et seq., 110Stat. 3009 (1996), do not operate as for-profit businesses, and have budgets that are completely separate from any school(including, but not limited to, elementary schools, secondary schools, colleges, or universities).

c. I certify that the schools, educational service cooperatives, libraries, or library consortia in the Participants district havesecured access, separately or through this program, to all of the resources, including computers, training, software,internal connections, maintenance, and electrical capacity, necessary to use the services purchased effectively. Irecognize that some of the aforementioned resources are not eligible for support. I certify that the entities I represent orthe entities listed in this application have secured access to all of the resources to pay the discounted charges for eligibleservices from funds to which access has been secured in the current funding year. I certify that the Billed Entity will paythe non-discount portion of the cost of the goods and services to the service provider(s).

d. I certify that all of the schools, educational service cooperatives, libraries, or library consortia in the Participants districtare all covered by technology plans that are written, that cover all 12 months of the funding year, and that have been orwill be approved by a state or other authorized body, and an SLD-certified technology plan approver, prior to thecommencement of service.

e. I certify that the schools, educational service cooperatives, libraries, or library consortia in the Participants district arecompliant, or will be compliant at the time funded services are provided, with the Childrens Internet Protection Act, Pub.L. 106-554 (2000).

f. I certify that the services the school, educational service cooperative, library, library consortia or district purchases atdiscounts provided by 47 U.S.C. Sec. 254 will be used solely for educational purposes and will not be sold, resold, ortransferred in consideration for money or any other thing of value, except as permitted by the Commissions rules at 47C.F.R. Sec. 54.500(k). Additionally, I certify that the Billed Entity has not received anything of value or a promise ofanything of value, other than services and equipment requested under this form, from the service provider(s) or anyrepresentative or agent thereof or any consultant in connection with this request for services.

g. I certify that the entities eligible for support that I am representing will retain copies of all documents related to E-Rateapplications and funding use for a period of five years from the last date of service covered by this letter of agency.Documents to be retained include, but are not limited to: E-rate forms, technology plans, discount rate support, competitivebids received, bid selection results, inventory records, invoices, and payments.

h. I certify that the entities eligible for support that I am representing have complied with all applicable state and local lawsregarding procurement of services for which support is being sought.

i. I certify that the Participants district, educational service cooperative, library, or library consortia has complied with allE-rate program rules, and I acknowledge that failure to do so may result in denial of discount funding and cancellation offunding commitments.

j. I understand that the discount level used for shared services is conditional, for future years, upon ensuring that the mostdisadvantaged schools and libraries that are treated as sharing in the service receive an appropriate share of the benefitsfrom those services.

k. I certify that no kickbacks were paid to anyone and understand that false statements on this form may be punished by fineor forfeiture under the Communications Act, 47 U.S.C.§§502, 503(b), may subject me to a fine and imprisonment pursuant

Note that this form must be used beginning with Funding Year 2001 (the Funding Year beginning July 1, 2001) and later. II. IMPACT OF CIPA REQUIREMENTS ON FORM 479 The Children's Internet Protection Act (CIPA) was signed into law on December 21, 2000. In order to receive discounts for Internet Access and Internal Connections services under the universal service support mechanism, school and library authorities must certify that they are enforcing a policy of Internet safety. The Internet safety policy must include measures to block or filter Internet access for both minors and adults to certain visual depictions. Beginning with Funding Year 2001 (the Funding Year beginning July 1, 2001), if the Billed Entity who represents an Administrative Authority is applying for Internet Access or Internal Connections, the Administrative Authority must certify on a Form 479 that it is in compliance with the requirements of CIPA or that it is undertaking such actions to comply with the requirements of CIPA prior to the start of the following Funding Year. (If the Administrative Authority is the Billed Entity, the Administrative Authority will certify on the Form 486.) The Billed Entity must receive your completed and signed Forms 479 before it can complete and postmark its Form 486. If the Billed Entity is applying only for Telecommunications Services, the requirements of CIPA do not apply and the Billed Entity is not required to collect Forms 479. (For more information, see the Children’s Internet Protection Act, as codified at 47 U.S.C. § 254(h) and (l).) Applying for funds. For the purpose of CIPA requirements, a school or library that is a recipient of service is considered to have applied for funds in a Funding Year only when a Form 486 for a Funding Request for Internet Access, Internal Connections or Basic Maintenance has been successfully data entered and USAC has issued a Form 486 Notification Letter. Determination of Your First Funding Year for Purposes of CIPA. The first Funding Year after Funding Year 2000 (the Funding Year beginning July 1, 2000) in which a school or library applies for funds (i.e. in which a Form 486 is successfully data entered for Internet Access, Internal Connections or Basic Maintenance and USAC issues a Form 486 Notification Letter) is the First Funding Year for purposes of CIPA. Once your First Funding Year is established, the next two funding years will be your second and third funding years for purposes of CIPA. (See “Applying for Funds” above.) In the first Funding Year, the applicant must be in compliance with CIPA or undertaking actions to comply with CIPA, in order to receive discounts for Internet Access, Internal Connections or Basic Maintenance services. Once the First Funding Year is established, the Funding Year immediately following the First Funding Year becomes the Second Funding Year for purposes of CIPA. If the school or library applies for funds for Internet Access, Internal Connections or Basic Maintenance in the Second Funding Year, it must certify that it is in compliance with CIPA unless state or local procurement rules or regulations or competitive bidding requirements prevent the making of the certification otherwise required. A school or library so prevented may request a waiver for the Second Funding Year. The Third Funding Year for purposes of CIPA is the Funding Year immediately following the Second Funding Year. If the school or library applies for funds for Internet Access, Internal Connections or Basic Maintenance in the Third Funding Year, it must be in compliance with CIPA. You must be in compliance with CIPA for any Funding Year thereafter. The following situations WOULD constitute the Administrative Authority’s First Funding Year: The Billed Entity submits a Form 486 for Internet Access, Internal Connections or Basic Maintenance, the Form 486 is

successfully data entered and USAC issues a Form 486 Notification Letter, but the Billed Entity cancels all of its Funding Requests on a Form 500.

The Billed Entity submits a Form 486 for Internet Access, Internal Connections or Basic Maintenance, the Form 486 is successfully data entered and USAC issues a Form 486 Notification Letter, but the service provider does not receive a corresponding disbursement.

The following situations WOULD NOT constitute the Administrative Authority’s First Funding Year: The Billed Entity receives a Funding Commitment for Internet Access, Internal Connections or Basic Maintenance, but

takes no further action. The Billed Entity receives a Funding Commitment for Internet Access, Internal Connections or Basic Maintenance,

submits Form 486, but the Form 486 is not successfully data entered and no Form 486 Notification Letter is issued. The Billed Entity applies only for Telecommunications Services.

III. SUBMISSION REQUIREMENTS AND GENERAL INSTRUCTIONS

Who Must Submit the Form 479? In general, the Administrative Authority is the entity that must complete Form 479. For a school, the Administrative Authority may be the school, school district, school board, local educational agency, or other authority with responsibility for administration of the school. For a library, the Administrative Authority may be the library, library board, or other authority with responsibility for administration of the library. If the Administrative Authority is also the Billed Entity, the Administrative Authority certifies on the Form 486. If the Administrative Authority is not the Billed Entity, the Administrative Authority must complete Form 479, and submit the Form 479 to the Billed Entity. The Billed Entity then certifies on Form 486 that it has collected duly completed and signed Form(s) 479. How Many Forms 479 to Submit to the Billed Entity? Generally, you will complete one Form 479 for each of your Billed Entities. When to Submit the Form 479? The Billed Entity must receive your completed and signed Forms 479 in time to postmark the Billed Entity’s Form 486 to meet Form 486 deadlines. You should work with your Billed Entity to determine when to submit your Form 479. You must complete Form 479 for each Funding Year in which you receive discounted services beginning with Funding Year 2001 (the Funding Year beginning July 1, 2001) and after. SPECIAL NOTE FOR LIBRARIES AND CONSORTIA CONTAINING LIBRARIES FOR FUNDING YEAR 2003: On July 24, 2003, the FCC released an Order (FCC 03-188) to bring its implementation of CIPA into compliance with the recent United States Supreme Court decision on CIPA. The Order states that library consortium members who filed the September 2002 version of the Form 479 for FY2003 funding requests prior to the effective date of the Order must file the revised Form 479 with their billed entity within 45 days after the effective date of the Order. USAC expects the effective date of the Order to be in August 2003. To determine the effective date of the Order and the deadline for refiling Form 479, applicants should check the USAC website or contact the Client Service Bureau (see “Where to Get More Information?” below). Note to Libraries in Funding Years 2003 and 2004.

For Funding Year 2003, libraries must be in compliance with CIPA or undertaking actions to comply with CIPA in order to receive discounts for Internet Access, Internal Connections or Basic Maintenance.

If a library that applied for funds for Internet Access, Internal Connections or Basic Maintenance in Funding Year 2003 applies for funds for Internet Access, Internal Connections or Basic Maintenance in Funding Year 2004, it must certify that it is in compliance with CIPA unless state or local procurement rules or regulations or competitive bidding requirements prevent the making of the certification otherwise required. A library so prevented may request a waiver for Funding Year 2004. (See “Applying for funds” above and the instructions for Item 6e below.)

If a library has applied for funds for Internet Access, Internal Connections or Basic Maintenance in both Funding Year 2003 and Funding Year 2004, it must be in compliance with CIPA in Funding Year 2005 and any Funding Year thereafter. (See “Applying for funds” above.)

Libraries that apply for funds for Internet Access, Internal Connections or Basic Maintenance for the first time in Funding Year 2004 or any subsequent year should follow the procedures in the “Determination of Your First Funding Year for Purposes of CIPA” above.

Where to Send Form 479? The Form 479 must be filed with your Billed Entity. DO NOT SUBMIT THE COMPLETED FCC FORM 479 TO THE SCHOOLS AND LIBRARIES DIVISION OR TO THE FCC; SUBMIT IT ONLY TO YOUR BILLED ENTITY.

Anyone filing false information is subject to penalties for false statements, including fine or forfeiture, under the Communications Act, 47 U.S.C. §§ 502, 503(b), or fine or imprisonment under Title 18 of the United States Code, 18 U.S.C. § 1001. All of the information required in the Form 479 must be completed in order for the Billed Entity to be able to certify as required on the FCC Form 486.

IV. SPECIFIC INSTRUCTIONS Type or clearly print in the spaces provided. Attach additional pages if necessary. Administrative Authority’s Form Identifier: Use this space to assign a number or letter of your own devising to facilitate communication with your Billed Entity about THIS particular Form 479. Your Administrative Authority’s Form Identifier can be very simple; for example, if you are filing three Forms 479, you might label them “A,” “B,” and “C.” The Administrative Authority’s Form Identifier can also be descriptive, such as “Internet.” Choose identifiers that suit your own record keeping needs. A. Block 1: Administrative Authority Information Block 1 of Form 479 asks you for your name, address, and basic identification information. Item 1 - Provide the name of the Administrative Authority. Item 2 - Provide the Funding Year (e.g., 2001 –2002) for which funds were requested or approved and for which the Billed Entity will be submitting a Form 486. Program Funding Years begin on July 1 and end on June 30. For example, Funding Year 2001 runs from July 1, 2001 through June 30, 2002. Cite only one Funding Year in this item. Item 3 - Provide the Administrative Authority’s full mailing address, whether a street address, Post Office Box number, or route number. Provide the Contact Person’s Name; 10-digit telephone number including the area code; fax number; and e-mail address. B. Block 2: Certifications and Signature Block 2 of Form 479 asks you to certify with respect to your status as the Administrative Authority and with respect to your compliance with the Children’s Internet Protection Act. Item 4 – Certify that you are an Administrative Authority for one or more schools or libraries for which Universal Service Support Mechanism discounts have been requested or approved for eligible services. You must make the required certification(s) for the purposes of the Children’s Internet Protection Act in order to receive discounted services. While the Billed Entity will not submit the Form 486 until after discount requests have been approved and a Funding Commitment Decision Letter (FCDL) has been issued, the Billed Entity may ask you to file the Form 479 in advance of approval of the requests so that the Billed Entity is ready to file the Form 486 once the FCDL has been received. Item 5 – Certify to your recognition that you may be audited pursuant to this certification and will retain for five years any and all records that you rely upon to complete this form. Item 6 - This item concerns compliance with the requirements of the Children’s Internet Protection Act (CIPA), as codified at 47 U.S.C. § 254(h) and (l), and the implementing rules as codified at 47 C.F.R. § 54.520. Check the box that describes the status of compliance with the Children’s Internet Protection Act. The certification statements in Item 6 of the Form 479 should be read as if you are addressing them to the Billed Entity.

An Administrative Authority can certify Item 6b only in the first Funding Year after Funding Year 2000 (the Funding Year beginning July 1, 2000) for which it is applying for discounts for Internet Access or Internal Connections services. An Administrative Authority can seek a CIPA Waiver under Item 6d only in its Second Funding Year. If you request a CIPA Waiver, you certify that your schools or libraries will be in compliance before the start of the Third Funding Year. You cannot seek a CIPA Waiver in your Third Funding Year, even if you have a new school or library branch/outlet under your authority in that Funding Year. (See “Impact of CIPA Requirements on Form 479” above for a discussion of First, Second and Third Funding Years.) Note that libraries can request a CIPA Waiver under Item 6e in Funding Year 2004. (See the instructions for Item 6e below.) Schools and libraries receiving E-rate discounts are required to have an Internet safety policy that addresses: (1) access by minors to inappropriate matter on the Internet and World Wide Web, (2) the safety and security of minors when using electronic mail, chat rooms, and other forms of direct electronic communications, (3) unauthorized access, including hacking, and other unlawful activities by minors online, (4) unauthorized disclosure, use, and dissemination of personal identification information regarding minors, and (5) measures designed to restrict minors’ access to materials harmful to minors. There is also a public notice and hearing/meeting requirement. For example, if in Funding Year 2001 you certify that you are undertaking such actions necessary to put the measures required by CIPA in place for Funding Year 2002, then in order to obtain discounts in Funding Year 2002, you must fully implement the measures prior to the start of services in that Funding Year, unless you obtain a waiver as a result of procurement/competitive bidding constraints.

NOTE THAT the certification language in the Form 479 is not intended to fully set forth or explain all the requirements of CIPA. Item 7 requires the signature of the Authorized Person. Item 8 - Enter the date the Form 479 was signed. This date must include the month, day and year. Item 9 - Print the name of the Authorized Person whose signature is provided in Item 7. Item 10 - Provide the title or position of the Authorized Person whose signature is provided in Item 7. Item 11 - Provide the telephone number, including area code, of the Authorized Person whose signature is provided in Item 7. This information is required in order to expedite any communications that may be necessary between the Authorized Person responsible for signing the Form 479 and the Billed Entity. Submit completed forms to your Billed Entity. V. REMINDERS • The person authorized to provide the information required by Form 479 must sign and date Form 479. • Provide data for all information items. • This Form 479 is applicable to you only if you are an Administrative Authority who is not a Billed Entity. starting services for which you were approved for discounts as the Billed Entity on a Form 471, please file a Form 486 and submit it to the Schools and Libraries Division.
