What Acyclovir Denk 5% Cream is and what it is used for Acyclovir Denk 5% Cream is a medicine that blocks the growth of certain viruses (antiviral agent, viral DNA polymerase inhibitor). Acyclovir Denk 5% Cream is used to relieve pain and itching associated with recurrent her- pes infections involving blistering in the genital area and of the lips (recurrent herpes genitalis and labialis). 2. Before you use Acyclovir Denk 5% Cream Do not use Acyclovir Denk 5% Cream – if you are allergic (hypersensitive) to aciclovir, to valaciclovir or any of the other ingredients of Acyclovir Denk 5% Cream. – on mucous membranes (e.g. inside the mouth, on the eye or in the vagina), as local signs of irritation can otherwise be expected. Take special care with Acyclovir Denk 5% Cream In case of severe immune deficiency a doctor should be consulted before beginning of treat- ment. If Acyclovir Denk 5% Cream is applied to the genital and anal area, the concomitant application of latex products (e. g. condom) can cause a reduced resistance to tearing of these products and therefore a reduction of security due to the excipients paraffin and vaseline. This pharmaceutical contains propylene gly- col and cetyl alcohol which may cause skin irritations. In case of hypersensitivity to either of these ingredients the cream should not be used. Taking/using other medicines Please tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including medicines obtained with- out a prescription. Pregnancy and breast-feeding Ask your doctor or pharmacist for advice before taking any medicine. The available data on use during pregnancy have shown no increased malformation rate compared to the normal population. As the malformations that occurred were not homo- geneous, no same cause can be imputed. This medicinal product should only be used during pregnancy after careful consideration of the benefit-to-risk ratio. Following systemic use, aciclovir is excreted in human milk. However, the dose that a child would absorb via breast-feeding is negligible following maternal use of Acyclovir Denk 5% Cream. Driving and using machines No special precautions are required. 3. How to use Acyclovir Denk 5% Cream Always use Acyclovir Denk 5% Cream exactly as your doctor has told you. You should check with your doctor or pharmacist if you are not sure. Dosage If not otherwise prescribed by your physician apply a thin layer of the cream to the affected skin 5 times daily at intervals of 4 hours. Mode of application Acyclovir Denk 5% Cream should be applied with a wadding stick, by applying the amount of cream required to cover the infected parts of the skin. When applying the cream, not only the visi- ble signs of herpes (pustules, swelling, red- dening) should be covered, but also the sur- rounding area. If applied by hands, the hands should be washed thoroughly before and after applica- tion, in order to avoid bacterial infection of the damaged skin or transmitting the virus to other areas of the skin or mucous membrane. Duration of administration In order to achieve the best results, the cream should be applied as soon as the first sign of herpes infection become noticeable (burning, itching, tension, reddening). Once the blister- ing crusts over, virustatic treatment with aci- clovir cream is no longer necessary. The dura- tion of treatment is normally 5 days. In indi- vidual cases the treatment should be contin- ued until a crust forms over the injured skin or the skin is healed. A course of treatment, how- ever, should not exceed 10 days. If you forget to use Acyclovir Denk 5% Cream In case too little aciclovir was applied or an application was forgotten, continue the treat- ment as prescribed. If you stop using Acyclovir Denk 5% Cream For successful treatment, it is important to apply Acyclovir Denk 5% Cream for a suffi- ciently long period of time. 4. Possible side effects Like all medicines, Acyclovir Denk 5% Cream can cause side effects, although not everybody gets them. The frequency of adverse drug reactions is classified as follows: Very common: More than 1 out of 10 patients treated Common: 1 to 10 out of 100 patients treated Uncommon: 1 to 10 out of 1000 patients treated Rare: 1 to 10 out of 10,000 patients treat- ed Very rare: Fewer than 1 out of 10,000 patients treated Not known: The frequency cannot be assessed on the basis of the available data To date, the following side effects have been observed: Skin and subcutaneous tissue disorders Uncommon: Upon application of the cream the patient may experience a temporary burn- ing or prickling sensation. Drying, itching and desquamation of the treated skin have been observed. Rare: Reddening, contact dermatitis have been reported on application of the cream. Allergic tests however showed that the additives in the cream and not the ingredient aciclovir were, in most cases, the cause. Contact der- matitis is probable if the above mentioned side-effects are more severe and also affect skin which has not been directly treated with the cream. In that case contact your physician. Immune system disorders Very rare: Immediate-type hypersensitivity reactions including angioedema (Quincke’s edema, swelling of the skin, especially in the facial area). If any of the side effects gets serious, or if you notice any side effects not listed in this leaflet, please tell your doctor or pharmacist. 5. How to store Acyclovir Denk 5% Cream Store below 25 °C. Do not refrigerate or freeze. Do not use after the expiry date which is stated on the carton. Keep out of the reach and sight of children. 6. Further information Pharmacological properties Pharmacotherapeutic group: Guanosine analogue, antiviral agent DNA polymerase preventing agent ATC Code: D06B B03 Aciclovir is a pharmacologically inactive sub- stance, which becomes virustatic only after penetrating a cell, which is infected with her- pes simplex or varicella zoster viruses. This activation of aciclovir is catalysed through herpes simplex or varicella zoster viruses thymidine-kinase, an enzyme, which the virus urgently requires for replication. The following individual steps take place: 1. Aciclovir penetrates the herpes infected cells after application in increased number. 2. The virus thymidine kinase, present in these cells, phosphorylizes aciclovir to aciclovir monophosphate. 3. Cellular enzymes change the aciclovir monophosphate into the actual antiviral agent aciclovir triphosphate. 4. Aciclovir triphosphate has a 10 to 30 times stronger affinity to the virus-DNA-poly- merase than to cellular-DNA-polymerase and in this way selectively inhibits the activity of the viral enzyme. 5. In addition, the virus-DNA-polymerase builds aciclovir into the virus DNA, which results in a chain breakdown of DNA syn- thesis. These individual steps result, in total, in a very effective reduction of virus production. Plaque reduction tests demonstrated that, in order to impede growth of HSV infected viral cells an average of 0.1 μmol aciclovir/l is required (cell culture from the kidney parenchyma of the green african monkey), in non infected cells on the other hand 300 μmol aciclovir/l is required. One can therefore determine for cell cultures therapeutic indices up to 3000. Range of activitiy in vitro: Very sensitive: Herpes simplex virus type I and II Varicella zoster virus Sensitive: Epstein Barr virus Slightly sensitive to resistant: Zytomegalie virus Resistant: RNS virus Adenovirus Smallpox virus Pharmacokinetic properties Examining the resorption of aciclovir from the cream: The cream was applied to 6 probands five times per day, over four successive days. The cream was applied to the skin of the back and was spread over an area of 710 cm 2 until no more cream was visible. During the tests aciclovir could not be detected in the serum. The limit of detection of aciclovir was less than 0.01 μmol/l. On the second day a detectable concentration of aciclovir was found in the urine, whereby the concentrations grew slightly during treatment and reached a level of 0.6 μmol/l on the fourth day. This value corresponds to less than 0.1 % of the amount of aciclovir applied to the skin. Although these values show that there is a minimal resorption of aciclovir from the cream, the actual extent of concentrations demonstrates that no systemic action is to be expected. No measurable aciclovir concentrations could be determined in the blood after use of the cream. Quand les vésicules ontdéjà formé des croûtes, un traitement avecAcyclovir Denk 5% Cream n’est plûs utile.

La durée normale du traitement est de 5jours. Dans certains cas le traitement devraitêtre poursuivi jusqu’à ce qu’une croûte seforme au-dessus de l’épiderme blessé oujusqu’à ce que l’épiderme soit guéri. 1. O que é Acyclovir Denk 5% Creame para que é utilizado Acyclovir Denk 5% Cream é um medicamentoque inibe o crescimento de determinadosvírus (virustático, inibidor da polimerase doADN viral).

O Acyclovir Denk 5% Cream é usado paraaliviar a dor e comichão associados às infec-ções recorrentes por herpes com erupções nazona genital e nos lábios (herpes genitalis elabialis recorrente).

2. Antes de utilizar Acyclovir Denk5% CreamNão utilize Acyclovir Denk 5% Cream– se tem alergia (hipersensibilidade) ao aciclo-

vir, a valaciclovir ou a qualquer outro com-ponente de Acyclovir Denk 5% Cream,

– em mucosas (por ex. na cavidade da boca,ao redor dos olhos ou na vagina); isto podeprovocar irritações locais da pele.

Tome especial cuidado com Acyclovir Denk5% CreamEm caso de deficiência imunológica gravedeve se consultar um médico antes de iniciaro tratamento.

A aplicação de Acyclovir Denk 5% Cream naárea genital/anal e administração simultâneade produtos a base de látex (par ex. preserva-tivo) pode causar uma redução da resistênciae até rasgor de estes produtos e consequente-mente uma redução da segurança devido aosexcipientes parafina e vaselina.

Este fármaco contém propilenoglicol eálcool cetílico que podem causar irritações napele. Em caso de hipersensibilidade a qual-quer um destes ingredientes, o creme nãodeve ser usado.Ao utilizar Acyclovir Denk 5% Cream comoutros medicamentosInforme o seu médico ou farmacêutico se esti-ver a tomar/usar ou tiver tomado/usadorecentemente outros medicamentos, incluindomedicamentos obtidos sem receita médica.Gravidez e aleitamentoConsulte o seu médico ou farmacêutico antesde tomar qualquer medicamento.

Os dados disponíveis acerca da utilizaçãodurante a gravidez não demonstraramnenhum aumento no índice de malformaçõesem comparação com a população normal. Asmalformações observadas foram heterogê-neas, de forma que não se pode concluir umacausa comum.

A utilização durante a gravidez devebasear-se numa avaliação cuidadosa da rela-ção risco-benefício.

Aciclovir é excretado no leite maternoquando administrado sistemicamente. Entre-tanto, a dose que uma criança pode receberatravés da amamentação depois da utilizaçãodo Acyclovir Denk 5% Cream é desprezível.Condução de veículos e utilização de máquinasNão é necessária nenhuma medida de precau-ção especial.

3. Como utilizar Acyclovir Denk 5%CreamUtilizar Acyclovir Denk 5% Cream sempre deacordo com as indicações do médico. Fale como seu médico ou farmacêutico se tiver dúvi-das. Posologia Se não for prescrito de outra forma, apliqueuma camada fina do creme na área afectada 5vezes por dia em intervalos de 4 horas.Tipo da administração O Acyclovir Denk 5% Cream deve ser aplicadocom um cotonete, espalhando a quantidadede creme necessária para cobrir as partesinfectadas da pele.

Ao aplicar o creme, não só os sinais visíveisdo herpes (pústulas, inchaço, vermelhidão)devem ser cobertos, mas também a área cir-cunvizinha.

Se aplicar com as mãos, estas devem serbem lavadas antes e depois da aplicação, a fimde evitar uma infecção bacteriana da peleafectada ou de transmitir o virus a outrasáreas da pele ou da membrana mucosa.Duração da administração De modo a conseguir o melhor resultado, ocreme deve ser aplicado assim que o primeirosinal de infecção de herpes se tornar visível(queimadura, comichão, tensão, vermelhidão).Assim que se formar uma crosta sobre a erup-ção, o tratamento virustático com aciclovircreme já não é necessário.

A duração do tratamento é normalmentede 5 dias. Em alguns casos é aconselhável con-tinuar o tratamento até que se forme umacrosta sobre a pele ferida ou esta fique cura-da. No entanto, o período de tratamento nãodeve exceder os 10 dias.Caso se tenha esquecido de utilizar Acyclovir Denk 5% CreamCaso tenha aplicado demasiado pouco aciclo-vir ou uma das aplicações foi esquecida, conti-nue o tratamento como prescrito.Se parar de utilizar Acyclovir Denk 5%CreamPara que o tratamento seja bem sucedido, éimportante aplicar o Acyclovir Denk 5% Creampor um período de tempo suficientementelongo.

4. Efeitos secundários possíveis Como todos os medicamentos, o AcyclovirDenk 5% Cream pode causar efeitos secundá-rios, no entanto estes não se manifestam emtodas as pessoas. A frequência dos efeitos adversos é classifica-da como se segue:

Pouco frequentes: 1 a 10 de 1000 pacientes tratados

Até o presente, os seguintes efeitos secundá-rios foram observados:Afecções dos tecidos cutâneos e dubcutâneosPouco frequentes: Após a aplicação do cremeo paciente pode experimentar uma sensaçãotemporária de ardor ou formigueiro. Disseca-ção, comichão e descamação da pele tratada.Raras: Vermelhidão, dermatite de contactoapós a aplicação do creme. No entanto, os tes-tes de alergia mostraram que os aditivos nocreme e não o ingrediente aciclovir eram, namaioria dos casos, a causa.
