    The Active Wandsworth Awards recognises the commitment andachievement of the many people involved in sport within Wandsworth.

    The awards provide Active Wandsworth with the opportunity to rewardtalented local sports performers, coaches, volunteers, teams and clubs fortheir exceptional achievements throughout 2013.

    Nomination rules

    Nominees mustreside in Wandsworth or attend a school in the borough or be a member ofa sports club or physical activity session based in Wandsworth.

    All achievements musthave taken place between 1 January 2013 and 31 December 2013.

    The decision of the selection panel isfinal and will be based on the information providedwithin this application.

    For each nomination you mustprovide a photo of the nominee. Where possible send it byemail, preferably in jpeg format as we use the pictures within the presentation.

    Awards Criteria

    Sports Club of the YearThis award is for a sports club that has:1. Encouraged more people to participate in their sport or activity and helped people

    improve their skills and level of performance.

    2. Has used innovative ideas to promote, develop or enhance its programmeand/or facilities.

    3. Received or working towards Clubmark accreditation; say how the club links in withother organisations, for example local schools.

    4. Made any significant achievements (promotions/trophies) in 2013 - highlight whatthese are.

    The Mayors AwardThis award recognises how an individual, organisation or project has increased

    opportunities for disabled people in London to participate in sport or physical activity.1. What has the individual or organisation done that will encourage more disabledpeople to participate in sport.

    2. How has the nominee worked in partnership with others to increase opportunities.3. What qualities has the individual or organisation demonstrated that sets them apart

    from others.

    Voluntary Services to Sport / Physical ActivityThis award recognises the contribution that a volunteer has made in their chosen sportor activity in 2013:1. What role(s) has the volunteer undertaken during 2013.2. How many hours a week have they completed in the roles outlined in Question 1.3. How has the volunteer encouraged more people to participate in their sport or activity.4. What qualities has the volunteer demonstrated that sets them apart from others.

    Outstanding Contribution to Sport / Physical ActivityThis award recognises the person who has made an outstanding contribution, in a voluntarycapacity, to the lives of many through sport and physical activity, over a minimum of 5 years.1. What is the background to the nominees involvement with the group or organisation.

    2. Please give examples of their commitment to sport/physical activity.3. What impact have they had within their sport/physical activity.4. How have they overcome any barriers to ensure opportunities have been introduced

    or maintained.

    Physical Activity Project of the YearThis award recognises a physical activity project which has increased activity levels andimproved residents of Wandsworths health and wellbeing:1. A project that has made a significant contribution to supporting people to overcome

    barriers to participation2. Demonstrates effectiveness in creating opportunities for inactive people to start,

    and maintain being active.3. Has built strong community relationships and links.

    Young Volunteer of the YearThis award recognises the contribution that a volunteer (under 18 years of age) hasmade in their chosen sport:1. What role(s) has the volunteer undertaken during 2013.2. How many hours a week have they completed to the roles outlined in Question 1.3. How has the volunteer encouraged more people to participate in their sport or activity.

    4. What qualities has the volunteer demonstrated that sets them apart from others.

    Team of the Year (Under 18)This award recognises the achievements of a sports team from a school or club in 2013.1. Please list any significant achievements in 2013.2. Demonstrated fair play and a good sporting attitude throughout the year.3. Shown significant improvements on previous years.

    Coach/Instructor of the YearThis award is for the coach who has:1. Increased participation by inspiring and encouraging others to progress in

    their activity.2. Achieved personal development in the last 12 months such as new coaching or

    instructor qualifications, please give evidence.3. Set themselves apart from other coaches and instructors - please outline their main

    achievements in 2013.

    Junior Sports Person of the Year (16 and under as of 31 December 2013)This award is for an individual who has:1. Made significant progress in their sport in 2013.2. Ranked and/or made significant achievement in 2013.3. Made a significant impact on sport in Wandsworth and been an inspiration to others.4. Exceeded expectations in their sporting performance or made a significant difference

    to a team.

    Sports Person of the Year (25 and under as of 31 December 2013)This award is for an individual who has:1. Made significant progress in their sport in 2013.2. Ranked and/or made significant achievement in 2013.3. Made a significant impact on sport in Wandsworth and been an inspiration to others.4. Exceeded expectations in their sporting performance or made a significant difference
