We are elated to wrap up our 2015-16 year with some fantastic results for our Green Prescription (GRx) and Active Families service, detailed below. In addition, we have also secured additional funding for the 2016-17 year (our fourth consecutive year) to run our popular and effective Healthy Lifestyle Programme interventions. Green Prescription The team is very pleased to have achieved 8/9 of our contracted Key Performance Indicators (KPIs) for the adult GRx service in this year’s survey results. The survey is conducted by Research NZ on behalf of the Ministry of Health, and takes the form of a self-reported questionnaire. The questionnaire tries to gauge health, physical activity, nutrition and behaviour change outcomes as a result of the GRx service received. The results for Maori participants were also analysed, and we are delighted with the overwhelmingly positive results. Please see page 2 for more detailed information on our GRx KPI achievements. Sport Wellington achieved 96% of our 3,904 referral target for GRx, and 105% of our Active Families target of 110 children. We have supported our participants with a range of services to lead healthier lifestyles, these include: phone support, face-to-face consultations (evaluation of this presented on page 2), Healthy Lifestyle Programmes, walking groups, physical activity and nutrition education sessions, cooking sessions and links to discounted community exercise options and events to name a few! It came to the attention of the Active Communities team that the phone-based model for healthy lifestyle intervention was not meeting the needs of some of the special population groups we were working with in our Green Prescription (GRx) service. As a responsive and adaptive service, a pilot for community based individual and whanau/family face-to-face meetings was undertaken for GRx patients in different locations throughout the region in 2014. After advocating back to the DHB in 2015 and sharing our evaluation, part of the Wellington GRx service was adapted to incorporate “GRxPlus”. We can now respond to the higher needs of some of the referrals we receive (e.g. type 2 diabetes, mental health, etc.) with more intensive and/or longer support such as face-to-face consultations and group education programmes.

Face-to-face consultations now run regularly and are offered to all patients living in Upper Hutt, Taita, Naenae, Wainuiomata, Stokes Valley, Porirua, Strathmore, Johnsonville, Kilbirnie, central Wellington and Kapiti, Results from our most recent evaluation are shared here.

Face-to-face backgroundGRx patients are encouraged to meet their support person face-to-face at least once, and up to four times. Meeting venues are selected based on convenience for GRx patients in varying community locations such as council operated recreation facilities and community centres. 97% of respondents were satisfied with the meeting venue, with general feedback being venues chosen were easy to access, close to home and reasonably private.

Physical activityOverall, GRx patients who received face-to-face support reported an increase in their physical activity habits. Post or during intervention, 97% of the respondents are

active every week, compared to 56% before.

NutritionSelf-rated nutrition habits were just above average at 5.9 (on a 10-point scale) before intervention and have increased to 7.6 post or during intervention. 90% of respondents report an increase in healthy lifestyle knowledge since starting. Cutting down on sugar/sweet foods was a notable comment made by a few of the respondents. 80% report noticing positive changes to their overall health since receiving face-to-face support such as weight loss, feeling fitter and feeling better.

Face-to-face versus phone supportAn aim of the survey was to gauge whether meeting people face-to-face is more effective than standard phone support.

The quality of support from a GRx support person was rated slightly higher (on a 10-point scale) for face-to-face interactions than over the phone when discussing physical activity and healthy nutrition. However, the phone based results are still largely positive which is pleasing as not all patients are able to attend face-to-face meetings.

There were numerous comments regarding the advantages of face-to-face meetings, such as:

“I thought that having a face-to- face meeting is an advantage. It feels more personable and supportive than just a phone call. I reckon it aims in achieving results for the person.”

“I like to meet face-to- face on a regular basis. Weekly to start with, then fortnightly then monthly.”

“Very helpful having face-to-face meetings – hopefully will keep me on track.”

Active Families updateThe Active Families team and participants have wrapped up a successful term two programme with brand new weekly sessions starting up in Johnsonville, Porirua and Strathmore. Children and their whanau/family have been getting to know their local community, participating in age/stage appropriate activities, learning about the sugar content of different beverages, and have had the opportunity to get involved in a community sporting event.

Sessions will have a physical activity and nutrition focus with children and whanau/families learning to identify and prepare healthy food choices while also engaging in age/stage appropriate physical activity. Active Families Upper Hutt continues to have strong participation from children and their parents/caregivers. As the child’s whanau/family has a big impact on dietary intakes and levels of physical activity, it is good to see consistent family involvement in this programme. Last term, Active Families Upper Hutt participated in self-defence lessons with Upper Hutt Martial Arts Academy, a children’s ‘Upper Cutt’ class at a local, low cost gym along with a taster of disc golf through Ignite Sports.

A highlight of the term was seeing the children from the Porirua programme participate in the Wellington Marathon Kids Magic Mile. Sport Wellington Active

Families have participated in this external event since 2008 and continue to receive fantastic support from organisers. When the easiest option would have been to remain in bed on a cold and dark Sunday morning, all fourteen of our children who registered chose to battle the elements. Parents/caregivers were blown away by the atmosphere and enjoyed being able to share and celebrate their children’s achievement. Some feedback was:

One eight year old boy was so elated when Melissa Moon hugged him and presented him his medal at the finish line, he couldn’t wait to go back to school and tell all of his friends. Event participation is an excellent way for children and

whanau/families on our programme to set goals, boost self-esteem and as they are often free or discounted they provide opportunities to traditional non-participants.

Advocacy, links and what’s in the news

I have always felt somehow disconnected from my body, like it had spent 45 years trying to prevent me from enjoying life. During my teens, I was diagnosed with severe endometriosis. I tried every medication recommended but only surgery seemed to keep it at bay. For years, I had surgery every 12 -18 months.

A year after the birth of my second child, I had a hysterectomy and started trying to work on getting my health back. For almost four years everything was great. I walked, I lost weight, and I felt fantastic. Then my world started to fall apart, I got sick again. Over the next three years I would battle the health system, I would battle my body, and I would have another four major surgeries. I would be pumped full of hormones and I would put on weight - a lot of weight.

In the middle of 2014, I topped the scales with an extra 20kgs and I felt awful. I started seeing a dietitian, and started to try and do some exercise. I lasted several months, trying to do it on my own, and it

just become too difficult. I gave up. My self-esteem was at an all-time low, and I was avoiding my friends and family. I had hit rock bottom.

In April 2015, I was told I could not have any more surgeries, and I would have to find another way to heal, and come to terms with my fate. There was nothing more the health system could do to help me. But, with the help of some wonderful friends, I took myself back to the doctor, and came up with a plan. I started some new pain medication, and I started working with the lovely ladies at Green Prescription (GRx). I started going to the pool once a week and was put in touch with an incredibly supportive group of women (part of Upper Hutt City Council’s ALIVE project). I started walking, and going to the gym with them.

With the most amazing support and encouragement, over the last ten months I have found myself again. I have lost over

10kgs, and I have met the most wonderful supportive people. My whole life has been turned around! For the first time in my life, I feel in control of my body, and like I can do this. I have so much to thank the GRx scheme for, without the support I have had, I never would have achieved as much as I have to date.
