About the Doncaster social prescribing pilot...
South Yorkshire Housing Association and Doncaster CVS are working in partnership with support from the Doncaster Innovation Fund to deliver a Social Prescribing project across North West and Central Doncaster. The pilot project will run from 1st August 2014 to 30th July 2015.
The service uses NHS predictive models to target patients who are at emerging risk of going into hospital or care and who have at least one long-term condition. Sheffield Hallam University will assist SYHA and Doncaster CVS to monitor and evaluate impacts and outcomes for customers and for the health and social care system.
