PARENTAL CONSENT & PROPERTY DAMAGE CONTRACTI wish to enroll my child in the Panther Volleyball Academy at Greenville College. I release the State of Illinois, Greenville College, and Panther Volleyball Academy LLC, and any and all persons associated with Panther Volleyball Academy LLC from any and all liability for any injury, medical or dental, sustained by my child. I also indemnify the same parties from any responsibility for liability, loss, or damage done to any facility or equipment by the participant as a result of negligent or willful act done by my child or ward. I accept responsibility for paying damage charges before my child leaves camp. Finally, I certify that I have medical insurance for my child.

2014 PANTHER VOLLEYBALL ACADEMY REGISTRATION FORM

EDUCATION: B.S. degree in Recreational Leadership
EXPERIENCE: Coach Ackerman is entering his 18th year at Greenville College, 2014 will be his 13th year as head coach. Coach Ackerman
was voted Coach of the Year in 2002, 2008, 2010 and most recently in 2013 after leading his team to their first ever SLIAC regular season outright championship and their 4th in the last 4 years. Ackerman has taken the Panthers to the conference tournament the last 8 years and the 2010 team earned the right to play in the NCAA National tournament, which was a first for the program.
Ackerman has trained internationally with coaches and players from Nicaragua, Dominican Republic, and Mexico.
Coaching at every level from elementary to college, as well as both girls and boys, has given him valuable experience. As the PVA Club Director Ackerman continues to educate players, coaches, and parents about the sport of volleyball.
