Construction on Memorial Health’s outpatient medical center in Urbana is progressing well and was celebrated with a Topping Ceremony in late July.

The $9-million, 30,000-square-foot facility will be located at the northwest corner of East U.S. Route 36 and North Dugan Road, and leased from developer Hplex Solutions. It will house Memorial Primary Care | Urbana, a clinic for rotating physician specialists, urgent care, x-ray imaging, lab testing services, physical and occupational therapy, and a medication therapies clinic.

Alexandra Pelaez, MD has joined John Adams, II, DO at Memorial Family Medicine | Plain City. She is accepting new patients, and appointments can be made by calling the office at (614) 873-3434.

Dr. Pelaez recently completed her residency in Family Medicine, with an elective in Palliative Care, at The Ohio State University Wexner Medical Center and College of Medicine’s CarePoint East location. She graduated from The Ohio State University College of Medicine, where she participated in the Honors Program in Family Medicine.

A. When it comes to treating patients, both believe in helping patients live as active of a lifestyle as they can.

Dr. Boucher is board certified in Family Medicine and completed his fellowship in sports medicine at OhioHealth Grant Medical Center in Columbus. Mackenzie Morris completed a Master’s of Science in Nursing in Family Practice from The Ohio State University College of Nursing and brings her experience as a certified Athletic Trainer to Memorial.

Memorial now offers full-scope midwifery services that will provide family-centered maternity care, along with well-woman care, family planning and other services. Lisa M. Smithers, Certified Nurse Midwife, has joined the team at Marysville Obstetrics & Gynecology. Smithers has been serving women and families through pregnancy, childbirth, and the postpartum period for over 12 years, most recently in the Bellefontaine and West Liberty areas.

Hospital Expansion

Memorial 2020MergingClinical Quality& Patient Experience

Following these in-depth discussions, a “canted” design was chosen for the inpatient rooms. Canting, or angling the headwall toward the window increases the patient's views to the outside and allows more natural light to cascade in. Team members also gave consideration to where the sun rises and sets on the hospital campus.

The layout was further configured so that the clinicians and family members can be engaged in care, right along with the patient, with expanded room for visitor seating.

Bathrooms will be located along the interior wall and will feature a full, private shower for greater patient privacy. A true upgrade from the current shared, dormitory-style showers that exist now.

The rooms are also designed for versatility, with each room having the flexibility to meet a variety of clinical needs, from observation and general medical/surgical to critical care. The specific-use rooms currently at the hospital only permit for a certain number of rooms for each level of care. Designing these multipurpose rooms will allow care to be delivered according to the patient’s needs – and adjusted accordingly.

The adaptable rooms will enable our clinical staff to modify the nurse-to-patient ratio to coincide with the medical needs of patients. Additionally, more critical care patients can be cared for at a given time, enabling patients to stay local, close to their loved ones.

According to Boyer, the nursing staff also provided their input for the modernized nursing “perches” outside of each room (see front cover for example). “Created to enable nurses to remain close to patients, the perches facilitate charting activities, without the centralized nurse station of yesterday. We chose this design element so our nursing staff can be visible and easily accessible to our patients.”

Memorial 2020 will completely modernize the patient experience by increasing privacy,

creating universal care rooms and enhancing the room function

and design. Groundbreaking for the Memorial 2020 project is anticipated for March 2018.

HENRY FORD has been quoted as saying, “Coming together is a beginning, staying together is progress, and working together is success." This certainly became true during the design stages of the new inpatient pavilion for Memorial Health.

Both new inpatient and outpatient pavilions are part of the Memorial 2020 project that will provide state-of-the-art facilities for the independent healthcare system.

INPATIENTS WILL ENJOY comfortable, best-in-class hospital rooms with full, private bathrooms. Memorial 2020 will replace all existing rooms, except those in Obstetrics and the Nursery, with 36 brand-new, multi-purpose patient rooms.

With the goal of matching the high-quality of care to the patient room configuration, the team at Memorial Health felt up to the challenge. Details of both the room layout and design were poured over by the clinical staff and administration team. The outcome will be a balance of patient experience essentials along with optimal clinical requirements.

Ashley Boyer, Assistant Vice President for Nursing Services at Memorial, was part of the design discussions with the entire team of nurses and other clinical experts. “We all agreed the design of the patient room for the future could not be a cookie-cutter solution. Our design needed to represent the goals and operations of our Memorial healthcare team, as well as what would be attractive for patients’ and their families’ comfort, healing and satisfaction. It was thrilling to see our entire team be a part of the process.”

The first floor of the inpatient pavilion will include a dedicated main entrance with a spacious, welcoming lobby.

Inpatient rooms will feature a canted or angled design to maximum window space, along with an increased area for visiting guests.

Pain Management

PATIENTS UNDERGOING abdominal or pelvic surgery are benefiting from a new and progressive post-surgery pain management technique used at Memorial Hospital. This newer technique, not available at all hospitals, is used to manage pain from an incision of the abdominal or pelvic area from procedures including hysterectomy or bowel surgery.

Many of Memorial Hospital’s Anesthesiologists and Certified Registered Nurse Anesthetists have trained to use a Transverse Abdominis Plane (TAP) block, which provides pain relief for the incision area for up to 24 hours after surgery. While the patient is under other anesthesia and not awake, providers use ultrasound to guide injections of anesthesia into peripheral nerves to provide effective, localized pain relief in and near the incision area.

Because narcotics are not used, patients recover more quickly without the side effects that affect the entire body. Regular breathing returns earlier, patients walk sooner, have less nausea, and avoid digestive constipation. Post-surgery hospital stays are often reduced. TAP is also a good option if the patient has a history, or risk, of opioid addiction.

Jon Veith, a Certified Registered Nurse Anesthetist in Memorial Hospital’s Surgery Department, says they started using the TAP procedure last fall, and have received very positive responses from patients, as well as surgeons. “We are very progressive in treating post-surgery pain at Memorial Hospital,” he explains. “We utilize a number of methods to manage it, and we tailor the method to the health and needs of the patient.” He recommends that patients discuss post-surgery pain management with their anesthesia provider when they first meet before surgery.

Concussion Management

Returning to ParticipationOnce your athlete has been symptom free for at least 24 hours, and your physician has given permission to begin the Return-to-Play protocol, he or she can work with an athletic trainer.

The Return-to-Play protocol exposes your athlete to increasing cardio and sport-specific stresses, while monitoring for occurrence of concussion symptoms. If your athlete experiences no return of symptoms, he or she will be able to participate without limitations five days after beginning the protocol. If symptoms return, the athlete must wait until they are symptom-free for 24 hours before they can return to the previous step.

In certain situations, an ImPACT test will be conducted to determine if his or her brain has returned to its pre-injury cognitive function. ImPACT is a computerized neuropsychological test that provides comparisons between a baseline test and a post-concussion test. If your athlete’s post-test scores are equal to his or her baseline scores, or at the expected norm, they then continue with the five-step progression.

After completing the progression, the athlete is usually permitted to return to full participation without limitations.

THE HUMAN BRAIN is an amazing organ. It is the command center for the human nervous system. It receives input from the sensory organs and sends output to the muscles. It is essentially the boss of our body – it runs the show and controls everything we do. Not too bad for something that looks like a big, wrinkly, gray sponge.

AND IT’S NO SECRET that our brains can suffer injuries. Concussions and traumatic brain injuries have been receiving national attention lately – we hear the word “concussion” everyday – from our student athletes to professional sports to blockbuster movies.

Understanding our brains and how they react to a concussion will help to manage this type of injury, particularly those that happen to our student athletes.

Signs & Symptoms of a Concussion

Concussion symptoms can develop from the time of initial injury up to 48 hours post–injury. Symptoms vary with each person and each circumstance, but may include:

» Loss of consciousness » Vomiting » Difficulty speaking » Loss of hearing » Loss of vision/ blurred vision » Loss of sensation in arms or legs » Difficulty swallowing » Difficulty making facial expressions » Headache, sometimes increasing in

severity with time » Amnesia or loss of memory » Slow responses to questions » Personality or behavior changes,

such as irritability or sadness » Confusion » Dizziness » Difficulty concentrating » Generally not feeling right

Concussion symptoms typically subside in 7 to 10 days in nearly 80% of diagnosed cases. However, concussion patients may experience symptoms up to 21 days or longer.

IS IT A CONCUSSION?If so, what’s next?

What is a Concussion?A concussion is an injury to the brain that may be caused by a hit, bump, or a jarring force to the head. It may also result when the hit occurs elsewhere on the body, after a fall or other traumatic incident that causes your brain to be jarred within the skull. Even if an athlete is wearing a helmet or protective gear, a concussion can occur.

If your athlete is experiencing concussion symptoms, or if you are in doubt about it, contact your primary care physician, the athletic trainer or visit Urgent Care or the Emergency Department. A thorough evaluation and assessment of prior history of concussion can help determine if there are any internal head injuries.

QA: Nephrologists have advanced training in evaluation and treatment of kidney disease. Kidneys work like filters, and blood is purified when it passes through them. Signs of kidney disease might include swelling of the legs, sudden worsening of high blood pressure, foamy urine, or red or brown urine. Often kidney disease is detected by blood testing, even before a person has any symptoms. People who suffer from kidney stones or difficult-to-control blood pressure may also benefit from seeing a nephrologist.

Q How is kidney disease detected?

A: Risk factors include diabetes, high blood pressure, family history of kidney disease, and frequent use of arthritis medications such as

ibuprofen or naproxen. Blood and urine tests are needed, and possibly ultrasound imaging. Progression can be delayed through early

detection, careful monitoring, evaluation/adjustments

in medications and nutrition

counseling.

A: Early stage kidney disease is often treated with medications that help control blood pressure and lower urinary protein. Some kidney diseases may require a biopsy to make a clear diagnosis, and then treatment with specific medications based on the biopsy result. If kidney function declines to less than 10 to 15 percent of normal, then either dialysis or a kidney transplant will be needed. In some cases, people are referred to local transplant centers for evaluation.

If you suspect you have a kidney problem, you can order a blood draw and a urine test yourself, without a doctor’s order. Visit one of Memorial Hospital’s Lab locations and ask for a Urine Dip test ($5) and a Basic Metabolic Panel blood test ($25).
