Attention 6th Graders that have received notifications from the school nurse that you are in need of shots to enter the 7 th grade. If you would

like to receive these required immunizations at school, please have the forms signed and returned

to Nurse Sharon!

The Health Department will be at our school for one day only on the morning on February 2nd to give these shots to

the students that have returned their forms.
