clinic carried by his father due to fever with
polyarthritis.
On examination, the arthritis was
asymmetrical, migratory, affecting large
joints.
Heart examination revealed distant
heart sounds. The parents gave
history of sore throat to the child 2
weeks ago.
What is the diagnosis?
What is the cause of distant heart
sounds and how to treat the case with
this presentation?

A 3 years old boy presented to the
causality room by marked pallor,
lassitude, cold extremities, palpitation
and exertional dyspnoea.
His mother gave history of a febrile
illness to the child 2 days before, for
which she gave him Novalgen
(metamizole sodium) and sulfa
containing medications. The mother
gave the history for similar condition
in her family.
