PRESENTING SYMPTOMS:

• Right‐sided chest pain, late 2017• Shortness of breath• Cough

Medical History

• Surgery done out of country for removal of a pelvic mass early 2017 (says she was told everything was fine)

• No diabetes• No BP• Not on any chronic meds

• No family history of cancer• G10 P10; youngest child 23 years• Never smoked• Alcohol stopped many years back• Never worked in a mine

Patient seen out of country in late 2017:• CXR done confirmed right pleural effusion• CT scan done confirmed pleural effusion with 

possible consolidation of a portion of the right lung with associated nodular infiltration and reduction in lung volume

• Pleural tap was done • Cytology results ruled out malignant cells• No TB

• Antibiotics• Empiric Anti TB treatment• Patient returned to her home country, where 

symptoms recurred• Had second pleural tap done• A Botswana‐based MD in private practice got 

involved in her case, and she was flown to another hospital out of country

Medical History (continued):• CT scan done in early 2018 showed similar 

findings, but no mention of a pelvic mass• Had biopsy of the lung which confirmed 

adenocarcinoma• EGFR mutant axon 19‐DEL

• Patient was not keen on conventional chemotherapy; however she agreed to target therapy

• Went back to her home country; eventually arrangements made to come to Botswana because of easier access to get the drugs for her

Medical History – in Botswana:

• First saw her toward the end of 2018• Presenting symptoms were:

• SOB which said had improved• Right lower chest pain• Cough• LOA• Belching

• Planned to start her on Tarceva (erlotinib)• Decided to restage her with CT scan

• CT results confirmed infiltrative lesions involving the right lung and right pleural effusion with contralateral metastasis as well as a 10cm pelvic mass and ascitis

• Ca 125 done 315• ? Ca lung with ovarian mets• ? Ovary with mets• ? Ovarian second primary

Treatment Plan:

• Taking into account the time the patient had in Botswana, we convinced her to start on chemotherapy

• Started on Carboplatin/Paclitaxel/Avastin• Had two cycles with excellent response• Received final pathology after the first cycle

DIAGNOSIS:• SPECIMEN 1 AND 2 (LABELED RIGHT OVARY 

AND LEFT OVARY ):• BOTH OVARIES INVOLVED BY A MODERATELY 

DIFFERENTIATED ADENOCARCINOMA.• FALLOPIAN TUBES PRESENT AND ALSO 

SHOWING TUMOUR INVOLVEMENT IN THE WALL.

• AN ADDENDUM REPORT TO FOLLOW ON RESULTS OF THE IMMUNOHISTOCHEMISTRY.

Pathology, continued (ovary):

Addendum #1:• The CK7 is diffusely positive in both ovaries 

as well as the TTF1. The P53 immunohistochemical stain also shows positivity in a subset of cells.

• The following immunohistochemical stains are negative: CK20 and WT1.

• The overall features including the immunohistochemical stains are in favour of adenocarcinoma metastatic from the lung.

• Patient to continue with the same treatment• ? Maintenance therapy with erlotinib on
