Colleagues,   On behalf of the American Association of Neuromuscular & Electrodiagnostic Medicine (AANEM), I welcome you to the 2016 Annual Meeting! I would like to personally invite you to attend two great networking opportunities that you won’t want to miss. First, please attend our meeting kick-off event, the President’s Reception, at 5:30 pm on Wednesday, in the Grand Ballroom of the Hilton New Orleans Riverside. The following evening, please join me at 5:30 pm in the Hall of Research (Exhibition Center C) for an evening celebrating research at the Abstract Award Winner’s Reception, where you can enjoy refreshments while abstract award winners present their research. Both events start early, so you’ll have plenty of time to enjoy dinner later on at one of New Orleans’ many great restaurants.

This week you’ll learn from other EDX and neuromuscular specialists who are able to address your questions – no matter your career stage or level of expertise. While you’re here, make sure to:• Learn about insights and innovation in neuromuscular

Complete the CME/CEU Recording Form for all hours of participation in general sessions (i.e. the Plenary, Ask the Experts, courses, symposia, poster sessions etc.). Please note:• Credit for workshops is recorded automatically, all other credit must be recorded manually.• Do not claim credit for self-assessment sessions, if participating in an Annual Meeting Self-Assessment Course.

CME for the full self-assessment activity will be awarded with successful completion of the post-test.• The CME/CEU Recording Form may be modified until December 31, 2016. Credit submitted after this date will

Meeting Objectives and Notes:The 2016 annual meeting will focus on improving patient care, medical knowledge, interpersonal communication, professionalism, and systems-based practices in the following areas: neuropathies, updates in NM disorders including CIPD, emerging gene therapies for NM disease, hyperexcitable membranes, iatrogenic and autoimmune disorders, EDX for focal neuropathies, genetics, infection, stem cell therapy, US and MSK disorders.

After attending this activity, attendees will: • Enhance their ability to obtain a comprehensive patient history and examination; improve their ability to develop a differential diagnosis and direct appropriate diagnostic work-ups; and assess rehabilitation potential for patients with NM and MSK diseases. (Patient care) • Develop technical skills necessary to perform neurologic, EDX, and rehabilitative procedures; identify and describe important EDX, biopsy, genetic, radiological, and US findings; develop awareness about the side effects of drug therapies and their management; and understand updated information on the genetic basis of NM disorders and their treatment. (Medical knowledge, Practice based learning)• Improve ability to communicate with and educate patients, families, and members of the healthcare team; develop awareness of ethical and biomedical legal issues related to patient care; enhance awareness of patient confidentiality issues as they relate to patient care; demonstrate professionalism in clinical, research, and academic practice; and demonstrate skills in end-of-life care and withdrawal of support. (Interpersonal communication skills, Professionalism)• Develop awareness of cost effectiveness of diagnostic studies and treatments

and resource limitations in health care; identify and access supportive healthcare services and mechanisms that improve patient care and patient quality of life. (Systems-based practice)

Accreditation Statement The AANEM is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Upper Extremity FunctionIn this lecture, Dr. Han will provide

a brief overview of some basic outcome measures and assessment concepts relevant for NMDs; focus on upper extremity functional assessment using motion sensor technology and the latest scalable technologies for use in clinical and research settings; research and findings with implications for various NM disorders; research applications in clinical trials; and some of the future innovative technology intervention developments including robotics and exoskeletons.Stuart Reiner Lecture: Begun in 1988 in recognition of Stuart Reiner’s contributions to the development of instrumentation.

In this lecture, Dr. Kissel will provide a brief yet comprehensive update and state-of-the-art review of the genetic diagnosis and outlook for definitive gene-based therapies in these challenging disorders. Attention will also focus on future technologies (e.g. CRISPR/Cas9) showing promise to treat or even cure other NM conditions.

Emerging Therapies for Hereditary ATTR Amyloidosis with Polyneuropathy (hATTR-PN): The potential role of RNAi therapeutics

Objectives: (1) Describe the unmet medical needs in hATTR-PN, (2) review the natural history and clinical assessment tools in hATTR-PN, and (3) discuss the development of investigational RNAi therapeutics as a potential treatment option for patients with hATTR-PN. Hereditary ATTR amyloidosis with polyneuropathy (hATTR-PN), also known as familial amyloidotic polyneuropathy (FAP), is an inherited, progressive disease leading to death within 5-15 years. It results from a mutation in the transthyretin (TTR) gene, causing misfolded TTR proteins to accumulate as amyloid fibrils predominantly in peripheral nerves and other organs. hATTR-PN affects ~10,000 patients worldwide, and can cause a range of neuropathy symptoms resulting in significant disability and death. Due to the wide range of symptoms, patients may remain undiagnosed. Current treatment options for hATTR-PN patients are limited/ aren’t available or suitable for all patients; thus there is a significant need for novel therapeutics for these patients. This forum aims to provide an overview of the clinical presentation, challenges in diagnosis, natural history, current treatment options and the unmet medical need of hATTR-PN. Distinguished Physician Award: John D. England, MD

John D. England, MD, is the recipient of the 2016 Distinguished Physician Award, which recognizes a member for superior achievements as a clinician, educator, and association volunteer.

“What I wanted to do my whole life was be a doctor. To be called a ‘distinguished physician’ is the highest honor I can think of,” said Dr. England. “It means I take good care of patients, and I have the interests of healthcare in mind. I hope that I have advanced the field in some small way, and helped provide guidance to other professionals.”

Distinguished Researcher Award: Andrew J. Haig, MD

Andrew J. Haig, MD, is the recipient of the 2016 Distinguished Researcher Award honoring an AANEM member who has made continuous significant contributions to clinical neurophysiology research.

“I’ve gotten many awards in my life, but this is the one that kind of took my breath away,” said Dr. Haig. “These are the people who know my research and appreciate what I’m doing. For the rest of my life, this will be the award that will mean the most to me.”

Enrica Arnaudo, MD, PhD, is the recipient of the 2016 Advocacy Award, in recognition of extraordinary contributions to increase public awareness and advocate to government entities or insurance companies regarding the role of EDX medicine in diagnosis and treatment.

“I have a passion for health policy,” said Dr. Arnaudo. A firm believer in grassroots advocacy, she said, “We need to join together, to build relationships inside and out of the medical profession. I believe we can improve the landscape for physicians if we do it collectively, with a unified voice.”

AANEM CME Programming: It is the policy of the AANEM to ensure balance, independence, objectivity and scientific rigor in all of its educational activities. This program is for scientific and educational purposes only and will not, directly or indirectly, promote the interests of any commercial interest. All CME sessions that are offered as part of the official AANEM Annual Meeting program are developed and implemented solely by the AANEM Annual Meeting education planning committees and the AANEM staff. Education planning committees are responsible for: identifying educational needs and gaps between current and ideal performance, competencies and patient outcomes; determining teaching methods; selecting presentation topics, choosing faculty, establishing objectives; and determining format for the presentation of content. Educational materials cannot contain any advertising, corporate logo, trade name, or product-group message. All scientific research referred to, reported or used in support of justification of a patient care recommendation will conform to generally accepted standards of experimental design, data collection and analysis. Seating at sessions is limited, and food is provided by the AANEM to Industry Forum attendees on a first-come, first-served basis.

INDICATIONS AND USAGEGAMMAGARD LIQUID is indicated as a maintenance therapy to improve muscle strength and disability in adult patients with Multifocal Motor Neuropathy (MMN).

CONTRAINDICATIONSGAMMAGARD LIQUID is contraindicated in patients who have a history of anaphylactic or severe systemic hypersensitivity reactions to the administration of human immune globulin. GAMMAGARD LIQUID is contraindicated in IgA-deficient patients with antibodies to IgA and a history of hypersensitivity. Anaphylaxis has been reported with the intravenous use of GAMMAGARD LIQUID. [see Warnings and Precautions: Hypersensitivity]

WARNINGS AND PRECAUTIONSHypersensitivity: Severe hypersensitivity reactions may occur, even in patients who had tolerated previous treatment with human normal immune globulin. In case of hypersensitivity, discontinue GAMMAGARD LIQUID infusion immediately and institute appropriate treatment. GAMMAGARD LIQUID contains trace amount of IgA (average concentration of 37μg/mL). Patients with antibodies to IgA have a greater risk of developing potentially severe hypersensitivity and anaphylactic reactions. GAMMAGARD LIQUID is contraindicated in patients with antibodies against IgA and a history of hypersensitivity reaction. [see Contraindications]Renal Dysfunction/Failure: Acute renal dysfunction/failure, acute tubular necrosis, proximal tubular nephropathy, osmotic nephrosis, and death may occur upon use of IGIV treatment, especially those containing sucrose.1 Acute renal dysfunction/failure has been reported in association with infusions of GAMMAGARD LIQUID. Assure that patients are not volume depleted prior to the initiation of infusion of GAMMAGARD LIQUID. In patients who are at risk of developing renal dysfunction, because of pre-existing renal insufficiency or predisposition to acute renal failure (such as diabetes mellitus, age greater than 65, volume depletion, sepsis, paraproteinemia, or patients receiving known nephrotoxic drugs, etc.), administer GAMMAGARD LIQUID intravenously at the minimum rate of infusion practicable (not exceeding 3.3 milligram IgG/kg/min (<2 mL/kg/hr).Periodic monitoring of renal function and urine output is particularly important in patients judged to be at increased risk for developing acute renal failure. Assess renal function, including measurement of blood urea nitrogen (BUN) and serum creatinine, before the initial infusion of GAMMAGARD LIQUID and again at appropriate intervals thereafter. If renal function deteriorates, consider discontinuation of GAMMAGARD LIQUID.Hyperproteinemia, Increased Serum Viscosity, and Hyponatremia: Hyperproteinemia, increased serum viscosity and hyponatremia may occur in patients receiving GAMMAGARD LIQUID. It is critical to distinguish true hyponatremia from a pseudohyponatremia that is temporally or causally related to hyperproteinemia with concomitant decreased calculated serum osmolality or elevated osmolar gap; because treatment aimed at decreasing serum free water in patients with pseudohyponatremia may lead to volume depletion, a further increase in serum viscosity, and a predisposition to thromboembolic events.2

Thrombosis: Thrombosis may occur following treatment with immune globulin products, including GAMMAGARD LIQUID. Risk factors may include advanced age, prolonged immobilization, hypercoagulable conditions, history of venous or arterial thrombosis, use of estrogens, indwelling central vascular catheters, hyperviscosity, and cardiovascular risk factors. Thrombosis may occur in the absence of known risk factors. Consider baseline assessment of blood viscosity in patients at risk for hyperviscosity, including those with cryoglobulins, fasting chylomicronemia/markedly high triacylglycerols (triglycerides), or monoclonal gammopathies. For patients at risk of thrombosis, administer GAMMAGARD LIQUID at the minimum dose and infusion rate practicable. Ensure adequate hydration in patients before

administration. Monitor for signs and symptoms of thrombosis and assess blood viscosity in patients at risk for hyperviscosity. [see Boxed Warning]Aseptic Meningitis Syndrome (AMS): AMS may occur with IGIV treatment, and has been reported with intravenous use of GAMMAGARD LIQUID. Discontinuation of IGIV treatment has resulted in remission of AMS within several days without sequelae. The syndrome usually begins within several hours to two days following IGIV treatment. AMS is characterized by the following signs and symptoms: severe headache, nuchal rigidity, drowsiness, fever, photophobia, painful eye movements, nausea and vomiting. Cerebrospinal fluid (CSF) studies frequently reveal pleocytosis up to several thousand cells per mm3, predominantly from the granulocytic series, and elevated protein levels up to several hundred milligram/dL, but negative culture results. Conduct a thorough neurological examination on patients exhibiting such symptoms and signs, including CSF studies, to rule out other causes of meningitis. AMS may occur more frequently with high dose (2 grams/kg) IGIV treatment and/or rapid infusion of IGIV.Hemolysis: GAMMAGARD LIQUID contains blood group antibodies that may act as hemolysins and induce in vivo coating of red blood cells (RBC) with immune globulin. This may cause a positive direct antiglobulin test [DAT (Coomb’s test)].3,4 Delayed hemolytic anemia can develop subsequent to GAMMAGARD LIQUID therapy due to enhanced RBC sequestration; acute hemolysis, consistent with intravascular hemolysis, has been reported.3-6 [see Adverse Reactions]The following risk factors may be related to the development of hemolysis: high doses (e.g., ≥2 grams/kg, single administration or divided over several days) and non-O blood group.3 Underlying inflammatory state in an individual patient may increase the risk of hemolysis3 but its role is uncertain.6,7 Monitor patients for clinical signs and symptoms of hemolysis [see Warnings and Precautions], particularly patients with risk factors noted above. Consider appropriate laboratory testing in higher risk patients, including measurement of hemoglobin or hematocrit prior to infusion and within approximately 36 to 96 hours post infusion. If clinical signs and symptoms of hemolysis or a significant drop in hemoglobin or hematocrit have been observed, perform additional confirmatory laboratory testing. If transfusion is indicated for patients who develop hemolysis with clinically compromising anemia after receiving IGIV, perform adequate cross-matching to avoid exacerbating on-going hemolysis.Transfusion-Related Acute Lung Injury (TRALI): Non-cardiogenic pulmonary edema (TRALI) has been reported in patients following treatment with IGIV products, including GAMMAGARD LIQUID. TRALI is characterized by severe respiratory distress, pulmonary edema, hypoxemia, normal left ventricular function, and fever. Symptoms typically occur within 1 to 6 hours after treatment. Monitor patients for pulmonary adverse reactions. If TRALI is suspected, perform appropriate tests for the presence of anti-neutrophil and anti-HLA antibodies in both the product and patient serum. TRALI may be managed using oxygen therapy with adequate ventilatory support.Transmittable Infectious Agents: Because GAMMAGARD LIQUID is made from human blood and may carry a risk of transmitting infectious agents, e.g., viruses, the variant Creutzfeldt-Jakob disease (vCJD) agent, and theoretically, the Creutzfeldt-Jakob disease agent. This also applies to unknown or emerging viruses and other pathogens. No confirmed cases of viral transmission or vCJD have been associated with GAMMAGARD LIQUID. All infections thought by a physician to possibly have been transmitted by this product should be reported by the physician or other healthcare provider to Baxalta US Inc., at 1-800-423-2090 (in the U.S.).Monitoring: Laboratory Tests: Periodic monitoring of renal function and urine output is particularly important in patients judged to be at increased risk of developing acute renal failure. Assess renal function, including measurement of BUN and serum creatinine, before the initial infusion of GAMMAGARD LIQUID and at appropriate intervals thereafter.Consider baseline assessment of blood viscosity in patients at risk for hyperviscosity, including those with cryoglobulins, fasting chylomicronemia/markedly high triacylglycerols (triglycerides), or monoclonal gammopathies, because of the potentially increased risk of thrombosis.1,2

If signs and/or symptoms of hemolysis are present after an infusion of GAMMAGARD LIQUID, perform appropriate laboratory testing for confirmation.If TRALI is suspected, perform appropriate tests for the presence of anti-neutrophil antibodies and anti-HLA antibodies in both the product and patient’s serum.Interference with Laboratory Tests: After infusion of IgG, the transitory rise of the various passively transferred antibodies in the patient’s blood may yield false positive serological testing results, with the potential for misleading interpretation. Passive transmission of antibodies to erythrocyte antigens (e.g., A, B, and D) may cause a positive direct or indirect antiglobulin (Coombs’) test.

ADVERSE REACTIONSMMN: Intravenous Administration: The serious adverse reactions in the clinical trial for MMN were pulmonary embolism and blurred vision. The most common adverse reactions for MMN (observed in ≥5% of subjects) were headache, chest discomfort, muscle spasms, muscular weakness, nausea, oropharyngeal pain, and pain in extremity.

• Renal dysfunction, acute renal failure, osmotic nephrosis, and death may occur in predisposed patients with immune globulin intravenous (IGIV) products including GAMMAGARD LIQUID. Renal dysfunction and acute failure occur more commonly with IGIV products containing sucrose. GAMMAGARD LIQUID does not contain sucrose.

• For patients at risk of thrombosis, administer GAMMAGARD LIQUID at the minimum dose and infusion rate practicable. Ensure adequate hydration in patients before administration. Clinical Trials Experience: Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug and may not reflect the rates observed in clinical practice.MMN: Intravenous Administration: The safety of GAMMAGARD LIQUID was evaluated in 44 subjects with MMN who received a total of 983 infusions. Two serious adverse reactions, pulmonary embolism, and blurred vision occurred. In the study, among the 317 non-serious AEs, 176 were considered ARs. Of these, 126 were mild (transient discomfort that resolves spontaneously or with minimal intervention), 37 were moderate (limited impairment of function and resolves spontaneously or with minimal intervention with no sequelae) and 13 were severe (marked impairment of function or can lead to temporary inability to resume normal life pattern; requires prolonged intervention or results in sequelae). Adverse reactions with a frequency of ≥5% (defined as adverse events occurring during or within 72 hours of infusion or any causally related event occurring within the study period) are shown in Table 1.

DRUG INTERACTIONS Passive transfer of antibodies may transiently impair the immune responses to live attenuated virus vaccines such as mumps, rubella and varicella for up to 6 months and for a year or more to measles (rubeola). Inform the immunizing physician of recent therapy with GAMMAGARD LIQUID so that appropriate precautions can be taken.

USE IN SPECIFIC POPULATIONSPregnancy: Pregnancy Category C. Animal reproduction studies have not been conducted with GAMMAGARD LIQUID. It is also not known whether GAMMAGARD LIQUID can cause fetal harm when administered to a pregnant woman or can affect reproduction capacity. Immune globulins cross the placenta from maternal circulation increasingly after 30 weeks of gestation. GAMMAGARD LIQUID should be given to a pregnant woman only if clearly indicated.Nursing Mothers: It is not known whether GAMMAGARD LIQUID is excreted in human milk. Because many drugs are excreted in human milk, caution should be exercised when GAMMAGARD LIQUID is administered to a nursing woman.Pediatric UseMMN: Safety and effectiveness in pediatric patients with MMN have not been established.Geriatric Use MMN: GAMMAGARD LIQUID was administered intravenously for treatment of MMN in 5 subjects 65 years and above. There were insufficient numbers of subjects aged 65 and over to determine whether they respond differently than younger subjects. [see Boxed Warnings and Warnings and Precautions]
