What is heart failure?Heart failure is when a weakened heart can no longer pump enough oxygen and blood to the rest of your body. This results in fatigue and shortness of breath. Simple activities may feel almost impossible.

When should I call my doctor?There are many symptoms of heart failure. But, by learning some of the warning signs of heart failure you will have a better understanding of when to call your physician. For more information, see the chart called “Heart Failure Rules & Warnings” in this brochure.

Why did my physician refer me to the clinic?Heart failure is a chronic disease that you will need to manage throughout your life. Many people who are diagnosed with heart failure

will end up in the hospital multiple times due to their illness.

By referring you to the heart failure clinic, we are able to work alongside your physician to treat you and help you manage your heart failure, in order to help you to live a better quality of life. By working with our staff, you may reduce the amount of visits to the hospital due to your condition.

You can expect an appointment with our clinic within one week of your hospital discharge. To manage your heart failure, our staff will:

• Listen to your concerns.

• Provide you with education on your condition.

• Review your medications.

• Help you find additional services, if you need them.

What can I expect from my visit?During your appointment we will:

• Assess your current health.

• Review your medications. And make sure they are working for you.

• Provide you with additional patient education.

• Address your concerns.

What should I bring to my appointment?You should bring a list of current medications you are taking as well as the actual prescription bottles.

Who staffs the heart failure clinic?The heart failure clinic is staffed by a nurse practitioner, registered nurse and medical/clinical assistant. Our staff will not only work closely with you as our patient, but we’ll work closely with your physician. They will be kept up-to-date on your progress.

The clinic offers post-hospital discharge appointments and follow-up appointments as needed.

Services provided:• Educate you about your medications and

how they work.

• Teach you how to manage your heart failure and control your symptoms.

• Make recommendations to your current treatment plan, if needed, based on the most recent research and guidelines for heart failure management.

• Treat and monitor your acute heart failure symptoms with IV medications (diuretics) in our office, if necessary.

• Partner with your primary care physician, cardiologist, and other providers to carefully adjust your medications.

• Provide convenient access to advanced heart failure specialist if your cardiologist recommends more advanced treatment options such as heart transplant or LVAD.

The heart failure clinic is located in the Heart Center at St. Luke’s Hospital. Please use Entrance No. 3 and park in the Outpatient Center parking lot. Enter into the Outpatient Center and follow the signs to the Heart Center. If you need assistance getting to the Heart Center via a wheelchair, our volunteers are available to assist you.
