Students are expected to use PEDs and School-Based Technology in a courteous, respectful, and otherwise appropriate manner consistent with school and CISVA policies, specifically the CISVA Acceptable Use of PED and School Based Technology Policy, and the guidelines and expectations outlined in the School’s code of conduct. 2. Prohibited uses of PEDs and School-Based Technology include, but are not limited to, the use of PEDs or School-Based Technology that: a. Compromises the academic integrity of the school or an individual within the school. b. Interferes with or disrupts the academic day or the teaching/learning environment. c. Violates a person's reasonable expectation of privacy (including, but not limited to, taking, distributing, or posting photos of other persons without their consent). d. Compromises personal and/or school safety (including, but not limited to, cyber bullying, and posting information about themselves or others that may put them at risk). e. Facilitates illegal and/or unethical activities, including but not limited to: i. transmitting materials in violation of Canadian laws; ii. receiving, viewing, duplicating, storing, or transmitting pornographic materials; iii. transmitting or posting threatening, abusive, or obscene messages or materials; iv. duplicating, storing, or transmitting any material that contravenes the Copyright Act; v. installing or reproducing unauthorized or unlicensed software; vi. sending, linking to, or otherwise making available material likely to be offensive, objectionable, or pertaining to criminal skills or activities; Page 1

Student Acceptable Use of PED and School Based Technology Agreement

Student and Parent Consent Form 2015-2016

We agree and understand that:

1. Students are expected to use PEDs and School-Based Technology in a courteous, respectful, and otherwise appropriate manner consistent with school and CISVA policies, specifically the CISVA Acceptable Use of PED and School Based Technology Policy, and the guidelines and expectations outlined in the School’s code of conduct.

2. Prohibited uses of PEDs and School-Based Technology include, but are not limited to, the use of PEDs or School-Based Technology that:

a. Compromises the academic integrity of the school or an individual within the school. b. Interferes with or disrupts the academic day or the teaching/learning environment.c. Violates a person's reasonable expectation of privacy (including, but not limited to, taking,

distributing, or posting photos of other persons without their consent).d. Compromises personal and/or school safety (including, but not limited to, cyber bullying, and

posting information about themselves or others that may put them at risk).e. Facilitates illegal and/or unethical activities, including but not limited to:

i. transmitting materials in violation of Canadian laws;ii. receiving, viewing, duplicating, storing, or transmitting pornographic materials;

iii. transmitting or posting threatening, abusive, or obscene messages or materials;iv. duplicating, storing, or transmitting any material that contravenes the Copyright Act;v. installing or reproducing unauthorized or unlicensed software;

vi. sending, linking to, or otherwise making available material likely to be offensive, objectionable, or pertaining to criminal skills or activities;

vii. utilizing applications to facilitate the downloading or exchange of music, movies, games or other materials in contravention of the Copyright Act;

viii. forging any document or message; obscuring the origin of any message, transmission, or file;

ix. using programs that harass users, prevent access, investigate, or infiltrate computer systems /or software components; and

x. promoting commercial uses or product advertising; andxi. participating in online gambling sites.

3. Students may not electronically transmit or post digital content (i.e. images, video, and/or text etc.) of any staff or students, either through the use of school-issued PEDs, School-Based Technology or personal PEDs while on school property without the permission of the person or persons involved and the consent of the parent/guardian, unless permission has been previously granted.

4. Students are responsible for their own individual account and will take all reasonable precautions to prevent others from being able to use their account. They are not permitted to share their password with other students. Students will change their password, seeking assistance from a staff member if necessary, if they believe that others may know of their password.

5. Students may not attempt to gain unauthorized access or go beyond my authorized access by entering another person’s account password, accessing another person’s files, or ‘hacking’ into any unauthorized accounts.

6. Students may not intentionally disrupt, or attempt to disrupt, School-Based Technology or any other computer system, or destroy data by spreading computer viruses or by any other means.

7. Students may not disable or otherwise interfere with or modify the virus scanning, security or network settings.

8. Students will immediately notify a teacher upon discovery of a possible security problem. 9. Students will not download or attempt to run or store any app and/or program file not directly

authorized by the school.10. Students will not attempt to install any software applications. All software is to be installed and

configured by school staff.11. Students will not take actions that place an excessive load on the system as to restrict or inhibit other

Users from using School-Based Technology or impacting the efficiency of the system.12. Students will not make any intentionally attempt to bypass any school’s filters or access any blocked

materials unless told to do so by a staff member.13. The CISVA and school is not responsible for any damage that may occur to a PED as a result of

connecting to any School-Based Technology or any electrical power source.14. Parents/guardians are responsible for and will reimburse the school for any damage that a student may

cause through the use of School-Based Technology with their PED.15. Students are ultimately responsible for ensuring that their use of School-Based Technology or PEDs on

school property and during school hours is done in accordance with the laws of Canada and British Columbia, and that if a student has any questions about the lawful use of School-Based Technology or PEDs on school property and during school hours, they should ask their parent/guardian.

16. The CISVA and school are not responsible for any damage to, theft or loss of a personal PED that a student brings to school.

17. Students have a diminished expectation of privacy on any PED brought into the school. When and where school administrators and/or teachers have a reasonable suspicion that a student may be in breach of school policy, they may search and/or temporarily confiscate my PED.

18. The school principal, or its delegate, has the right to monitor any network activity that utilizes School-Based Technology in order to maintain its operation and appropriate function.

19. If a student breaches this policy or otherwise engages in inappropriate use of PEDs on school property, PEDs during school hours, a school-issued PED or School-Based Technology generally that they will be subject to sanctions including the restriction of their ability to access and use PEDs or School-Based Technology and may be subject to disciplinary action. Failure to comply with this Agreement may result in their losing the privilege of utilizing School-Based technology or PEDs on school property and during school hours, and other disciplinary actions consistent with school and CISVA policy.

20. Violations of this policy may be reported to the appropriate law enforcement authorities and may also be subject to criminal investigations and/or criminal charges.

I have read and understood this Agreement and recognize that it is based on the CISVA Acceptable Use of PED and School Based Technology Policy, available on the CISVA’s website. I will abide by the terms and conditions described within this Agreement.

Parent/Guardian Consent

I agree that:

1. I have read and understood this Agreement and recognize that it is based on the CISVA Acceptable Use of PED and School Based Technology Policy, available on the CISVA’s website;

2. I recognize that this Agreement is designed for my child’s grade level and that the full CISVA Acceptable Use of PED and School Based Technology Policy, is available on the CISVA’s website;

3. I will emphasize the ethical and responsible use of technology to my child;4. I grant permission for my child to use School Based Technology and PEDs, in accordance with any

conditions of the School, and I am aware that my child may be given instruction with respect to the proper use of such technology at school;

5. I understand that the school will, from time to time, and without prior notice to students, access and/or monitor the School-Based Technology for routine or emergency maintenance or repairs;

6. I understand employees of the school may investigate or inspect my child’s PED or use of School-Based Technology in the event there are circumstances which indicate my child’s use may be in breach of the CISVA Acceptable Use of PED and School Based Technology Policy, available on the CISVA’s website;

7. I understand that I may be required to compensate the CISVA in the event that my child’s use of a PED or School-Based Technology causes damage to School-Based Technology, including, but not limited to physical, software and hardware damage.

8. I will emphasize to my child the importance of abiding by the terms and conditions described within this Agreement and the requirements set out in the CISVA Acceptable Use of PED and School Based Technology Policy;

9. I understand that failure to comply with this Agreement may result in my child losing the privilege of utilizing School-Based technology or PEDs on school property and during school hours, and other disciplinary actions consistent with CISVA policy;

10. I will emphasize to my child that he/she is ultimately responsible for ensuring that his/her use of School-Based Technology or PEDs on school property and during school hours is done in accordance with the laws of Canada and that if he/she has questions, to ask me.

Definitions:

Personal Electronic Devices (PEDs): are wireless and/or portable electronic handheld devices, including, for example, cell-phones, tablets (e.g. iPads), laptop computers, and wearable technology (e.g. google glasses, apple watch).

School-Based Technology: means all CISVA school-based networks, including, but not limited to, school servers, school computers, school software, school printers, online services provided by the school, and the networks (wired or wireless), which connect all of the above or to the Internet.
