Hooley Hall is located at the Washington County Fairgrounds 12300 North 40th Street – Lake Elmo, MN 55042
Obedience, Rally, Showmanship on Tuesday nights 6:00 - 8:00 p.m. starting April
17th and running through to September 18th.
Agility will be every Monday and Thursday nights starting April 23rd and
running through to September 20th.
County show, Obedience, Rally, Showmanship will be Saturday July
28th from 8:00 a.m. to 3:00 p.m.
County Agility show will be Sunday July 30th from 7:00 a.m. to 2:00 p.m.
State Dog Show will be September 22nd and 23rd at the MN State Fair grounds.
Must be enrolled in 4-H, additional fees do apply. Dog program fee is $25.00for early birds (by April 3rd) and $35.00 after April 3rd.

4-H Dog Project Forms/ Payments Checklist 2018
The following must be completed by April 3rd before participation in the Dog Project:
1. Active 4-H Member—Enrolled online and paid dues
$45 Check made out to Washington County 4-H
2. Dog ID form completed and signed by veterinarian
If either 1 or 2 is not done, member and dog must be sent home from dog training classes
3. Dog training fee of $35 which includes shirt (***Please note: early bird fee is $25)
Need t-shirt size written on check memo line
4. If dog is in agility, a completed and signed form is required
5. If dog is being leased, a completed and signed form is required
6. ID (with photo), Agility and Lease forms can all be uploaded by family to 4-HOnline
through their family account. If unable to upload than all forms need to be sent to
4-H office

RE: IMPORTANT - Animal Identification Process
Thank you for enrolling in a 4-H animal science project. We hope you find this project both challenging
and rewarding. This letter outlines the online animal identification process.
For the fifth year, Minnesota 4-H is using its online enrollment system, 4HOnline, to record all animal
identifications. Online animal identification reduces inaccuracies and inefficiencies that were a part of
our past paper system. More information and a 4HOnline user guide can be found at www.4-
H.umn.edu/animal-ID.
The process that will be followed in 2018 is as follows:
1. 4-H Family completes the “Affidavit and Vaccination Verification” form (enclosed). You will
need a veterinarian’s signature, so plan ahead to have this done prior to beginning training the
last week in April.
2. Existing project animals that are being carried over to this project year must be re-activated by
May 15th. Instructions can be found in the document Animal ID Data Entry by Families, page 5,
at www.4-H.umn.edu/animal-ID.
3. 4-H Family self-enters information for ALL project Dogs into 4HOnline and prints entry
report for personal record. One Dog can be ID'd be multiple family members. In those cases,
the same animal should be entered under EACH member ID'ing that animal. Families should
refer to the Minnesota 4-H Dog Rule Book at www.4-H.umn.edu/Dog for details and limitations.
If an animal is leased, the lease agreement must be scanned and uploaded in 4HOnline or
submitted to the 4-H office. The lease form is (attached in this form) is also available at www.4-
H.umn.edu/animal-ID. Upload photos in a .jpg file in 4HOnline.
NOTE: The final deadline for dogs to be identified is May 15th, forms must be postmarked or
submitted to the County Extension Office by May 15th or brought to the orientation.
Families can complete the ID process (meet all requirements to ID) without having a vaccination form,
but they must have submitted or uploaded a valid vaccination form before the dog can participate
in any 4-H event, including training. It is the 4-H’ers responsibility to update the vaccination form as
soon as it comes due. Like the lease form, if a family doesn't have the ability to scan and upload the
affidavit and vaccination verification form, do the rest of the ID process online, and then submit the
form to the office in hard copy.

If your family cannot self-enter the ID information: 4-H Family brings in or mails completed ID
worksheet and other required items, including signature(s), to County Extension office, on or before
close of business (4:30pm) May 15th.
ID worksheets can be turned in at Dog Orientation on April 3th or any time prior or after
that at the county office. Please ensure that the worksheet is given only to Heather
Cory so we can ensure proper record keeping. This ID worksheet will be kept on file
in the Extension office.
Within one week, Support Staff or designated volunteer enters info online into your
family’s 4HOnline account. After that, your family may view the information in
4HOnline to check for accuracy. The user guide, Reviewing and Printing Animal ID
Data, is available at www.4-H.umn.edu/animal-ID to walk you through the process.

project and every family should have a copy of the rule books that apply to their participation
(Obedience, Showmanship, Rally and/or Agility). If you can’t go online, contact your County Extension
Office for a copy.

Exhibitors: Take this form to your veterinarian. This Vaccination Certificate must be completed and signed by a licensed, accredited veterinarian. All dogs MUST have current rabies vaccinations. Your veterinarian must document on this form whether your dog’s Rabies vaccination is current for 1 year, 2 years, or 3 years by filling in the “Date Expires” blank. Additionally, a current Distemper, Hepatitis, Leptospirosis, Parainfluenza, and Parvovirus (DHLPP) is required for 4‐H dogs, unless otherwise noted, either by titer or dog is unfit for vaccination, below by veterinarian. In case of vaccination being waived by a veterinarian or a titer, proof must be included with this form.
