13 Skin lesions are not well described by the current Annex L Body Part Examined anatomic codes.

14 Many dermatologists use the existing New York University Melanoma Clinical Cooperative Group's numbering system. Another15 similar scheme derived from this is used by the Mayo Clinic.

16 A group of dermatologists have recently developed a new hierarchical terminology, mapped where possible to SNOMED CT and17 NYU concepts, which they have contributed to WHO ICD11 as an Extension code set (Specific Anatomic Detail > Topography >18 Body Regions > Skin). Their hierarchy and mapping is published at http://anatomymapper.com/terms/, and their approach is described19 in a forthcoming article Kenneweg, KA, Halpern, AC, Chalmers RJG, Soyer, HP, Weichenthal, M, Molenda M. Developing an20 international standard for classification of surface anatomical location for use in clinical practice and epidemiological research. Journal21 of American Academy of Dermatology. In Press. The mapping in this CP has been cross-checked with their mapping and where22 possible made consistent. Further, where their new terminology includes additional (particularly more granular) concepts not in the23 NYU or Mayo systems, some (but not all) have been added to the new context group. There are some differences in the modeling24 approach. For example, the NYU terms for the fingers distinguish palmar and dorsal whereas the new hierarchical terminology does25 not; conversely, the segments (proximal, intermediate and distal phalanges) of the fingers are not defined in the NYU scheme yet26 are in the new hierarchical terminology, as are the perionychia. Not all of the new hierarchical terminology proposals have been27 incorporated in the ICD-11 extension (i.e., have codes), so for the time being, these are not included in the context group proposed28 here.

29 Traditionally in DICOM, and in the new dermatology hierarchical terminology, laterality is post-coordinated, so the same approach30 is followed here. This leads to a higher yield of SNOMED CT mappings since laterality is rarely pre-coordinated in SNOMED CT by31 policy. To support the post-coordination of laterality, including the concept of the median (mid-line, as opposed to left or right) part32 of a structure, an extended version of the Laterality context group is also added.

33 Code String CS values for Body Part Examined are not added since the Anatomic Region Sequence is expected to be used instead.34 Correction Wording:

10 1. It is desirable to use more general standard coding schemes such as SNOMED CT whenever possible, to support inter-11 operability across specialist domains, yet the numeric codes from the NYU and Mayo systems are well known to derma-12 tologists. Multiple codes may be encoded in the image by using the equivalent code mechansism. See PS3.3 8.913 "Equivalent Code Sequence" . Standard Coding Scheme Designators (NYUMCCG and MAYOASRG) are defined.

SNOMED CT and FMA "skin of" or "mucosa of" specific concepts are used here when available, rather than the more15 generic "structure of" concepts; this sacrifices commonality with the anatomic regions used for more general applications,16 but is appropriate for dermatologic applications. E.g., (T-02508, SRT, "Skin of anus") is used instead of (T-59900, SRT,17 "Anal structure").

18 3. The DICOM convention is to use "stucture of" concepts rather than "entire" concepts when both are defined, and that19 convention is followed here. E.g., (T-02136, SRT, "Skin structure of lateral canthus") is used in preference to (T-0213D,20 SRT "Entire skin of lateral canthus").

21 4. The illustrations of the NYU anatomy identify two separately numbered structures associated with the female urethral22 orifice; it is not clear what separate structure is identified (perhaps Skene's Glands (71648, FMA, "Lesser vestibular23 glands"), but in the absence of further information, it is assumed that NYUMCCG:504 refers to the urethral orifice and24 NYUMCCG:517 is ignored (i.e., not included in this Context Group).

25 5. The illustrations of the NYU anatomy use female genitalia to show the genitocrural fold, but it is believed that the intent26 is to identify the region irrespective of sex, so (T-0104, SRT, "Groin skin crease") is used rather than (T-02512, SRT,27 "Skin of crurovulvar fold").

28 6. There is some uncertainty regarding the distinction between the antitragus and the intertragal incisure and whether the29 latter is represented at all in the NYU and Mayo drawings. The more recent mapping decision of Kenneweg et al has30 been adopted here, even though the text meaning differs from that for the NYU and Mayo schemes.

31 7. There is some uncertainty regarding the distinction between the skin of the forehead and the frontal region of the scalp;32 (T-02104, SRT, "Skin of forehead") is used rather than (R-FB4E1, SRT, "Skin of frontal region of scalp"), even though33 the later would be more consistent with the other scalp regions (parietal, temporal and occipital).

34 8. Laterality is pre-coordinated in the NYU and Mayo codes, but by convention is factored out and encoded seperately in35 DICOM. See CID nnn0 “Laterality with Median”.
