Maintenance supervisor who is sitting next to the driver orders to far away from well and reports the incident to the Fire Station by using Handy Talky (HT). HSE Supervisor at Fire Station reports the incident to the On Scene Commander then directly forwarded to the Incident Commander and Emergency Response Commander as well.

I. Event Scenario

Production well under normal conditions of operation when the Pick Up vehicle of Maintenance to the location where the road is in slippery conditions because of the rain and makes the Pick Up slips right to Christmas Tree with no fences leading to crash and leaking the pipeline flange 4", therefore oil gush occurs to the surrounding and burns due to conclusion causing flame.

The Pick Up Vehicle survived but not burn, but it leads two maintenance workers who sit in the back of a pick up inhaling H2S gas which comes up along with the oil gush at the scene.

Maintenance supervisor who is sitting next to the driver orders to far away from well and reports the incident to the Fire Station by using Handy Talky (HT).

HSE Supervisor at Fire Station reports the incident to the On Scene Commander then directly forwarded to the Incident Commander and Emergency Response Commander as well.

Incident Commander with the approval of the Emergency Response Commander declares that situation is emergency and orders to activate the emergency sirens.

Incident Commander commanding the On Scene Commander along with the Fire Brigade immediately go to the scene and take the response action.

Emergency response is accomplished and the Incident Commander by the approval of the Emergency Response Commander declares that the location is safe.

Security Officer conducting evacuation towards the community at vicinity location of Christmas Tree (well #12) in order to prevent the victims due to the H2S gas exposure.

At the scene, HSE Officer measures the H2S gas surrounding the scene in order to ensure that H2S gas released not more that TLV-TWA (ACGIH- 10 ppm) that can endanger the team within the emergency response. Based on this measurement appointed that the safe radius is 40 m from the gush source.

On Scene Commander coordinates the emergency response by ordering Fire Brigade to use Breathing Apparatus when doing fire fighting and to close the valve on Christmas Tree to stop the gush of oil and gas containing H2S.

Fire Brigade conducting fire fighting by using Water Crash Tender with the strategy to close the valve by making water curtain from Water Spray Nozzle 2,5 amounted 2 units on right and left position along the fire man in the middle whose the task is close the valve move forward together as wind direction.

Valve can be closed rapidly so the oil gush can be stopped and the fire can be extinguished, thus continue with cooling to prevent the flash back.
