“Asset Integrity course for senior managers to be investigated” The previous Asset Integrity workshops date back to 2007-09, the context of the action was to re run this to refresh and take account of senior personnel turnover in the industry. The sessions were aimed at senior leadership and delivered to Maersk, Nexen, Woodgroup, Total, Apache, Talisman etc etc.
• The workshops focused to some degree on the HSE KP3 process and were intended to raise awareness of asset integrity to industry leaders especially targeting non-technical people commercial finance ec delivered over 1 day, also looked at major accident case histories.

1. Analyse the structure of the OIR 12 with a view to minimising the number of questions presented to the respondent for a given incident.
2. Provide a subsidiary questionnaire to cover elements thought to be missing from the current question set eg:-
a. details of incidents arising from inadequate process isolation standards/permit to work controls (Detailed questionnaire incorporating the scope of process isolation detailed in HSG 253 and qualitative failure mechanisms indicating multi causal factors contributing to a hydrocarbon release).
b. causation factors related to ageing and life extension issues
c. others?.....
3. Time permitting:-
(a) testing of the new structured questionnaire on past reported incidents
(b) interrogation of past incident data to reveal multi causal factors in standards of process isolation/permit to work related incidents with a view to identifying trends ie ranking of most common failures, and validation of new detailed question set "
