Am I Eligible for Benefits?

Full-time employees are eligible for bene!ts after the 1st of the month following 30 days of service. Employee’s whose 30th day falls on the !rst will be eligible on the !rst of that month. A full-time employee is an employee working 30 hours or more a week on a regular basis.

Spouse/Dependent Child Eligibility

Eligible dependents include:

• Your legally-married spouse

• Your legally-de!ned domestic partner, regardless of sex

• Your children, under age 26, including child by birth, stepchildren, legally adopted children, and children for whom you have been appointed a legal guardian

• Your unmarried grandchild, under the age of 19, who resides with you and is not employed on a full-time basis, provided the grandchild’s parent is under the age of 26 AND also covered under the Monro health care plan

You may be required to provide proof of eligibility for any new dependent you add to your 2021 coverage under the Medical, Dental, and Vision plans. Monro, Inc. may conduct a dependent eligibility audit at any time.

Alternatives to Monro’s Healthcare Plans

While Monro offers health insurance to eligible teammates as de!ned by the Affordable Care Act, there is also another way for you and your family to purchase health insurance as part of Healthcare Reform Legislation. The Marketplace, also known as The Healthcare Exchange, offers “one-stop shopping” to !nd and compare private health insurance options. A great resource that is easy to use (by zip code search) and available throughout the year in the event your needs change is http://www.healthcare.gov. You can also call 1-800-318-2596 to speak to a bene!t representative.

Discovery Bene!ts Inc. also provides Monro teammates with resources to !nd affordable healthcare bene!ts at www.discoverymarketplace.com. You can also call 1-855-633-5315 to speak to a bene!t representative. Either resource will provide you with a viable resource to help you make an informed decision about the right healthcare plan for you and your family.

Take these steps to have a successful enrollment:

Between November 29 and December 10, log in to https://monro.smartben.net or by using the SmartBen NOW app to review your suite of Monro bene!ts and enroll. (See next page for more information.)

Check to see if your doctor and/or dentist is in any of the networks

You MUST enroll if you want the following:

Decide whether you want to cover dependents

Documentation may be required to verify eligibility for any NEW dependents.

Make sure you have the Social Security number and date of birth for each of your dependents handy. You must enter them during the enrollment process.

Make sure the bene!ciary information for your Company Basic Life Insurance, Supplemental Life, Whole Life, and Critical Illness bene!ts is up to date.

ENROLL BY THE DEADLINE! If you need to make any changes, you must do so by December 10.

HOW TO ENROLL

What if I moved to part-time in 2021 and am now back to full-time status? Do I need to enroll?

Action Required: You must enroll if you did not re-enroll in bene!ts upon moving back to full-time status in 2021.

How to View Your Status: Check out the tile “Full-Time/Part-Time Status” in SmartBen.

What if I currently am enrolled in benefits? Do I need to enroll?

No Action Required: Unless you want to make changes for 2022, any bene!ts you are currently enrolled in for 2021 (except the FSA) will automatically carry over to 2022.

You MUST enroll if you want to contribute to the Flexible Spending Account (FSA) in 2022.

When can I enroll?

November 29 through December 10, 2021

REMEMBER!

You must designate a beneficiary for your Company-provided Life, Supplemental Life, Whole Life, and Critical Illness insurance.

To enroll dependents, you must provide Social Security numbers and dates of birth.

How Do I Enroll? Log on to https://monro.smartben.net:

1. Enter your Username and Password

• Username: The word “monro” + your 6-digit teammate ID number

• Default Password (passwords have been reset for enrollment): Your date of birth as MMDDYYYY

2. From the home page, click the Begin Enrollment icon, then the Annual Enrollment button.

3. Click on “Manage People” on the right hand side of the screen to enter dependent and bene!ciary information.

4. To make a change, select the plan you wish to change, then click on the coverage election you want. Click “Con!rm Election” at the bottom of the page when you are !nished.

Any bene!t that has a “red light” next to it will require your attention before you can continue.

Any bene!t option that has a “green light” will roll over into 2022 if no changes are made. To review and/or make changes, simply click into the bene!t.

5. Enter your initials at the bottom of the Con!rmation page and click Continue.

6. Congratulations! Success! You have enrolled. Print the con!rmation statement for your records.

HOW DO I MAKE QUALIFYING LIFE EVENT CHANGES?

If you have experienced a Qualifying Life Event, you can either:

Download the SmartBen NOW app and complete your request from your smartphone

Contact the Monro Bene!t Resource Center at 1-888-395-8631 to con!rm your eligibility to make a change

Log on to https://monro.smartben.net, and select “Life Event”

With this change:

You will be required to provide supporting documentation to verify the life event and new dependents

Retroactive payroll deductions apply

You must request a Qualifying Life Event benefit change AND provide supporting documentation within 30 days of the event or you will not be able to make the change until the next annual enrollment period. (See Medicare, Medicaid, CHIP exceptions below.)

You will receive instructions on how to verify a new dependent at a later date.

Qualifying Life Events include:

• Marriage, divorce, legal separation, or death of spouse

• Change in your or your spouse’s work status that affects bene!ts eligibility

• Termination or commencement of your spouse’s employment

• A dependent child becoming eligible or ineligible for coverage

• Birth, adoption, placement for adoption, or death of a dependent child

• Becoming eligible and enrolling in Medicare, Medicaid, or CHIP (the Children’s Health Insurance Program available through the federal government) – MUST be reported within 60 days

FLEXIBLE SPENDING ACCOUNT (FSA)

Health care #exible spending accounts can save you money by allowing you to set aside tax-free dollars to pay for eligible health care expenses.

Am I Eligible for an FSA?

Monro has reduced the eligibility waiting period to participate in a Flexible Spending Account (FSA) to the 1st of the month following 30 days!

Teammates who elect either the Kaiser Traditional HMO plan or the Kaiser Bronze HDHP plan may enroll in an FSA.

Can I Make Changes to or Enroll in an FSA Mid-Year?

No, you must wait until the next annual enrollment.

Elements of an FSA

Provides you with a tax-free way to pay for eligible Medical, Dental, and Vision expenses – such as eyeglasses, deductibles, coinsurance, and orthodontia – that are not reimbursed or covered by another plan

Each year you may contribute up to the IRS Limit. For 2022, the limit is $2,850.

Rollover up to $570 into the next calendar year

Monro will cover all maintenance fees saving you approximately $40 per year!

Funding and Using Your FSA

Your annual contribution amount is divided into equal amounts and is deducted from your paycheck, pre-tax, each pay period. Eligible expenses are then reimbursed from your account. If you terminate employment, your FSA debit card will terminate on the same date. You can still submit expenses incurred prior to termination for up to 90 days.

How Can I View My Account Balance

The website https://bene!tslogin.wexhealth.com/Login allows you to view your balance, !le a claim, or submit a receipt. Search for the WEX mobile app in the App Store or Google Play.

How to Get Reimbursed for Expenses

When you enroll in an FSA you may:

• Use your Flexible Spending Account VISA® debit card and have the eligible expense debited directly from your account.

• Pay for eligible expenses out-of-pocket and submit a claim for reimbursement (contact WEX

at 1-866-451-3399)

Use It or Lose It

Any unused funds (above $550) remaining in your Flexible Spending Account after December 31, 2023, will be forfeited to the plan.

VISA® Debit Card

You can use your Flexible Spending Account VISA® debit card to pay for eligible copays, deductibles, and healthcare products and services from quali!ed locations, including hospitals, physician of!ces, vision providers, pharmacies, and home healthcare services. Keep all receipts in case you need to submit one or more to WEX for validation.

Special Note for 2022

Under the Consolidated Appropriations Act of 2021, FSA holders will be allowed to carry whatever amount remains in their account on December 31, 2021 into the 2022 calendar year (the $570 carry over limit for 2021 has been temporarily suspended). 2021 funds must be used by December 31, 2022 or they will be forfeited to the plan.

DENTAL INSURANCE

The plan features a network of dentists and specialists who have agreed to provide services at a discounted price. You’ll pay less out-of-pocket if you stay in the network.

*Dental Plan 1: There is a 6-month waiting period for basic and major services if you previously declined dental coverage.

Disability Insurance (Short-Term)

A disabling illness can strike at any time. Are you !nancially prepared?

State Mandated STD Benefits

Some states have mandatory, income based short-term disability bene!ts that you will be eligible to receive after a 7-day waiting period. However, these bene!ts can fall quite short of providing you what you need !nancially while out of work.

Filing for Short-Term Disability in California?

1. Advise your supervisor of the need to be out of work longer than 7 days.

2. Contact Lincoln Financial at 1-800-213-7322 or !le a claim at www.mylincolnportal.com. Provide any requested documentation directly to Lincoln.

3. All enrolled bene!ts will continue during STD duration; premiums will be billed directly to you on a monthly basis.

Returning to Work after Short-Term Disability

1. Contact your Supervisor to let them know of your intended return to work date.

2. Provide return to work clearance from your physician to your Supervisor, Lincoln Financial and the HR Bene!ts Team (email: [email protected]). Any bene!t premiums missed while you are out on disability will be deducted along with normal pay period deductions upon your return to work.

DISABILITY INSURANCE

Long-Term Disability (LTD)

Monro provides LTD coverage automatically to full-time teammates April 1 or October 1 after 12 months of continuous service. After 90 days of disability (your short-term disability period, if covered), LTD will replace 60% of average weekly wages (to a maximum of $7,000 per month) if you cannot return to work.

This bene!t will continue until you return to work, reach Social Security Normal Retirement Age, or per the Reduced Bene!t Duration schedule shown below, or if you die, whichever comes !rst.

In accordance with the provision of the policy, your payable bene!t is offset by any bene!t received under a Monro, Inc. de!ned bene!t pension plan, Social Security, or other state mandated bene!t program.

If your disability is due strictly to a work-related injury, Workers’ Compensation bene!ts are an offset to the LTD bene!t.

Disability bene!ts are not automatic. All claims are subject to the approval of the insurance carrier, Lincoln Financial.

Basic Life and AD&D Insurance - No Cost to You!

Monro provides Basic Life Insurance and Accidental Death and Dismemberment (AD&D) insurance for full-time teammates through Lincoln Financial at no cost. You are automatically eligible for coverage on the !rst of the month following 30 days of continuous employment. No action is required on your part to receive this bene!t, however YOU MUST provide your beneficiary designation(s) during enrollment. Mandatory coverage reductions will occur at ages 70 (35% off original coverage amount) and 75 (50% off original coverage amount).

Supplemental Life and AD&D Insurance

Monro offers additional low-cost Supplemental Life Insurance through Lincoln Financial to all eligible teammates, their spouses and children. Teammates must insure themselves in order to insure their dependents. Teammate speci!c Supplemental Life Insurance premiums will be provided in SmartBen during enrollment.

Increase coverage by one level during subsequent annual enrollment periods

Teammates will be required to complete Evidence of Insurability for Supplemental Life Insurance if:

Enroll in more than $150,000 of coverage or increase coverage more than one increment for employee or spouse

Enroll in employee, spousal or child(ren) coverage after the initial eligibility period

LIFE INSURANCE

Premier Whole Life Insurance through Voya

Whole Life Insurance is designed to provide life insurance coverage over a lifetime. Its features include life insurance protection, tax-deferred* cash accumulation (based on current tax laws) and cash value loan privileges – all in one policy.

The premium you pay for your Premier Whole Life Insurance coverage is based on the death bene!t you select, as well as your age and tobacco status. Plus, the insurance coverage, premium amounts and cash value are guaranteed as long as you make

the required premium payments. You can see your individual rate options in SmartBen.

VOLUNTARY BENEFITS

Critical Illness Insurance1 through Voya

Helps cover the extra expenses associated with a serious illness such as heart attack, stroke, major organ failure, end stage renal failure, invasive cancer and more. This coverage, provides a lump-sum payment upon diagnosis. In the event you suffer more than one covered condition, another bene!t may be payable. Critical Illness insurance is designed to supplement your health insurance, NOT replace it.

A wellness bene!t of $50 is available to you and your spouse and $25 per child, up to $100 per family, when submitting a receipt for health screening test(s).

Teammates MUST insure themselves in order to insure their spouse or child(ren).

Teammate speci!c premiums will be provided in Smartben upon enrollment.

401(K) PLAN

Monro offers a competitive retirement bene!ts package to eligible teammates through 401(k) contributions, as well as tools and resources you need to plan for your future.

When Am I Eligible and When Can I Enroll?

Immediately upon hire as long as you are 18 years of age or older

You can enroll the week after you receive your !rst pay check from Monro

Your 401(k) Contributions

For every $1.00 you contribute, Monro will contribute $0.50 up to a maximum match of 3% per pay period. In other words, Monro matches 50% of the !rst 6% you save, for a maximum match of 3% per pay period.

You can contribute any percentage from 0% to 50% of your pre-tax pay up to the annual IRS limit of $20,500 in 2022. (Highly Compensated Teammates are currently limited to 1.92%.)

You may change (increase or decrease) your contribution at any time during the year.

If you are 50 or older in 2022, you can make additional catch up contributions up to the IRS Limit of $6,500 annually once you contribute the IRS maximum regular contribution of $20,500 for 2022. Catch-up contributions are not eligible for matching contributions.

100% vested in your contributions and the earnings on those contributions.

Monro matching contributions are subject to a vesting schedule. You are 25% vested after 2 years of service, 50% vested after 3 years of service, 75% vested after 4 years of service, and 100% after 5 years of service. Any amounts that are credited to your account but not vested will be forfeited if you separate from service before meeting the service requirements.

Lower your taxes: Contributions are taken out before federal income taxes and can possibly place you in a lower your tax bracket.
