The purpose of the 2019 NCDB Survival Guide is to:

1. Guide the user through the 2019 NCDB Call for Data export and edit process.

2. Discuss how to determine data submission completeness 3. Discuss how to review FIN/Archive FIN numbers. 4. Provide Field Index Numbers to easily find fields to correct.

Call for Data 2019 Submission Specifications for Standards 5.5 and 5.6

• Submission window opens: April 1, 2019 • Final date for initial submissions AND corrections: June 30, 2019 (closes 12:00AM,

July 1, 2019 Central time) • ALL analytic cases diagnosed 2004-2017 (Class of Case 00,10-14,20-22) on or after

the program’s reference date that were modified after 12/01/2017 through the date the submission file is created is subject to Standard 5.6

• All cases diagnosed 2004+ must pass edits • GOOD NEWS…..no new edits have been added. They have only been updated to

accommodate proper diagnosis dates. Important: NCDB has not released their edits at the time of this upgrade. They are not expected to be released until 04/01/19. Since NCDB edits are based off the standard NAACCR edits, those edits can be used until the release of the NCDB metafiles.

• IMPORTANT!!! No extensions will be given for 5.6 • Rejected cases from any diagnosis year have technically not been submitted and

must be corrected and resubmitted by 06/30/19

1. Go to Reporting, Data Exports, and NCDB Export. 2. Choose Auto Select 3. Enter the Hospital Code 4. Choose Cases Diagnosed in 2017 and prior OR the option for only cases

diagnosed in an individual year. 5. Exclude cases prior to reference year and enter year. 6. Name the Filename. 7. Choose NCDB Export. 8. Click Run.

What Happens During the Export Process?

• A list of ALL the cases will be populated to the screen which may be printed. At the very bottom of the report there will be a breakdown of the number of cases in the file by Diagnosis Year. This is valuable information for determining submission completeness.

• Review the report that appears on the screen, notice the Number of Cases that is supposed to be in the file. Make sure this number matches all the other reports. THIS IS VERY IMPORTANT!!

• The report may be printed by selecting the print button. • Clicking on the download button will download the zip file again

Creating the Files

• A zip file will be created in the download folder. Within the zip files will be individual txt files that the user will use GenEdits on and/or submit. Note: If Internet Explorer is being used, the user will have to click Save or Save As instead of the file going directly to your download folder. But either way a zip file is created.)

• Open the Zip file & copy/paste those files into another file so you can have access to them.

• If there are less than 5000 cases, only one file will be created. If there are more than 5000 cases, CRStar will create one file with all the cases, but it will also create sub-files that contain no more than 5000 cases each. The NCDB doesn’t want any more than 5000 cases in any one file submitted.

CRStar will create an Auto-population of Cases Selected. Which can used to run reports for QA as well. Note Do Not try reselecting the Auto-pops! Use them as is!) If selected, it will change the records selected to 0 and in order to get the population label or labels back, the cases will have to be exported again. In this example, the auto population is for all cases. If one year at a time is exported, a population for each will be selected.

Running Edits

Please refer to E-News titled 2018 Edit E-News for complete and detailed instructions on downloading the Genedits Plus 5 program and creating edit configurations. Note: NCDB edits expected release date 04/01/19.

Genedits Plus 5 Error Reports

After the edits have run, the report option window opens. There are various options to view the report in different formats (txt, Word, XML or CSV). The report can be displayed in a detailed format, summary only or combined. The print screen on the right, which displays date/time/version, the name of the data file, number of cases, how many edits, etc.

The Summary report, lists each edit types by name, with a total of how many errors with each.

Reading the Detailed Edit Report

Identifies the record with errors.

Identifies the name of the edit and the Accrediting body.

The number in () after the name of the field indicates the column number in the NAACCR layout.

Utilize the CRStar screens and NAACCR edits document. Look for edit 781 and 782 to see which fields need to be reviewed. Or utilize the NAACCR Data Dictionary to see a detailed description of the edit.

• After all edits are cleared, delete the old saved

export file(s), then run the export again. • Run the files through NCDB Genedits Plus AGAIN

and print a copy of the CLEAN Edit Report, file it along with the list of cases exported to the NCDB.

• Just a reminder, for commendation all cases diagnosed in 2004-2017 must be error-free at initial submission.

• IMPORTANT!! If this is your First Call for Data Submission after initial accreditation, you will need to submit all analytic cases diagnosed on or after the program’s Reference Date (not earlier than 1985) through the most recent year of the Call for Data. Please put in a support call so we can walk you through the steps to create a custom population if applicable.

• These are not the same specifications as for existing programs of newly added or updated cases since the last call for data

• New programs need to submit all cases from the reference year forward.

Do a List Pop of the cases being sent to NCDB and review various fields to make sure there is not an overuse of unknown and generic codes. (Refer to NCDB Completeness and Default Overuse Reports-Recommend that registries identify data items for which defaulting exceeded benchmarks and review abstracting processes.)

Run Clinical Studies Export for 2017 and clean those up.

Determining Submission Completeness

• How is “all required cases” or data submission completeness determined? • Beginning with the 2012 call for data, the NCDB Call for Data submission

specifications were revised to require submission of the full analytic caseload for the most current diagnosis year, as well as any cases that have been added or updated since the close of the last year’s Call for Data

• A case is to be submitted if it has been updated in any manner (treatment, tumor characteristics, follow up, recurrence, or vital status)

• Hospital registry software automatically identifies cases that have been updated by looking to a data item called “Date Case Last Updated” (system-generated date item that is updated every time a registrar updates a case)

At the end of every Export report you’ll see the list of cases in the batch broken down by diagnosis year- save this information.

Run a Cross Tabulation of Cases being Submitted by Date of Diagnosis

Use the Population Auto created during the NCDB Export.

The Date of Diagnosis Ranges will Begin with 2017 and go as far back as the Reference Year.

Access the Cross Tab report by going to Reporting, AdHoc Reports and then Cross tab.

Determining Submission Completeness

Review this report, does the number of cases submitted decrease gradually and not suddenly drop off for a year or two?

This is a good example, cases being submitted each year gradually decreasing.

Example of a WARNING SIGNAL! See the big drop off beginning in year 2016! MUST INVESTIGATE!

In this example, not enough 2014-2016 cases had follow up entered. To fix the situation, perform follow up on these years. Once you find some of those cases, the date last updated or modified will change and the cases will be included in the NCDB export when you run it again.

• Determine the Number of 2017 cases being submitted in the 2019 Call for Data. (Review either the table that automatically was created for you at the end of the NCDB Export process or review the Cross Tab of Population being sent to NCDB determine this information.)

• Access CoC Datalinks to determine the number of 2016 cases that were submitted in Last Year’s Call for Data

• The expected number of cases for each facility is based on a percentage determined below.
