The SourcePoint Services Directory is a guide to local resources relating to the health and wellness of older adults and family caregivers. This is not intended to be a comprehensive directory of all community organizations and resources in or near Delaware County. Instead, the book’s focus is on the aging-related services most often used by adults over the age of 55.

Within the directory, you’ll find 13 sections. Many sections contain subcategories to narrow your search, and in each section, you’ll find brief descriptions of the content.

Because information in this directory may change, we encourage you to contact us should you have questions about the organizations or services listed in this directory. Our information and referral specialists, available by calling 740-363-6677, can help point you in the right direction. ABOUT SOURCEPOINTSourcePoint is Delaware County’s primary resource for professional expertise, services, and programs for adults who want to thrive after 55. Whether you’re 55 or 95, SourcePoint can help you set your own course to live well as you age.

We are home to a 44,000-square-foot enrichment center, open to all Delaware County residents age 55 and older, that hosts a variety of community programs, including fitness classes, educational programs, art classes, social gatherings, special events, support groups, and more.

SourcePoint also is the county’s premier aging services agency,

providing quality in-home care services to help you live safely at home. Services include Meals on Wheels, help with home chores, transportation to medical appointments, emergency response systems, care management, and more. If we are unable to provide direct access to a service, our information and referral specialists can point you in the right direction with referrals to other community resources.

SourcePoint is overseen by a volunteer board of directors appointed to reflect the diversity of the county. Our primary financial support comes from a local senior services property tax levy, which has been approved by voters since 1994. Additional support is received through corporate and private donations, sponsorships, grants, the Central Ohio Area Agency on Aging, and the work of hundreds of volunteers. SourcePoint is a United Way agency.

SourcePoint proudly serves all of Delaware County, including Ashley, Delaware, Galena, Kilbourne, Lewis Center, Ostrander, Powell, Radnor, Shawnee Hills, and Sunbury, as well as portions of Columbus, Dublin, and Westerville within county lines.

INFORMATION AND REFERRAL SERVICESSourcePoint’s information and referral specialists are available to answer aging-related questions and link you to community resources that best meet your individual needs.

IN-HOME CARE SERVICESSourcePoint’s in-home care services are designed to help adults live safely in their own homes with independence and dignity. We provide direct access to in-home services, as well as referrals to community resources and answers to your aging-related questions.

SourcePoint provides a variety of services for Delaware County residents age 55 and older, including:

Adult Day CareAdult day care provides safety, socialization, and support to elderly, chronically ill, and disabled adults, while allowing them to return home at the end of the day.

Care ManagementOur licensed social workers work with each client to customize a care plan of needed services. These care consultants handle all the service arrangements, including coordinating appointments and customer service with providers.

Emergency Response SystemsA small help button, typically worn around the neck or on the wrist, provides a direct link to 24-hour emergency assistance.

Homemaker ServicesFor those unable to perform household tasks themselves, services may include light cleaning, cooking, and laundry. Homemaker escorts also are available to accompany you on local errands.

Meals on WheelsOur Meals on Wheels program provides hot, delicious, nutritious meals delivered by a caring volunteer. In addition to providing good nutrition, a home-delivered meal provides family the security of knowing someone is regularly checking on their loved one.

Medical TransportationTransportation to medical appointments, social service agencies, and adult day care centers can be provided.

Mental Health CounselingCounseling is available for individuals enrolled in our in-home care program.

Nursing ServicesRegistered nurses are available to help you achieve optimal health and well-being through health screenings, medication monitoring, and more.

Personal CarePersonal care assistants or home health aides help with daily activities, such as bathing and dressing.

Respite CareRespite services are designed to provide relief and rest for family caregivers, so that stress and tension is minimized. Services can be provided in the home or at a local facility. Respite workers also can perform personal care tasks, such as help with bathing, grooming, and dressing.

SourcePoint often can provide direct access to additional services, such as:

Chore ServicesHelp with a variety of chores, such as changing light bulbs, tacking down cords or rugs, raking leaves, shoveling snow, cleaning out garages or basements, and more.

Home Energy Assistance Programs (HEAP)The Ohio Department of Development offers several programs to help lower-income residents pay utility bills and improve the heating efficiency of their homes.

FIRST SERVICE COORDINATORSSourcePoint partners with local fire departments through its First In Response Service Team (FIRST). SourcePoint places service coordinators at select fire stations, where they work closely with first responders and local senior living communities. Service coordinators can link individuals to helpful resources and perform home safety checks, evaluating residences to help reduce the risk of falls.

SourcePoint’s service coordinators work in collaboration with Delaware’s fire and police departments. If you or someone you love could benefit from a home safety check, call 740-363-6677.

COMMUNITY PROGRAMSSourcePoint is proud to offer a variety of programs to the Delaware County community, promoting healthy aging for everyone. From local cafes and caregiver support groups to Medicare classes and a thriving enrichment center, SourcePoint is our community’s premier aging resource.

NUTRITION PROGRAMSourcePoint’s nutrition program provides services, such as:

CafesSourcePoint operates several cafes in Delaware County. These social dining centers generally serve hot lunches on a donation basis throughout the week. Older adults can enjoy a hot, nutritious meal while socializing with friends, both old and new.

Farmers Market VouchersEach summer, SourcePoint provides vouchers for fresh foods from select local farmers markets. The vouchers can be used to purchase fresh fruits and veggies, herbs, honey, and even food-bearing plants.

Free GroceriesTo help combat senior hunger in the county, SourcePoint partners with the Mid-Ohio Foodbank to provide free monthly groceries to qualifying residents age 60 and older.

Meals on WheelsSourcePoint’s Meals on Wheels program provides hot, delicious, nutritious meals delivered by a caring volunteer. In addition to providing good nutrition, a home-delivered meal also provides family members the security of knowing someone is regularly checking on their loved one. We also

offer special therapeutic meals with restricted carbohydrates or low sodium.

SourcePoint’s Meals on Wheels program and dining centers are funded in part by the Central Ohio Area Agency on Aging.

FAMILY CAREGIVER SUPPORTAccording to the National Alliance for Caregiving and AARP, more than 34 million Americans provided unpaid care to an adult age 50 or older in the past year. These informal caregivers—family, friends, and neighbors—share the responsibility, reward, and challenge of providing care and comfort to their older loved ones.

Being a caregiver for a partner, parent, or loved one takes a great deal of time, effort, and hard work. But you don’t have to do it alone. SourcePoint helps care for the caregiver by providing resources and support, including:

Caregiver CoalitionThis committee of volunteers creates a connected community of caregivers and helps provide feedback about which local programs and resources are most beneficial. Coalition members assist with setting the direction of SourcePoint’s caregiver program by providing feedback about current programs, as well as suggestions for new ones.

Caregiver Support GroupsFree caregiver support groups are held each month and are led by trained staff. Local family caregivers are invited to join a group to share experiences, meet other caregivers, discover new resources, and ask questions. Support groups are available at SourcePoint and in various communities throughout the county.

Family Caregiver ForumAn online forum for caregivers to provide and receive support, seek comfort, discover resources, and connect with professionals and other caregivers in the community.

One-on-One ConsultationsIn addition to the group support provided in various programs, local caregivers also can seek guidance and support from our professional caregiver program coordinator.

Powerful Tools for CaregiversThis is a series of six workshops that helps caregivers develop tools and strategies to face the challenges of caregiving.

SourcePoint’s caregiver programs are funded in part by the Central Ohio Area Agency on Aging.

INSURANCE EDUCATIONOur insurance specialists, certified by the Ohio Senior Health Insurance Information Program (OSHIIP), can answer questions and help resolve problems with complicated and confusing medical billing and insurance issues. In addition to private counseling sessions, free New to Medicare classes and other workshops are available to Delaware County residents who are turning 65 years old or are eligible for Medicare due to disability.

ENRICHMENT CENTERSourcePoint’s enrichment center is designed for adults age 55 and older to engage in health and wellness activities, educational opportunities, creativity, and recreation among friends.

The 44,000-square-foot center features a fitness room, warm-water exercise pool, computer lab and library, dance and aerobics room, art room, music room, billiards room, and many other flexible spaces for group activities. The center also houses Cafe 55, one of SourcePoint’s social dining centers located throughout Delaware County.

There are nearly 200 programs scheduled each session throughout eight major categories: aquatics, arts, community, education, fitness, social, trips, and wellness. Activities are scheduled in four quarterly sessions each year. Some activities are fee-based, while others are offered at no additional cost.

SourcePoint also is a participant in the SilverSneakers®, Silver&Fit®, and Optum® Fitness Advantage programs.

Financial assistance is available to Delaware County residents who meet certain eligibility requirements. Assistance may be used to cover the cost of a SourcePoint membership, as well as class, program, or activity fees.

VOLUNTEER OPPORTUNITIESSourcePoint was founded by volunteers, and they continue to remain a vital key to our success. We depend on the energy and skills of our volunteers to fulfill our mission and serve the public. We welcome individuals, couples, and groups for short- or long-term volunteer experiences.

We offer a variety of opportunities, including the chance to:

• Serve as a Meals on Wheels driver or assistant, or help with food preparation.

• Transport clients to medical appointments.

• Distribute brochures and educational materials to local businesses and senior living communities.

• Organize a group to assist older adults with exterior home chores.

• Staff health fairs and special events.

• Contribute to our enrichment center by teaching a class, giving tours, or assisting in the fitness center.

• Become certified to assist with Medicare enrollment.

According to the National Alliance for Caregiving and AARP, about 44 million Americans provide 37 billion hours of unpaid, care each year for family members and friends. The loved ones receiving care often have chronic illnesses or conditions that prevent them from handling daily activities, such as bathing, managing medications, or preparing meals on their own. Family caregivers, particularly women, provide more than 75 percent of caregiving support.

Because of the multifaceted role that family caregivers play, they deserve reliable resources and support that allow them to care for their loved ones and themselves. The resources in this section, combined with other services in this directory, can help provide support, knowledge, and relief.
