listed under Priority 7 and the

Eligibility

Community-based organizations, healthcare systems, health departments andother health-related agencies and non-profit organizations that are based inWisconsin, serve Wisconsinites and workon cancer-related issues are eligible toapply

You do NOT need to be a member of theWI Cancer Council to apply

Requires collaboration

All activities must be alcohol andtobacco free

Awards & Funding

3-5 awards

$12,000-$20,000/each over the grant period

(November 26, 2018 – June 28, 2019)

Funding Restrictions:

• May not replace dollars currently earmarked for cancer programs/projects

• Administrative fee may not exceed 10% of total budget

• May not be used for clinical or direct patient services, research, lobbying, purchasing equipment, general office supplies, computers and software, phone bills, rent, utilities, alcohol, cash prizes or gift certificates

• By June 2019, increase the number of Hmong patients who have been screened for colorectal cancer at the HealthyLife clinic from 10 in first half of 2018 to 30 in first half of 2019.
