HHig h De du ctib le Health Plan (HDH P) – AH N Base Plan

The HDHP; also referred to as Consumer Driven Health Plan (CDHP) has a higher deductible ($2,600 for an individual and $5,000 for a family). Except for preventative care services, which are paid at

401(k) Retirement Savings Plan

Vest ingBeginning January 1, 2004, employees are 100% vested in the contributions they choose to defer and the matching contributions made by American Health Network. These contributions cannot be forfeited.

Hardship withdrawals may be allowed prior to age 59 1/2 (if actively employed), but are limited to one per year, subject to penalty and require suspension of participation in the Plan for a 6-month period.
