WHAT: Free Dental Care

WHEN: February 5, 2016

WHERE: Your Local Dental Offices

WHO IS ELIGIBLE?

Children who DO NOT have dental insurance, CHIP or current Idaho Smiles (Medicaid) card

Children ages 3 to 18 years.

Children must have a parent or legal guardian accompany them to the dental office to allow consent for service and provide current medical history.

Meet the family income guidelines provided below

Appointments are made on a first call, first serve basis -it may not be possible for all participants to have all needed

treatment completed on this day. Services provided may be preventive or restorative. Please schedule your child as soon as possible!
