ACADEMIC ELIGIBILITY REQUIREMENTS

o Students must pass 5 classes for the 1st semester.

o Students must have a 2.0 GPA for the 1st semester.

o Waivers: are only available for the 2.0 GPA

requirement not for the pass 5 classes requirement.

ELIGIBILITY REQUIREMENTS

oDate of Birth: a student cannot reach his/her 15th birthday on or before August 1st of the current school year.

oStudents are only eligible for 6 consecutive semesters, with the 1st semester beginning when the child enters the 6th grade.

oTransfer Rule: 6 or 8 games

o$250 fine for giving false information regarding where you live.

INSURANCE VBCPS offers a supplemental insurance to your

current provider. Notify coach immediately if athlete is being seen by a

doctor or emergency room due to accident during practice or game.

Claim forms may be issued by the SAC. Treatment for any injury must be received and the

claim form filed within 90 days of the date of the injury.

Plan provides 70% of Usual & Customary coverage. $25,000 maximum benefit.

The deductible is $100 per injury. There is a $500 limit on physical therapy claims per

injury.

PLAY-OFFS/CHAMPIONSHIP GAMES:• $4/adults; $2/students

Beach District Family Passes are not accepted at MSL events.

PARENTS/SPECTATORSThe following are required of all parents/spectators:

1. Realize you represent Plaza, so encourage through positive behavior.

2. Being a good sport is more important than victory.3. Recognize and show appreciation for an outstanding

play by both teams.4. Be a “team fan” as well as a “my kid” fan.5. Remember that a ticket to a school event is a privilege

to observe the contest.6. Treat visiting teams and officials as guests.7. Be modest in victory and gracious in defeat.8. Respect the judgment and integrity of officials,

realizing that their decisions are based upon game conditions.

9. Bottom line… be a positive role model

IMPORTANT INFORMATION

DISCIPLINE:O 1st ISS: team member will not participate the day

ISS is served and may not start the next competition.

O 2nd ISS: team member will be removed from the team.

O OSS: team member will be removed from the team.

GAMES:O We will provide transportation to away games.O We ask that you allow your child to ride the bus

back to Plaza from away sites.O Only a parent/guardian may take a child from an

away site.

IMPORTANT INFORMATION

Cancellations of Games/Practices:• Made after 2:00 pm. Please refrain from

calling after the first rain drop/snow flake. • There are few dates for make ups, so if at

all possible we will play.• Be sure coaches have your email address.

Uniforms:• Responsibility of team members as soon as

they take possession.• Please help ensure we receive them back

in the same condition they were issued.

CHKD ATHLETIC TRAINER

Christine Moore On site approximately every other day.

Inhalers/epi pens must be given to coach for the first aid kit.

Self carry forms must be completed for inhalers & epi-pens.

Emergency Cards will travel with team and should be completed this evening.

CONCUSSION INFORMATIONPlease be available by phone while

your child is practicing or competing.If your child is suspected of having a

concussion during practice or a game and you are not present, the trainer or coach will call you. It is mandatory that you pick your child up. They will NOT be put on the bus.

You will receive home instructions from the person signing your child over to you.

MORE CONCUSSION FACTS

Your child will begin the Graduated Return to Play progression only after they are symptom free FOR 24 HOURS.

There are 5 stages and each stage last a minimum of 24 hours.

If at any time during the stage process they experience any symptoms, the process stops for 24 hours and they start with the last stage where they had no symptoms.

OIf the original symptoms resolve within 15 minutes of the incident AND the student has experienced NO other symptoms overnight, the student is cleared to return to play at the discretion of the ATC.

If symptoms last longer than 15 min

and/or return overnight: The student MUST see a physician.

There is a specific form that must be completed by the physician in order for the student to be considered “cleared to start the 5 day progression.”

This form can be obtained from the school ATC or SAC.

REMEMBERO These guidelines and activity restrictions are

for any and all physical activity to include but not be limited to physical education classes, rec. games and practices, club games and practices.

O Rules are in place to protect the student athlete.

O As a parent, help the student/athlete take time to get better.

O Your support, as parents, is vital to the success of the program and safety of the students.
